In five immunocompetent patients, 3 of whom were moderately ill, herpes simplex virus (HSV) oesophagitis was diagnosed. HSV oesophagitis is a frequent infection in immunocompromised hosts. Nevertheless the English medical literature (Medline) contains at least 33 cases of HSV oesophagitis in immunocompetent persons over the period 1983 to 1993.

and chest pain are common symptoms. Endoscopy is necessary for diagnosis: brush cytology, biopsy for histology and viral culture are the tools for identification of herpes simplex as the cause of the oesophagitis. HSV oesophagitis in an immunocompetent patient is an acute but self-limiting disease. Nevertheless acyclovir per os or intravenously may be recommended if started early after onset of symptoms.

Esophageal conditions due to fungal, ulcerative, and neoplastic causes often signal the onset of symptomatic HIV infection. Most cases are fungal and due to Candida albicans, which is characterized by esophageal inflammation causing pain on swallowing (
dysphagia
and odynophagia). Ulcerative esophageal disease is commonly associated with cytomegalovirus (CMV), idiopathic causes, and herpes simplex virus (HSV). CMV, characterized by odynophagia resulting from ulcerations in the distal third of the esophagus, is clinically indistinguishable from idiopathic ulceration. HSV is more widespread and abrupt than other ulcerative processes, and its erosive injury can cause
painful swallowing
, ulceration and oral cavity lesions. Patients with esophageal distress, low CD4 counts, and little possibility of other GI conditions most likely suffer from Candida infection and should immediately begin an empiric trial of antifungal therapy. If an individual's first bout of odynophagia does not respond to empiric oral azole therapy, the diagnosis of fungal esophagitis is probably incorrect and an upper endoscopic evaluation should be performed. Patients generally respond quickly and completely to treatment of a first episode of fungal esophagitis; therefore, neither primary prophylaxis nor long-term suppressive therapy are recommended due to the risk of infection with a resistant strain. Failure of patients on suppressive therapy to respond to antifungal medication usually indicates resistant fungal infection that may require treatment with intravenous amphotericin. If CMV-isolated esophagitis is diagnosed, the patient should begin intravenous ganciclovir, followed by IV foscarnet if the healing after three weeks is minimal.

are clinical signs associated with a wide spectrum of potential causes. Our patient was an 84-year-old woman, who suffered from odynophagia and
dysphagia
for over 10 years. The interstinoscopy showed blister-like lesions and signs of inflammation only in the proximal part of the oesophagus. The diagnosis of pemphigus vulgaris was supported by immunohistological results. Typical oropharyngeal and cutaneous lesions of pemphigus were missing in our patient. Involvement of the oesophagus is described in literature, but it seems to be underdiagnosed in clinical everyday's work. Restriction of the lesions to the oesophagus without oropharyngeal and cutaneous manifestation - as we have seen in our patient - is rare.

A case of parathyroid adenoma causing a spontaneous cervical hematoma is reported. A 55-year-old woman presented with
painful swallowing

, and dyspnea. Primary hyperparathyroidism and a parathyroid tumor on the left side of the neck had been found 2 years earlier. Fiberoptic laryngoscopy and computed tomography (CT) showed a narrowing of the airway as a result of a retropharyngeal hematoma, and tracheostomy was carried out. Parathyroidectomy was performed 5 months after the absorption of the hematoma, with no complications. A pathological diagnosis revealed a parathyroid adenoma with hemosiderin deposition and fibrosis. Serum calcium and intact parathormone levels normalized postoperatively. Although a spontaneous cervical hematoma resulting from parathyroid tumors rarely occurs, it may lead to an airway compromise. Physicians should be aware of this if such tumors are managed conservatively without surgery.

In paracoccidioidomycosis (PCM), the primary lung infection remains silent. In this study, attempts were done to define the primary target organ by correlating lung radiographic abnormalities with the time course of mucosal/skin lesions concurrently exhibited at diagnosis by 63 patients in whom microscopy and/or isolation of Paracoccidioides brasiliensis from respiratory secretions had been positive. Mucosal and skin lesions were found in 65.1% and 12.7% of the patients, respectively.

We report a case of fractured genial tubercles associated with a mandibular denture in an elderly female patient and review the literature reports of this condition over the past 50 years. The period without natural teeth and the amount of atrophy of the mandible appear to be associated with the condition, especially when atrophy has reached an extent where the genial tubercles become a direct part of the denture bearing area. The signs and symptoms generally associated with this condition include pain and swelling, haematoma of the floor of the mouth, painful and limited movement of the tongue,

, and an obvious experience of discomfort while wearing the lower denture. The radiographic assessment of the mid-line region of the face on conventional radiograph views is often complicated by superimposition of the spine, and therefore computerised or cone-beam volumetric tomography will provide a better view of the fractured segment. The management of the condition is either by conservative care, or by surgical exposure and removal of the fractured tubercles. Irrespective of the chosen mode of treatment, the reports indicate that recovery can be expected to be uneventful in a relatively short period of time. Consensus suggests that the indications for surgical intervention in fractures of the genial tubercles are minimal, as the majority of patients do not require any active treatment and had resolution of their symptoms after conservative management.

A male patient, 41 years old, swallowed a chunk of muskmelon because his attention was not centralized.

were the two main clinical manifestations. The esophagus barium swallow examination showed a foreign body in the upper esophagus. The patient did the rigid esophagoscope examination in the operating room under general anesthesia. There was a chunk of muskmelon in the upper esophagus and could not take it out from the esophagus by using foreign body forceps because the muskmelon was very crisp. In the end the muskmelon was sent in the stomach. The patient's symptoms were alleviated after operation.

Hyoid bone fractures due to blunt trauma are exceedingly rare. Here, we present an isolated hyoid bone fracture caused by blunt trauma as well as a detailed discussion of the injury and treatment options. A 32-year-old male was admitted to emergency department with odynophagia and severe neck pain. He had been hit in the neck with a metal rod during a fight. Computed tomography scan revealed a fracture on hyoid bone and local swelling of adjacent soft tissues. The patient\'s head was elevated, and ice packs were used to reduce the swelling. Diclofenac sodium and prednisolone were administered. Patient was discharged with a recommendation of out-patient control.

and dyspnoea should alert the physician to possible hyoid or laryngeal damage. Fibre optic laryngoscopy and neck CT are important diagnostic steps to reveal a possible life-threatening injury. Conservative treatment is usually adequate, and patients rarely require surgical intervention.
