Interest in the concept of caring and the ethics of caring has grown as technology and
depersonalization
of health care delivery have increased. Major concepts of the ethics of caring are reviewed, with special focus on relating caring and virtue ethics. Assuming that caring is a virtue, it is concluded that ethically virtuous nurses must possess the caring attributes and demonstrate caring actions.

Fears of a future shortage of experienced social workers in the acquired immune deficiency syndrome (AIDS) field because of burnout prompted this study of 128 hospital social workers who work with AIDS patients. Results show that hospital AIDS social workers had slightly higher rates of emotional exhaustion and
depersonalization
on the Maslach Burnout Inventory but also felt a substantially higher level of personal accomplishment. Age, autonomy, and belonging to a support group explained 19 percent of the variance in burnout. Hospital administrators are urged to examine their administrative policies concerning these factors and to explore ways to preserve and enhance the most satisfying aspects of the work. Further research is encouraged to explore the relationship between young age and burnout and to better understand the factors that make AIDS work satisfying.

Forty-nine nurses working in neonatal intensive care were surveyed by using measures of job stress, burnout, anxiety, and perceived social support. On the Maslach Burnout Inventory, they scored in a moderate range of burnout for emotional exhaustion and
depersonalization
and in a high range of burnout for sense of personal accomplishment. Stepwise regression analyses revealed that higher job stress scores, higher anxiety scores, perception of less supervisor (head nurse) support, and less experience were associated with higher burnout subscale scores.

, and high emotional exhaustion. Perceived stress was a significant predictor of the sense of personal accomplishment and emotional exhaustion. 2. Both male and female subjects had low to moderate mean scores on the social support scale. Social support as measured in this study was limited to that received from coworkers and supervisors. 3. The results of this study point to a need for intervention in this setting. Specific interventions for decreasing stress and thus decreasing chances for burnout are discussed. 4. The development of a broad employee health care program is an example of a primary prevention intervention to decrease stress in this setting. Such a program would include instruction in methods of stress reduction and stress management, including exercise and relaxation techniques.

Burnout among 67 residents in four family practice training programs was explored. The residents' scores on the emotional exhaustion,
depersonalization
, and lack of accomplishment subscales of the Maslach Burnout Inventory were used to assess burnout. These scores were examined in relation to situational and background measures, two personality instruments (the Millon Clinical Multiaxial Inventory and the Myers-Briggs Type Indicator), and to regrets about career decisions. Few significant relationships were found between the background and situational factors and the burnout scores, but numerous relationships were found among personality measures, burnout scores, and measures of regret. The pattern of these relationships indicates the importance of interpersonal comfort and skills in mitigating burnout. Although personality factors were more predictive than demographic and situational variables of the variability in burnout among residents in the sample, the variables shared across the sample--long hours, little time for leisure activities and social contact, and compulsive personality characteristics--may contribute to the moderate level of burnout shared by these residents.

Occupational therapists who would better understand and advocate against
depersonalization
in health care can find specific references in narratives to the attitudes and behaviors that seem problematic. Patients argue that helpers fail to recognize that illness and disability are events charged with personal meaning. Instead of communicating with patients, helpers establish a distance that diminishes them. They withhold information in a manner that precludes hope, they use brusque manners, and they misuse their powers. Each of these behaviors seems unreasonable and impersonal, and each discourages patients. Together these narratives might inspire therapists to value interactive reasoning as central to practice, to recommit to their consideration of persons, and to enact a climate of caring.

Depersonalization disorder comprises one of the four major dissociative disorders and yet remains poorly studied. There are no reports describing the application of dissociation scales to this population. Our goal was to investigate the applicability of four such scales to depersonalization disorder and to establish screening criteria for the disorder. Two general dissociation scales and two
depersonalization
scales were administered to 50 subjects with DSM-III-R depersonalization disorder and 20 healthy control subjects. The depersonalization disorder group scored significantly higher than the normal control group in all scales and subscales. Factor analysis of the Dissociative Experiences Scale (DES) yielded three factors as proposed previously, absorption, amnesia, and
depersonalization
/derealization. A DES cutoff score of 12, markedly lower than those previously proposed for the screening of other dissociative disorders, is required for the sensitive detection of depersonalization disorder. Alternatively, the DES pathological dissociation taxon (DES-taxon) score recently generated in the literature appears more sensitive to the detection of depersonalization disorder and is better recommended for screening purposes. The other three scales were fairly strongly correlated to the DES, suggesting that they may measure similar but not identical concepts, and cutoff scores are proposed for these scales also. General implications for the screening and quantification of
depersonalization
pathology are discussed.
