The concept of the conventional aqueous outflow pathway (through the trabecular meshwork and other components of the canal's inner wall, along Schlemm's canal to collector channels, and out of the eye through the collector channels) was modeled on tubes with leaky walls and a network of electrical resistances. Analysis of these models suggested experiments to test the models and to assess the magnitude of the resistance of portions of the pathway. We found that the well-known increase in outflow resistance with increased intraocular pressure was, in large measure, the result of blockage of the collector channels, probably by the canal's inner wall. A model of the trabecular meshwork predicted that tension on the scleral spur and trabecular mesh would cause the meshwork to arch over Schlemm's canal. We depressed the lens to generate such tension, and found that the resistance of the inner wall of the canal decreased. In one eye, lens depression also decreased resistance to flow in the collector channels and the canal of Schlemm and prompted the suggestion that when secretory inhibitors are used in glaucoma, cyclo-tonic agents should be used simultaneously to relieve canal collapse and blockage of collector channels by inner canal wall.

Prescribing tricyclic antidepressants presents potential hazards to patients with heart disease, glaucoma, prostatic hypertrophy and epilepsy for their symptoms may be aggravated. Mianserin, on the other hand, has little effect on the heart and the parasympathetic nervous system and this drug may be used safely in these circumstances. Tricyclic antidepressants and mianserin also differ in their toxicity when taken in overdose. Poisoning with mianserin rarely causes more than drowsiness except when other drugs have been taken. In contrast overdose with tricyclic antidepressants frequently causes epileptic convulsions, arhythmias, hypotension, and anticholinergic signs. Death occurs in 2-3% of overdoses, usually due to cardiovascular collapse, respiratory depression or status epileptic's either alone or in combination.

Late-onset visual impairment due to cataracts, glaucoma, macular degeneration, or diabetic retinopathy afflicts approximately 10% of people older than 65, and often results in depression and social dysfunction. Whereas the majority of sighted older adults are active participants in their community, individuals suffering from progressively worsening vision experience heightened levels of isolation and reduced social support, and participate in fewer reinforcing recreational activities. This article describes our social skills training package for the treatment of depressed, visually impaired, older adults. Our treatment focuses on increasing the frequency and level of assertiveness with which visually impaired older adults interact with each other. In addition to specified treatment methods, our empirically derived program employs standardized assessment measures to evaluate therapeutic progress. Sessions are implemented to reinforce efforts to apply social skills in the environment and reestablish skills that have begun to fade.

It is known that the prevalence of normal tension glaucoma (NTG) or primary open angle glaucoma (POAG) is higher in the myopic population and that the prevalence of myopia and NTG is higher in Japan than in western countries. To evaluate the influence of myopic refractive error in the central 10 degrees of the visual field in NTG eyes, the data obtained from NTG eyes with the Humphrey Visual Field Analyzer were analyzed in comparison with data from POAG eyes. One hundred and twenty eyes of 86 NTG patients and 197 eyes of 138 POAG patients whose ages were 65 years or younger were included in this study. All of them had clear ocular media, refractive power less than +1.0 diopter and no myopic degeneration in the fundus. Multiple linear regression analysis was performed with the dependent variable of the deviation of the measured threshold value from the normal reference value (TD; STATPAC 2), and independent variables of myopic refractive power and the mean deviation (MD; STATPAC 2) as an index of the disease stage at each test point of the Humphrey 10-2 program and at the most central 12 test points of the 30-2 program. Myopic power was found to have a significant positive correlation with the depression in the lower cecocentral area in both NTG and POAG eyes, while only in NTG eyes it had a significant negative correlation with the depression in the upper arcuate area extending just nasal to the fixation point.

Effects of a Russian b-a-adrenoblocker proxodolol on the intraocular pressure, ocular hemodynamics, pupil diameter, ocular functions, arterial pressure, and heart rate were studied in 105 patients (163 eyes) with primary open-angle glaucoma. A manifest hypotensive effect of proxodolol was due to depression of aqueous humor production and improvement of its outflow. Comparative study of the efficacies of proxodolol and timolol maleate by the blind test and randomization demonstrated the identity of these drugs. A synergic effect on intraocular pressure was observed when proxodolol was combined with pilocarpine and/or klofelin.

