This study was carried out with 48 patients received surgery, i.e., 23
stomach cancer
, 8 colon cancer, 6 rectal cancer, 9 breast cancer etc. Patients in group A received UFT in combination with OK-432. Each of UFT or OK-432 was given to the patients in groups B or C, respectively. Changes in the skin reaction to Su-PS were measured before and after dosing, and concentrations of Tegafur and 5-FU in serum and tumor tissues were determined after administration. Analysis of the skin reaction to Su-Ps revealed that patients with positive skin reaction before surgery in group A didn't manifest
depression
due to sensitization by UFT therapy. Although average values of the skin reaction after dosing were slightly lower compared to those before dosing in group B, sensitization was effective. Values of the skin reaction after dosing were significantly (p less than 0.05) high compared to those before dosing in groups A and C. Concentrations of Tegafur and 5-FU in serum reached to the peak 2 hr later and were maintained high enough to expect clinical responses even at 4 hr after administration in groups A and B. Especially there was not a significant difference between groups A and B in tumor tissue levels of 5-FU, and a high effective concentration was obtained. Combination therapy of UFT with OK-432 exhibited no significant interaction between them in adjuvant immuno-chemotherapy, and satisfactory results were expected in clinical cures.

Patients with cancer of the upper gastrointestinal tract such as the esophagus and stomach are malnourished as a result of their cancer-bearing status. In such patients, malnutrition reduces the effects of anti-cancer therapy including surgical treatment. Therefore, we studied the effect of nutritional support in patients with upper gastrointestinal cancer. The incidence of postoperative complications was correlated with the preoperative nutritional status. Thus, improvement of nutritional status by preoperative nutritional support is thought to be important. As metabolic disturbances frequently accompany advanced cancer, the quantity of calories and quality of nutritional regimen were found to be important for improving such metabolic disorders. In the postoperative period, total parenteral nutrition (TPN) prevented the

of cell-mediated immunity, increased the tolerance to anti-cancer drugs during chemotherapy as an adjunct to surgery, and prolonged the disease-free interval, in patients undergoing absolute non-curative gastrectomy for advanced

. The results of chemotherapy were better in recurrent upper gastrointestinal cancer patients whose nutritional parameters were in a favorable range on admission or improved after 2 weeks of TPN.

We studied the effect of vitamin B complex (vitamin B1, B6 and B12 complex) on the immune responsiveness in
gastric cancer
patients who underwent surgery. The
depression
of blastogenic responses to both PHA and PWM was observed 2 weeks after surgery in half of the patients treated with Vitamedin but the degree was significantly less than that in the control patients without vitamin B treatment whose lymphocyte responses were depressed. Moreover, the blastogenic responses were induced by vitamin B administration 2 or 4 weeks after surgery in 5 of the 8 stage III-IV patients whose lymphocytes had not responded prior to surgery. Four weeks after surgery, the patients without vitamin B treatment showed only a tendency of recovery of their lymphocyte responses, whereas the recovery of blastogenic responses in the patients treated with vitamin B was significant. Essentially similar results were obtained with skin reactions to PHA and PPD. These results suggest that the administration of vitamin B1, B6 and B12 complex is useful for the protection against and the recovery of immune dysfunction produced by surgery in cancer patients.

Standard immunological parameters measuring non-specific cellular immune reactivity were determined in 175 patients with different stages of
gastric cancer
prior to surgery and during follow-up. Several tests measuring monocyte activity were also employed. The total number of T cells and their subpopulations Ta and T29o was unchanged except
depression
of T29o in stage IV. The blastogenic response of lymphocytes to PHA as assessed by stimulation of protein synthesis was only depressed in stage IV. In contrast the PHA-induced lymphokine production was increased in all patients but the differences were significant for stage III and IV. Monocyte Fc receptor expression was increased in stages II-IV, while nitro blue tetrazolium reduction and antibody dependent cellular cytotoxicity of monocytes was elevated in stage IV. The number of extractable monocytes was not increased. Longitudinal studies suggested that most of the parameters normalized during follow-up. No major long-term impact of chemoimmunotherapy (5-FU + BCG) on the immune parameters was observed except a transient increase in PPD reactivity approximately 1 year after commencement of treatment. In patients with stage III gastric cancer the increased occurrence of suppressor cells (mostly monocytes) and elevated cytostatic activity of monocytes was associated with a longer survival while the increased lymphokine production and Fc receptor expression were seen in the group of patients succumbing earlier. We concluded that most of the changes in immune parameters were seen only in advanced disease and paradoxically disappeared in the course of disease. The determination of monocyte activity seems to be a sensitive indicator of immune system dearrangements in earlier stages of cancer and a useful prognostic factor in

The immunosuppressive effect of chemotherapy associated with surgery is poorly understood. The effect of combination chemotherapy--fluorouracil, adriamycin and mitomycin (FAM)--on monocyte phagocytosis was tested in patients operated on for
gastric cancer
. The adherence, engulfment and total phagocytic process were studied with a fluorescence-quenching technique 1 hour, 24 hours and 1 week after 19 FAM treatments in nine patients. The engulfment of yeast cells and the total phagocytosis were significantly impaired 1 hour post-treatment (p less than 0.05 and less than 0.01, respectively). These functions had normalized after 24 hours, but after 1 week the engulfment step was significantly depressed (p less than 0.05). Significantly increased adherence of yeast cells (p less than 0.05) was found at the same time. The initial, transient
depression
of monocyte phagocytosis probably was related to inactivation of cell-surface receptors. The late (1 week)
depression
probably was due to bone-marrow toxicity. This functional impairment of the blood monocytes was not reflected as clinical liability to infections.

gastric cancer
patients, as well as to determine the correlation of PMN function with extent of disease and the degree of differentiation of the lesions. Furthermore, the effects of surgery and administration of Mitomycin C (MMC) on PMN functions were studied. In early
gastric cancer
and Stage I disease, the mean values of chemotaxis and CL were elevated in comparison to controls. As the disease progressed, the PMN function decreased and significant
depression
was noted in Stage IV cases. With respect to the relationship of PMN functions to the grade of differentiation of cancer, patients with papillary adenocarcinoma had significantly elevated mean chemotaxis in response to one of the chemoattractants, while patients with poorly differentiated and signet ring cell carcinomas had significantly lower chemotactic responses to chemoattractants compared to controls. There were no significant differences in CL according to the degree of differentiation of histologic type. Surgical removal of the tumor resulted in a tendency towards improvement of PMN chemotaxis while postoperative administration of MMC resulted in a decrease, but there were no statistically significant differences between responses before and after surgery. No improvement of the CL response was demonstrated approximately four weeks after surgery.

The association of suppressor cells with survival of patients with
gastric cancer
was investigated. Phytohemagglutinin (PHA)-induced lymphocyte response and the presence of nonspecific suppressor cells were assessed in patients with different stages of
gastric cancer
. The presence of suppressor cells was determined by their ability to inhibit the PHA response of normal peripheral blood mononuclear leukocytes.
Depression
of the PHA response was related to the stage of disease and was also associated with the presence of suppressor cells. Of 245 patients tested, 76 (31%) had suppressor cells. Adherent, nonspecific esterase-positive cells (presumably, monocytes) accounted for the suppression in most cancer patients. The occurrence of suppressor cells and the tumor load were related because the incidence of detectable suppressor cells decreased after surgery in patients with resectable tumor but increased in patients undergoing palliative surgery. In patients with advanced disease who had a generally poor prognosis, the occurrence of suppressor cells was associated with a significantly increased survival. Hence the common view that a depressed lymphocyte response correlates with a poor clinical outcome may not be valid in all types of cancer.
