A case of hypothyroidism that manifested as
depression
and deteriorating functional status and that ultimately resulted in the death of a 41-year-old patient with AIDS is described. Postmortem examination revealed destruction of the thyroid gland by Kaposi's sarcoma. Analysis of stored serum samples revealed that the patient had become profoundly hypothyroid during his terminal illness. Practitioners are reminded of the need to exclude metabolic causes when treating encephalopathy in patients infected with human
immunodeficiency
virus.

immunodeficiency
virus (HIV)-seropositive pregnant patients, a history of drug abuse was significantly associated with increased psychopathology, particularly
depression
and personality disorders. Psychiatric disorders preceded the HIV diagnosis in most of the patients studied. Intravenous drug abuse was also associated with increased prostitution, incarceration, and previous suicide attempts. Psychiatric findings were influenced much less by the HIV diagnosis and much more by the presence or absence of a history of drug abuse.

Fifty-six gay and bisexual men were tested for antibody to the human
immunodeficiency
virus. Twenty-two subjects who tested positive, 22 subjects who tested negative, and 12 subjects who chose not to learn the test results were surveyed by questionnaire after one week and after about six months after testing. Subjects who tested positive and those who tested negative were also compared two weeks after learning results. Subjects who tested positive experienced an increase in anxiety,
depression
, and AIDS anxiety, and subjects who tested negative experienced a decrease in these feelings after learning results. Subjects who did not learn the results of their tests experienced no change in these feelings. All three groups altered their sexual behavior. Subjects were more likely to tell test results to lovers and to regular sexual partners than to casual sexual partners. Implications for social work practice are discussed.

Of 4,073 reported patients diagnosed with AIDS from 1978 through 1983, 821 (20%) were not reported to be dead by January 1987. Of these apparent long-term survivors, 780 (95%) were reported from 14 states or from local areas where collaborating health departments conducted special follow-up investigations: 119 (15%) were found to be alive, 475 (61%) were dead, and 186 (24%) were lost to follow-up. Health departments obtained consent to collect additional clinical and laboratory information on 48 of the living patients. Six (13%) had no laboratory evidence specific for human
immunodeficiency
virus (HIV) infection (antibody, antigen, viral isolation, or polymerase chain reaction assay); 41 (85%) had a positive result on at least one test; and one was not tested. Of the 41 infected patients, 25 (61%) had Kaposi's sarcoma (KS) and two (5%) had Pneumocystis carinii pneumonia as the only AIDS-indicative disease; the remainder had multiple diseases. CD4+ cell counts were low (less than 30% of total T lymphocytes) by the time of enrollment in 34 (87%) of 39 patients tested. When enrolled survivors with KS were compared with KS patients who had died within 2 years after AIDS diagnosis, survivors were less likely to have had other diseases in addition to KS than were nonsurvivors (31% versus 51%). While overall mortality by 1987 for patients diagnosed in 1978-83 was high (92-96%), a small number have survived and were doing relatively well clinically, despite evidence of continued CD4+ cell

We investigated the ability of peripheral blood mononuclear cells (PBMC) from human
immunodeficiency
virus (HIV)-seropositive asymptomatic and mildly symptomatic homosexual men with known time of seroconversion to mediate antibody-dependent cellular cytotoxicity (ADCC) specific for HIV. PBMC from HIV-seronegative and -sero-positive subjects lysed T (CEM) cells persistently infected with HIV to a significantly greater degree than uninfected CEM cells in the presence of HIV antibody-positive serum in a 4-hr 51Cr release assay. The response was mediated by CD16+ cells. ADCC responses were lower in PBMC of 13 men tested 9 to 25 months (average, 16.1 months) after seroconversion to HIV as compared with seronegative subjects, and were further decreased in 11 men tested 26 to 38 months (average, 31.6 months) after seroconversion. Decreases in numbers of circulating CD16+ cells appeared to contribute to
depression
in ADCC activity. The suppressive effect of HIV infection on ADCC effector cell activity may be important in the immunopathogenesis of acquired immune deficiency syndrome.

Preexistent feline leukemia virus (FeLV) infection greatly potentiated the severity of the transient primary and chronic secondary stages of feline
immunodeficiency
virus (FIV) infection. Of 10 FeLV-FIV carrier cats, 5 died of experimentally induced FIV infection, compared with 2 deaths in 10 cats infected only with FeLV and 1 death in 7 cats infected only with FIV. FIV-infected cats with preexistent FeLV infections developed severe
depression
, anorexia, fever, diarrhea, dehydration, weight loss, and leukopenia 4 to 6 weeks after infection and were moribund within 2 weeks of the onset of signs, whereas cats infected only with FIV developed much milder self-limiting gross and hematologic abnormalities. Pathologic findings in dually infected cats that died were similar to those observed previously in cats dying from uncomplicated primary FIV infection but were much more widespread and severe. Coinfection of asymptomatic FeLV carrier cats with FIV did not increase the levels of FeLV p27 antigen present in their blood over that seen in cats infected with FeLV alone. The amount of proviral FIV DNA was much higher, however, in dually infected cats than in cats infected only with FIV; there was a greater expression of FIV DNA in lymphoid tissues, where the genome was normally detected, and in nonlymphoid tissues, where FIV DNA was not usually found. Dually infedted cats that recovered from the primary stage of FIV infection remained more leukopenic than cats infected with FIV or FeLV alone, and their CD4+/CD8+ T-lymphocyte ratios were inverted. One of these cats developed what was considered to be an opportunistic infection. It was concluded, therefore, that a preexistent FeLV infection in some way enhanced the expression and spread of FIV in the body and increased the severity of both the resulting transient primary and chronic secondary stages of FIV infection. This study also demonstrated the usefulness of the FIV model in studying the role of incidental infectious diseases as cofactors for
immunodeficiency

To identify the diagnostic characteristics and counseling needs of individuals at risk for acquired immune deficiency syndrome (AIDS), standardized DSM-III-R diagnoses were given to 207 physically asymptomatic adults when they sought serological testing for the human
immunodeficiency
virus (HIV). The subjects had high lifetime rates of mood disorders. Even after the 20 subjects with intravenous drug use as a risk factor were eliminated, lifetime rates of nonalcohol substance dependence were also high. These findings suggest that even before they are notified of HIV test results, many individuals at perceived risk for AIDS may be vulnerable to future

Expression of tumor necrosis factor (TNF alpha), tissue factor (TF), and interleukin 1-beta (IL-1 beta) mRNA was evaluated in monocytes isolated from patients infected with human
immunodeficiency
virus (HIV). There was a significant
depression
(66%) of the induced level of TF mRNA expression in response to lipopolysaccharide. Conversely, the response of TNF alpha and IL-1 beta, following LPS induction, was "normal." TF mRNA reduction was also observed to a lesser degree in AIDS-related complex patients (20%) but not in asymptomatic seropositives. TF is necessary for initiation of the coagulation protease cascade, leading to thrombin production and fibrin deposition, which play a role in inflammatory responses. Its selective reduction may be a factor in the diminished resistance to secondary infections observed in AIDS. Further, since the TF defect increases as patients progress toward AIDS, it may serve as a marker for disease progression.