Brimonidine is a relatively selective alpha-2 adrenoceptor agonist that is being developed for the treatment of glaucoma. Because brimonidine is chemically related to clonidine and has affinity for the nonadrenergic imidazoline receptor, its ocular effects may be unrelated to alpha-2 receptor activation. The objective of this study was to determine the pharmacology of the intraocular pressure (IOP) response to brimonidine in rabbits and monkeys and the side effects (miosis, cardiovascular depression) in monkeys. Conscious albino rabbits and cynomolgus monkeys were pretreated topically with the following receptor antagonists: rauwolscine (alpha-2), idazoxan (alpha-2, imidazoline), SKF 105854 (vascular postjunctional alpha-2), and prazosin (alpha-1). Intraocular pressure, pupil size, or blood pressure/heart rate was monitored noninvasively for 6 hours following dosing. Binding experiments were performed using [3H]brimonidine in membrane preparations from rabbit iris/ciliary body and from monkey cerebral cortex and brain stem. In rabbits, the ocular hypotensive response to brimonidine was unilateral and was inhibited by rauwolscine > idazoxan >> SKF 105854 = prazosin; this ranked order of potency correlated with displacement of [3H]brimonidine in the rabbit iris/ciliary body. In monkeys, brimonidine decreased IOP bilaterally and suppressed cardiovascular function suggesting a CNS site of action. Intraocular pressure and cardiovascular responses to brimonidine were inhibited by idazoxan >> rauwolscine > SKF 105854 = prazosin; a similar profile was obtained for displacement of [3H]brimonidine in monkey brain tissue. Both rauwolscine and idazoxan inhibited the miotic response to brimonidine in monkeys. Taken together, these results indicate that brimonidine stimulates an ocular alpha-2 adrenoceptor to decrease IOP in the rabbit and a CNS imidazoline receptor to decrease IOP, blood pressure, and heart rate in the cynomolgus monkey. The miotic response in the monkey is mediated by an alpha-2 adrenoceptor. The alpha-1 and vascular postjunctional alpha-2 adrenoceptors do not appear to play a role in mediating these responses.

The authors analyze the value of automated static perimetry for the early diagnosis of glaucoma. A total of 232 patients were examined, in 74 of them with primary open-angle glaucoma belonging to two age groups a reliable reduction was revealed of the mean retinal light sensitivity (p < 0.0001 vs. control and in suspected glaucoma in subgroups 1 and 2 p < 0.0001 and p < 0.01, respectively). Diffuse and local depression of light sensitivity in the central visual field was found in glaucoma patients and subjects with the condition suspected. Kinetic perimetry revealed narrowing of the peripheral borders of the visual field in 20% of glaucoma patients.

To investigate the association between depression and vision, 100 cataract operation patients (25 were men and 75 women) aged 71 to 76 years were studied. One day before the operation and 3 mo. after, the patients' depression was tested with the short form of the Beck Depression Inventory and their personalities with Mini-Mult MMPI. The cataract operation restored visual acuity sufficient for reading (minimum E-test value 0.40) to 79% of the subjects. The analysis indicated that their depression was significantly correlated with vision only after the cataract operation. Depression increased with weakened visual acuity and diminished with improved visual acuity. Part of the postoperation depression was, however, associated with glaucoma and serious somatic diseases (asthma, cerebrovascular disorders, and heart diseases).

In a comparative study, 21 in-patient and 27 out-patients with primary open-angle glaucoma and 18 cataract patients were investigated to determine whether psychiatric symptoms were more frequent in patients with primary open-angle glaucoma. In all three groups we used Beck's Depression Inventory, the Zerssen Symptom List and the Maudsley Personality Inventory. In addition, in the case of the glaucoma in patients we applied the unsteadiness Test, the Social Interview Schedule and a psychiatric interview. Two-thirds of the glaucoma inpatients showed psychiatric symptoms, had higher scores for depression and psychosomatic complaints and had reduced emotional stability. The glaucoma out-patients had statistically significantly high scores for psychosomatic complaints. The control group showed normal scores in all tests. The present tests seem to pick up psychiatric symptoms in patients with primary open-angle glaucoma. Therefore, we can recommend them as an aid to orientation in hospital and private practice.
