Thirty-nine cases of decreased libido in
glaucoma
patients on carbonic anhydrase inhibitors are reported. This symptom completely reversed or markedly improved after discontinuation of the drug in all cases. Twelve of these patients restarted their carbonic anhydrase inhibitor medication which resulted in a recurrence of decreased libido symptoms. There were 3 cases of impotency which reversed after discontinuation of the drug. Most likely, these symptoms are a result of the malaise and
depression
occurring in some patients on carbonic anhydrase inhibitor therapy.

We perfused enucleated human eyes via the anterior chamber by the constant pressure technique. Infusion of human serum into the anterior chamber of enucleated human eyes for 30 minutes at 23 mm Hg pressure induced a 42% decrease in facility of outflow, which was not relieved by irrigation of the anterior chamber with balanced salt solution or alpha-chymotrypsin. Diluted serum also reduced the facility of outflow. Measured in a glass viscometer, diluted serum had less viscosity than undiluted, but interfered with outflow from the eye more than anticipated on the basis of viscosity alone. When we used lens
depression
to induce tension on the iridocorneal angle to simulate the effects of contraction of the ciliary muscle, outflow facility increased in control eyes that had not been exposed to serum and in serum-perfused eyes. However, the partial obstruction to outflow that had been induced by serum persisted. Normal serum components may become adsorbed or entrapped in the aqueous outflow system so as to obstruct outflow, and this may result in secondary
glaucoma
in eyes with chronic uveitis.

Multiple serum chemical values were examined in 92 patients with chronic
glaucoma
who were treated with the carbonic anhydrase inhibitors (CAIs) acetazolamide or methazolamide, seeking relationships between serum composition and symptomatic side effects. Of the 92 patients, 44 complained of a symptom-complex of malaise, fatigue, weight loss,
depression
, anorexia, and loss of libido, which we have found most commonly to threaten continuation of therapy. Patients who had this symptom complex were significantly more acidotic than those without it. Ten of 24 patients who had chemical evidence of excessive acidosis reported a dramatic alleviation of symptoms when sodium bicarbonate was administered, although their serum CO2-combining power changed little. There was no correlation of the symptom complex with serum potassium concentration, except in a few patients who were simultaneously receiving chlorothiazide diuretics for systemic hypertension and who became frankly hypokalemic.

The connection between memory and learning with vision was investigated by studying 100 cataract operation patients, aged 71 to 76 years, 25 of them being men and 75 women. The cataract operation restored sufficient acuity of vision for reading (minimum E-test value 0.40) to 79% of the subjects. Short-term memory was studied with series of numbers, homogenic and heterogenic inhibition, and long sentences. Learning was tested with paired-associate learning and word learning. Psychological symptoms were measured on the Brief Psychiatric Rating Scale and personality on the Mini-Mult MMPI. Memory and learning improved significantly when vision was normalized after the cataract operation. Poor memory and learning scores correlated with monocular vision before the operation and with defects in the field of vision, due to
glaucoma
and exceeding 20%, postsurgery. Monocular vision and defects in the visual field caused a continuous sense of abnormalness, which impaired old people's ability to concentrate on tasks of memory and learning. Cerebrovascular disturbances, beginning dementia, and moderate psychological symptoms obstructed memory and learning on both test rounds.
Depression
was the most important psychological symptom contributing to poor memory and learning scores after the cataract operation. The memory and learning defects mainly reflected disturbances in memorizing.

One-hundred and seventy-seven subjects (207 eyes) were tested on the Humphrey Field Analyzer to evaluate the usefulness of static and kinetic peripheral nasal visual field testing in identifying glaucomatous defects. In 20 of the 187 glaucomatous or ocular hypertensive eyes glaucomatous defects were found in the peripheral nasal field while the central 30 degrees remained normal or showed non-specific diffuse
depression
. Kinetic peripheral nasal field testing identified 17 of these eyes while the static testing identified eleven. This difference was not statistically significant (P greater than 0.05, Stuart-Maxwell test). Twenty control eyes showed one glaucomatous peripheral nasal defect on static testing, and none with kinetic testing. These results differed statistically from the diagnoses in the diseased population (P less than 0.005, chi 2-test). This study suggests the potential value of nasal peripheral field testing in selected
glaucoma
patients for which static testing has no apparent diagnostic advantage over kinetic techniques.

Effect of instillations of 0.5% betaxolol solution (Alcon) on intraocular pressure, ocular hydro- and hemodynamics, function, systemic arterial pressure, and pulse was studied in 35 patients (57 eyes) with primary open-angle
glaucoma
. The effects of betoptic and arutimol on the eyes are compared. Betoptic is characterized by a marked hypotensive effect explained by
depression
of aqueous humor production. The drug does not influence ocular function and hemodynamics. Comparison of the hypotensive effects of betaxolol and arutimol has shown that arutimol more effectively reduces intraocular pressure.

