We investigated the role of genitourinary tract infections, structural and psychologic abnormalities in 75 men referred to a "chronic prostatitis" clinic. Patients had had symptoms for an average of eighty-three months and had received an average of ten weeks of antimicrobial treatment during the three months prior to evaluation. Specific infectious diseases, structural or functional causes were identified in 34 (45%) of the 75 patients. Genitourinary tract pathogens were isolated from 26 patients and inflammation was documented in urethral or prostatic secretions of 24 patients. Structural or functional abnormalities that merited treatment were identified in 8 cases. In addition, major criteria for
depression
were documented in 9 of 17 patients who agreed to this evaluation. Overall, a comprehensive approach to diagnosis led to specific treatment for 37 (49%) of the 75 men with chronic
prostatitis
, despite longstanding symptoms and failure of multiple prior courses of therapy.

Personality testing was performed on 56 patients with clinical symptoms of chronic

. The psychological methods used were RO, TAT, FAM and Beck's

Q factor analysis of the patients was performed and yielded four factors representing four different personality pathologies, i.e. psychosomatic structure, alexithymic personality, borderline personality and narcissistic personality. . The patients were characterized by prolonged symptoms and poor response to treatment by general practitioners. Some degree of psychic difficulty was revealed in 80% of the patients, and signs of severe psychic disturbance in 20 to 50%. The outstanding psychic symptoms were anxiety,

, affect-lability, weak masculine identity and other sexual disorders, and even psychotic features. This pattern of psychic disorders corresponds with that described for borderline personality. patients was compared with a group of 50 patients seen for a vasectomy. Psychologic measures were taken by means of a personality inventory (NVM, Dutch short form of the MMPI), a symptom checklist (SLC-90), and a

inventory (IDD). Results showed statistically significant differences between the groups, with the chronic

patients scoring consistently higher on the measures than vasectomy patients. However, these differences in scores were not of a great magnitude and minor compared with differences in scores from psychiatric patients. Discriminant analysis suggested somatization and

to be the key variables to distinguish chronic

patients from vasectomy patients. Overall, it seemed unfounded to label chronic

Additionally, psychosomatic factors have often been neglected. A number of personality variables have turned out to play an important role in questionnaire-based studies, namely somatization and

, as well as anxiety, hypochondriasis, and weak masculine identity. However, these preliminary results need further evaluation. In the future, diagnostic and therapeutic strategies should pay attention to the complex interweavement of somatic and psychogenic factors in the pathogenesis of CP/CPSS.

A prospective multi-center Italian urology outpatient survey, undertaken to determine the prevalence of
prostatitis
in Italy, provided an ideal opportunity to identify and characterize the typical
prostatitis
patient presenting to Italian urologists. A total of 70 urologists, representing a cross section of urologic centers in Italy, counted and recorded the overall total male patients reported in the clinic and the overall total patients diagnosed with
prostatitis
over a 5 week period in late 2001. Data on demographics, previous diagnoses, symptoms (frequency and severity), quality of life, physical examination and laboratory data were prospectively collected. A total of 1,148 patients were identified with
prostatitis
(12.8%). Of these, 1,074 patients had complete data and could be included in this characterization analysis. The mean age of the
prostatitis
patients was 47.1 years (range 16-83); two thirds had experienced their first symptom within the last year. A family history of
prostatitis
was reported by 20.4%. The most common urinary diseases were benign prostatic hyperplasia (17.4%), recurrent urinary tract infection (11.2%) and urinary calculogenesis (11.1%), while the most common concurrent diseases were diabetes (7.2%) and
depression
(6.8%). The most frequently reported and most severe symptoms at the time of evaluation were irritative voiding symptoms, perineal and suprapubic pain and discomfort. Over three quarters of the patients were dissatisfied with their quality of life. Bacteria were cultured in 15.6%, 17.7% and 14.0% of expressed prostatic secretions, urine specimens after prostatic massage and semen specimens, respectively.
