95 million worldwide have undergone sterilization, 6 million of the in the US, and on the North American continent 800,000 sterilizations are performed annually. Since 1972, sterilization has been available liberally in Czechoslovakia. 85% of women bear 2 children by age 30, and more than 40% of abortions are repeat abortions, a large percentage of them for women 30. Microsurgery can restore fertility in 50-70% of cases. The negative effect of tubal sterilization on the menstrual cycle was studied in a caseload of 52,026 operations at 45 institutes in 24 countries. 1555 women aged 25-34 with a parity of 2-6 children were selected, and they were followed up for 3 cycles prior and 3 cycles after sterilization. 82% claimed regular menstruation, 54% had irregularity, in 8% irregularity changed after the operation, and in 5% the regular cycle became irregular. In 26%, the days of bleeding decreased, and they increased in 22%. In 2 groups of 86 women each who underwent tubal ligation and hysterectomy in the course of Cesarean section, postoperative infection was 9.3% with hysterectomy and 19.7% with tubal ligation. The negative effect of tubal sterilization on ovarian steroid genesis was shown in 112 women under age 46 with regular menstruation 2 years subsequently: urogenital, cardiovascular, and psychological disorders occurred. In a group of 59 women 41 and another group of 59 women aged 20-41, a significant drop of total estrogen excretion was found 24 hours after sterilization. In another 14 cases,

The consequences of altered immuno-competence resulting from anaesthesia and surgery are potentially hazardous to the patient with malignant disease. We report a retrospective study of 15 patients with prostate
carcinoma
and diffuse bone metastases treated with sodium 32P for palliation of pain at Downstate Medical Center and Kings County Hospital from 1973 to 1978. The response rates, duration of response, and toxicities are compared with those of other series of patients treated with 32P and with sequential hemibody irradiation. The response rates and duration of response are similar with both modalities ranging from 58 to 95% with a duration of 3.3 to 6 months with 32P and from 75 to 86% with a median duration of 5.5 months with hemibody irradiation. There are significant differences in the patterns of response and in the toxicities of the two treatment methods. Both methods cause significant bone marrow

. Acute radiation syndrome, radiation pneumonitis, and alopecia are seen with sequential hemibody irradiation and not with 32P, but their incidence can be reduced by careful treatment planning. Hemibody irradiation can provide pain relief within 24 to 48 h, while 32P may produce an initial exacerbation of pain. Lower hemibody irradiation alone is less toxic than either upper hemibody irradiation or 32P treatment.

A novel antitumor antibiotic, 2a,3,4,5,6,6a,7,11b-octahydro-11-methoxy-12-methyl-3,6-imino-1H-2-oxa-11 c- azanaphth(1,2,3-cd)azulene-5-carboxylic acid monocitrate (quinocarmycin citrate; KW2152) was selected for investigation in a number of experimental tumor systems because of its efficacy against P388 leukemia. In the initial studies with P388 leukemia (i.p.-i.p.), KW2152 gave an increase in life span of greater than 80%. The activity was schedule dependent and daily administration was the most effective. KW2152 caused marginal activity against L1210 leukemia, B16 melanoma, and M5076 sarcoma. The effect on cultured cells suggested that KW2152 was not cross-resistant to Adriamycin (ADM) but was cross-resistant to mitomycin C (MMC); however, KW2152 caused prolongation of life span against mice bearing P388/ADM or P388/MMC. In tests against human tumors xenografted s.c. in nude mice, KW2152 significantly inhibited the growth of MX-1 mammary
carcinoma
with all tumors cured at i.v. doses of 4.4 mg/kg/day and p.o. doses of 26.2 mg/kg/day given daily for 7 days. KW2152 also inhibited distinct human gastric carcinomas, St-4 and St-15 tumors, and colon carcinoma Co-3 by daily administration for 7 days. Against St-4, KW2152 gave a treated versus control percentage of 27, compared to 52 for cis-diamminedichloroplatinum. Against Co-3, KW2152 was at least as effective as MMC, ADM, cis-diamminedichloroplatinum, and bleomycin, giving a treated versus control percentage of 18 at a dose of 8.6 mg/kg/day given daily for 7 days. KW2152 showed growth inhibitory activity against cultured murine tumors and human cells. The order of in vitro efficacy of KW2152 against murine tumors, P388 leukemia greater than L1210 leukemia, B16 melanoma, correlated with the order of the sensitivity on the i.p.-i.p. systems of these tumors. The 50% inhibitory concentrations against P388 leukemia cells were 5.3 X 10(-6) and 1.1 X 10(-7) M after 1 and 72 h exposure, respectively. KW2152 caused significant inhibition of RNA synthesis after a short time exposure. In P388 leukemia cells exposed for 1 h with KW2152, the 50% inhibitory concentration for RNA synthesis was 10(-5) M, 30-fold less than that for DNA synthesis. White blood cell
