A 23-year-old woman gravely ill with Pseudomonas septicemia secondary to presumed drug-induced bone marrow aplasia received marrow transplantation from two male HL-A identical sibling donors. She had a successful engraftment with excellent but temporary clinical improvement. The palliative treatment of esophageal carcinoma has included intubation, bypass, dilation, irradiation, and esophagogastrectomy. The last has been criticized by some on the basis of high operative morbidity and mortality. To assess the success of this method at our institution, we reviewed the 60 consecutive resections performed for carcinoma of the esophagus from January, 1972, through June, 1983. Forty-six patients had squamous cell tumors and 14, adenocarcinomas. There were 47 men and 13 women, and the mean age was 59.9 years (range, 38.5 to 78.9 years). The most frequent preoperative findings included
dysphagia
(55), weight loss (34), chest pain (22), and
vomiting
(49). Fifty (83%) out of the 60 resections were performed by the resident staff under the supervision of an attending surgeon. Four patients died within 30 days of operation, an operative mortality of 6.7%. Immediate causes of death included respiratory failure, myocardial infarction, hemorrhage, and renal failure. One of the patients who died and 3 of the survivors had an anastomotic leak. There were 27 additional complications in 24 patients: respiratory problems (8), arrhythmias (5), pleural effusion (4), gastric outlet obstruction (2), wound infection (2), and 1 each of pulmonary embolus, acute brain syndrome, congestive heart failure, myocardial infarction, chylothorax, and empyema. The one-, two-, three-, and five-year actuarial survival rates were 46%, 27%, 10%, and 5%, respectively. Mean survival for the 46 patients dead at the time of this study was 13.5 months. Outpatient follow-up data were available on 53 (95%) of the operative survivors and showed an absence of
dysphagia

, and weight loss. Upper airway obstruction due to a chronically inflamed and scarred epiglottis necessitated tracheostomy. Biopsy of tissue from a solitary cutaneous lesion demonstrated a subepidermal bulla. Direct and indirect immunofluorescence confirmed a diagnosis of cicatricial pemphigoid. Therapy was begun consisting of prednisone, 2 mg per kg per day, in combination with dapsone, 2 mg per kg per day. He did not improve until the prednisone dosage was increased to 4 mg per kg per day.

A full-term infant developed

, and regurgitation immediately after birth. On examination at 5 months of age, he was malnourished and had radiographically proven megaesophagus. On direct examination his blood was positive for Trypanosoma cruzi. Additionally an enzyme immunoassay for T. cruzi IgM antibodies was positive. He underwent corrective surgery for the megaesophagus but died post-operatively of aspiration pneumonia. At autopsy, the esophagus was markedly dilated. There was an extreme reduction of neurons in Auerbach's plexus, and inflammation of this plexus and the muscular layer of the esophagus. Nests of parasites were seen in the esophagus and bladder. It is suggested that this patient was not in the chronic phase of Chagas' disease but represents an acute digestive phase of this infection. This is the fourth case in the literature of congenital Chagas' disease with esophageal involvement and clinical signs, and the first in which megaesophagus was seen grossly at post-mortem examination.

Prescription of so inoffensive-seeming and common a treatment as mucilaginous laxative may have major (if rare) side effects, such as sudden esophageal obstruction. This usually occurs in old people who do not take enough water with the laxative and who may have minor esophageal pathology such as motility disorders or epibronchic diverticulum. The usually typical symptoms are sudden onset, with retrosternal pain,

or total aphagia, alimentary

and pseudohypersialorrhea. Diagnosis is always by radiography and endoscopy. Radiography must be performed with gastrografin, due to the risk of bronchoaspiration or esophageal fissure. Endoscopy may demonstrate the mucilagenous mass responsible for the obstruction and in most cases restore patency of the esophagus. This technique should always be attempted, if necessary several times, before resorting to surgery. The authors stress that in patients with a risk of esophageal obstruction, such as old people with esophageal disorders, it is essential to explain clearly to the patient that the laxative must be taken with a sufficient quantity of liquid.

Using a clinico-psychological method, 170 patients with functional psychosomatic pathology of the alimentary apparatus (functional

, gastralgias, the syndrome of the irritable large intestine) were examined. The study showed that these diseases manifested themselves after a considerable psychotraumatic situation. In women, the role of the stressor was played by disruptions in familial relations while in men, job-related stresses were usually involved. Particular vulnerability of the gastrointestinal tract expressed in episodic dysfunction from early childhood was a predisposing factor in all observations. The personality characteristics of the patients (as judged from the data obtained by techniques of Taylor, Aizenk, M.H.Q., "incompleted sentences" and the index of the stability-instability of marriage) are characterized by high anxiety, neurotism, depression, and liability to obsessive-phobic reactions. Comparison of the research findings with the relevant results obtained in an organic psychosomatic disease (duodenal ulcer) demonstrated that with further somatization of the affect, the above personality features tend to decrease. According to the data obtained functional psychosomatic disturbances progress to organicones only in 7.4% of cases.
