To determine the validity of the DSM-IIIR category organic mood disorder, we compared 50 psychiatric consultations with this diagnosis to 50 psychiatric consultations diagnosed with major depression in the medically ill. Organic mood disorder patients were more likely to be in the index affective disorder episode and have a negative family history of depression. Despite similar pharmacologic treatment between groups, the organic mood disorder group was less likely to be completely recovered at 4 years follow-up. This study suggests organic mood disorder is a valid diagnosis in the psychiatry consultation service.

Assessment of the relationship of depression to chronic fatigue syndrome (CFS) is a complicated but important topic. This relationship may range from the misdiagnostic (i.e., depression misidentified as CFS) to the etiologic (i.e., CFS causes an organic affective syndrome). Assessment should focus on the symptoms and syndromes of depressive disorder, utilization of a single rating scale to assess presumed depression is discouraged, and alternate approaches to classification that allow for symptomatic overlap of a major depressive disorder and CFS are suggested. Careful attention needs to be given to the use of external validating criteria in empiric studies, such as natural history, clinical course (including treatment response), and family history.

Thirty two cases of chronic renal failure with Continuous Ambulatory Peritoneal Dialysis (CAPD) hospitalized from November, 1984 to July, 1986 were investigated by interview, Hamilton Depression Scale (HAMD) and Symptom Check List (SCL-90). Results revealed that the occurrence of depression among dialyzed patients was 56.3%, i.e. 18 patients with high total score of HAMD belonged to organic affective syndrome defined by DSM-III. Causes of developing depression during CAPD were evaluated and analyzed. Authors consider that depression should be viewed as interaction of genetic, psychosocial and neurobiological variables.

40 cases of breast tumor (cancer group 20 cases, benign tumor group 20 cases) hospitalized from October, 1986 to April, 1987 were investigated by interview, HAMD, SCL-90 and so on. Results showed that 10 cases met diagnostic criteria of organic affective syndrome defined by DSM-III. The morbidity of depression among all cases was 25 per cent, but the morbidity of depression in breast cancer group was higher than in benign tumor group. Causes of depression in breast cancer were analysed. Authors consider that causes of depression are not relating to operation on breast, but probably relating to biological and psychosocial factors.

Ten (17%) of 58 patients with chronic viral hepatitis treated with a four- to 12-month course of recombinant human interferon alfa developed psychiatric side effects. The psychiatric side effects fell into three categories: an organic personality syndrome characterized by irritability and short temper; an organic affective syndrome marked by extreme emotional lability, depression, and tearfulness; and a delirium marked by clouding of consciousness, agitation, paranoia, and suicidal potential. These psychiatric side effects appeared after one to three months of therapy, usually improved within three to four days of decreasing the dose of interferon alfa, and invariably resolved once therapy was stopped. The organic personality and affective syndromes tended to occur in patients who received the highest dose of interferon alfa, who had relatively mild hepatitis, and who lost weight during interferon treatment. Delirium tended to occur in patients with severe hepatitis who had previous evidence of organic brain injury or dysfunction or previous drug and alcohol abuse. Failure to recognize these side effects quickly and to treat them with supportive therapy and modification of the dose of interferon alfa could result in limitation of therapy and serious personal and interpersonal consequences.

There is thought to be a high incidence of depression in intensive care units (ICUs), both as a result of the psychological toll of serious illness and threat to life as well as the physiologic derangements caused by illnesses and treatments. The authors outline a strategy to classify ICU depressions into major depressive illness, adjustment disorders, dysthymic disorder, or organic affective syndrome. Other psychiatric disturbances which may simulate depression are also discussed. A summary of environmental, behavioral, psychotherapeutic, and psychopharmacologic treatments for these disorders and their special application in the ICU setting is presented. The limitations and precautions with antidepressant drugs in this clinical site are reviewed.

Previous epidemiological and clinical studies of humans exposed to polychlorinated biphenyls (PCBs) indicate that the majority of patients have neurological complaints (e.g., headache, vertigo, paresthesias, poor memory and concentration, fatigue, depression). Since only a small minority of PCB-exposed patients demonstrate abnormalities on objective neurological measures (e.g., CT-scans, EEC, nerve conduction velocity), it is particularly unfortunate that objective neuropsychological data has not been published to substantiate patient complaints. The present study provides neuropsychological test data on two patients exposed to PCBs. In both cases, PCB exposure is documented by an analysis of PCB levels in the patients' work environments. Despite the absence of abnormalities on CT-scans and EEC, both patients displayed a variety of cognitive deficits and emotional disturbance. Serial assessment of one patient with high blood levels of PCBs revealed a dementia (sharing certain features with Alzheimer's disease) and an organic affective syndrome. Assessment of a second patient exposed to PCBs (but with no detectable blood levels of PCBs) suggested that his cognitive impairments were not due to PCB exposure. The present study provides data which points to the importance and sensitivity of neuropsychological examination in cases of PCB-exposure.