A geriatric study was conducted on 213 institutionalized geriatric
glaucoma
patients (mean age 83.9 years) and 100 control patients (mean age 81.3 years). A 12-lead electrocardiogram (ECG) analyzed according to the Minnesota code was recorded for 212
glaucoma
patients and 95 control patients. The most frequent finding (in 56% of the
glaucoma
patients and in 38% of the control patients, P less than 0.05) was a negative or isoelectric T-wave, suggestive of ischemic heart disease. ECG findings suggestive of coronary heart disease (Q/QS patterns, ST-segment
depression
, negative or isoelectric T-wave, third or second degree AV block, left bundle branch block or right bundle branch block, intraventricular block or atrial fibrillation or flutter) was seen significantly more often in
glaucoma
patients (164/212; 77%) than in the control patients (59/95; 62%). Seventeen percent of the
glaucoma
patients had atrial fibrillation (AF), which was significantly more than for the control group (8/95; 8%). There was no difference in the number of ECG changes between patients with bilateral open-angle
glaucoma
and bilateral angle-closure
glaucoma
. The mean intraocular pressure of patients having AF (15.9 +/- 8 mmHg) was significantly lower than that of the other patients (18.4 +/- 11 mmHg) (P less than 0.05). Fifty-five
glaucoma
patients were considered blind (visual acuity less than 0.05 in the better eye). The visual acuity of patients having AF was lower than that of the other patients, and severe visual field defects (arcuate scotoma or a residual field in the temporal periphery) occurred, slightly more frequently in patients with AF (in 70% vs 51% of the other patients). Arrhythmias, especially AF, are connected with impairment of visual acuity and visual field defects in
glaucoma
patients. The result of this retrospective study indicate that ECG changes occur frequently, suggesting coronary heart disease in elderly
glaucoma
patients.

A prospective study was conducted to identify the value of the differential light threshold changes in
glaucoma
patients. Using Humphrey Field Analyzer, we determined the differential light threshold on 42 preselected points in the Bjerrum area in the age-matched early
glaucoma
and normal group, each in 50 eyes, whose initial Goldmann perimetries were normal. It was found that both generalized and localized sensitivity of the retina declined obviously in the glaucomatous group. After Goldmann perimetry follow-up for 2.5 years, 24 eyes showed reproducible typical visual field defects (VFD) in the
glaucoma
group, which appeared in the same location where light threshold had reduced previously. Therefore, a typical
glaucoma
VFD was preceded by the slight localized
depression
of light sensitivity in the Bjerrum area.

We examined the physical, sociodemographic, and psychosocial health status of diabetic Mexican Americans aged 45 to 74 years using data from the Hispanic Health and Nutrition Examination Survey. Diabetic Mexican-American women had lower education levels and lower employment rates in comparison with nondiabetics.
Glaucoma
, retinopathy, and activity limitation were more prevalent in diabetic than in nondiabetic men and women. Diabetic women also had a higher prevalence of hypertension, kidney problems, and cataracts. No significant differences were found in
depression
levels between diabetic and nondiabetic Mexican Americans of either sex. Among diabetics, duration of diabetes was associated with increased prevalence of stroke and activity limitation. These findings on the health status of diabetic Mexican Americans furthers our understanding of the health service needs of this population.

Both delta 9-tetrahydrocannabinol (delta 9-THC) and cannabigerol, two naturally occurring marihuana cannabinoids, produced only a modest fall in intraocular pressure after acute topical application to the eyes of cats. After chronic administration unilaterally to the cornea via Alzet osmotic minipumps and connecting extraocular cannulas, however, a considerable fall in ocular tension amounting to 4 to 7 mm Hg occurred. After systemic administration of delta 9-THC to rats, polyspike discharges appeared in the cortical electroencephalogram initially during wakefulness and behavioral
depression
. These polyspikes subsequently became evident within rapid eye movement sleep episodes. Cannabigerol was devoid of this effect. After removal of either sympathetic or parasympathetic input to the eyes of cats, the intraocular pressure lowering effect of delta 9-THC was not changed. Neither delta 9-THC nor cannabigerol altered the rate of formation of aqueous humor. On the other hand, both cannabinoids produced a two-to three-fold increase in aqueous outflow facility. These results suggest that cannabigerol and related cannabinoids may have therapeutic potential for the treatment of
glaucoma
.
