Despite recent attention to death and dying, many questions remain about the diagnosis and treatment of depression in cancer patients. To provide a framework for effective treatment, this article distinguishes the following types of depression: (1) transient stress reactions, (2) major psychiatric disturbances requiring prompt attention, and (3) other depressive reactions. Depressions in the third general category, comprising the majority, are often mismanaged because of characterological, interpersonal, and organic factors receive insufficient attention. Understanding of such factors provides a basis for rational intervention with both patients and their families.

A recently developed model of the human granulpoietic system had been used to analyze the toxic effects of 5-fluorouracil administered to cancer patients. From this analysis there appeared to be a relationship between the level of the early leukopenic depression and the rate at which the drug kills cells in the marrow. In particular, it may be possible to estimate this kill rate from the extent of the maximal depression in the white blood count during the second or third week after treatment begins. This estimate may be useful when planning modifications in treatment in order to prevent the onset of hematologic crises.

The antitumor agent chloroethylcyclohexyl-nitrosourea (CCNU) was examined using a rat glioma model. A i.p. administration of three times 40 mg/kg CCNU was highly effective and increased the life-span of tumor-bearing rats from 53 to 86%. A total quantity of CCNU amounting to less than 30 mg/kg was not effective. On the other hand, large dosages exceeding LD10 proved to be toxic. The depression of platelets and white blood cells was mild after a single dosage of 40 mg/kg CCNU which recovered on about the 6th day. In the CCNU-treated animals there was an increase of the extent of necrobiosis. Ballooning of tumor cells with nuclear pyknoses and a lack of mitotic features occurred. Microcystic changes appeared sometimes to be more frequent than in control groups.

Malignant tumours of nonendocrine tissues may produce ectopic hormones. The most likely mechanism is depression of genes which code for hormones. Ectopic hormones are invariably peptides, and each is identical to some peptide product of an endocrine gland. However, the majority of ectopic hormones occur as biologically inactive precursors or subunits and therefore remain occult unless they are specifically sought. When appropriate assays are made for such inactive forms, it is found that ectopic production of hormone-like peptides occurs frequently. Clinical syndromes result only in the relatively rare patients in whom a biologically active form is synthesized in large quantities. Laboratory research in this area improves our understanding of genetic control mechanisms in neoplasia. Ectopic hormones may be of limited use in diagnosis of cancer, especially when multiple markers are measured simultaneously.

The injection of a syngeneic Gross-virus-induced lymphoma into W/Fu rats induced peaks of cytotoxicity in the spleen attributable to non-T cells and T cells 3 and 10 days later, respectively. The conditions required for augmenting the cytotoxicity of the non-T cells in various lymphoid compartments (shown elsewhere to closely resemble NK cells) were analysed using the ip and iv routes of inoculation and a variety of tumour cells including those normally susceptible or resistant to lysis by NK cells in vitro. Using an ip inoculation of W/FuG-1 cells (a tumour susceptible to lysis by NK cells), a short-lived, 3-fold increase in cytotoxicity was observed in the spleen at day 3 and a 5-fold increase in the PEC at day 5. Cytotoxicity in other lymphoid organs remained unchanged. Tumours resistant to lysis by NK cells also stimulated cytotoxicity in the spleen or PEC, although the effect depended on the dose and route of inoculation used, and depression of cytotoxicity was observed under some conditions.

Urinary and blood hormonal profiles were studied throughout a monthly cycle in a patient with familial breast cancer. Two comparison cohorts (one high-risk and one low-risk) were studied concurrently. Findings disclosed that our breast cancer-affected patient showed a distinctive hormonal pattern characterized by significant elevation throughout the cycle of plasma estrone, estradiol, and prolactin. Save for a depression in plasma FSH in the early follicular phase, this hormone, as wells as LH and progesterone patterns in our patient, were similar to the comparison cohorts. Urinary estrone and estradiol patterns in our patient were elevated early in the follicular phase. Our patient also showed a depression in urinary estrone, estradiol, and estriol following ovulation, which persisted throughout the luteal phase. Blood and urinary hormone patterns in the high-risk cohort were not demonstrably different from the low-risk cohort, with the exception of plasma prolactin. The results on the latter hormone showed an unexpected significant depression throughout most of the menstrual cycle in this low-risk cohort. We conclude that estrone and estradiol elevations, as clearly evidenced in our breast-cancer-affected patient, may provide clues that ultimately might be used as an etiologic discriminant for breast cancer risk and which may also play a pathogenic role in this disease. Since this involved a single patient, our conclusions must be interpreted cautiously.

PMID:Endocrine profile in a patient with familial breast cancer: a case-control study. 49 52

77 patients were examined during a period from 1 to 30 days following gastrectomy for cancer. Depression of the exocrine function of the pancreas was observed. The concentration of amylase, lipase and trypsin was found to be reduced during the first 5 days after the operation. Following 19-30 days the amylase concentration is restored, the lipolytic activity is enhanced, whereas the tryptic one remained considerably impaired. The small intestine functioning is markedly disturbed, the processes of hydrolysis and fat absorption suffering most of all; the protein absorption is changed but to a less extent.

Recently developed techniques for the investigation of iron kinetics were used to study the disturbance of iron metabolism in 19 untreated patients with Hodgkin's diseases (HD). The erythroid abnormality in newly diagnosed HD appears to be confined to those patients with systemic symptoms of weight loss, night sweats and fever, and consists of depression of marrow erythroid activity. These patients had a significnatly lower haemoglobin and serum iron concentration and a higher serum ferritin concentration, both when compared to normal subjects and to those patients with HD who lacked systemic symptoms. Ineffective erythropoiesis and red-cell destruction were not significantly increased. The present findings, confirm that HD patients with systemic symptoms have a depression of erythropoiesis, and that in these patients the marrow fails to respond to the stimulus of mild anaemia.

Thirty-nine male and 90 female patients aged 40 and over, who had been given a primary diagnosis of depression, were followed up for 2 1/3-4 years. During this period 9 male and 9 female patients died. Five male patients and 1 female died from cancer that had not been diagnosed at the time of their psychiatric admissions. The male cancer deaths are significantly higher than expected. The possible relationships of malignant neoplasm to affective disorder are discussed.

In gastric cancer there was revealed a suppressed delayed allergy to dinitrochlorobenzene (DNCB) and failure of dermal macrophages to render depression in the zone of percutaneous phytohemagglutinin injection. The most unfavourable prognosis was noted in cancer patients non-responsive to DNCB.
