Seven patients with severe or complete obstruction of the esophagus by malignancy were treated with photoradiation after presensitization of the tumor with intravenous hematoporphyrin derivative. The 625- to 635-nm therapeutic light was delivered from a tunable dye argon laser system coupled through quartz fibers, passed through the biopsy channel of a flexible esophagoscope, with local anesthesia. All tumors (adeno, squamous, and melanoma) responded, and swallowing was improved. Although tumor is still present, one patient is 11 months from initial treatment and is eating a regular diet. Another patient is 8 months from initial treatment and has no dysphagia. One patient died of aspiration of gastric tube feedings and 3 others died of their disease at 3 weeks, 3 months, and 6 months, respectively, from their initial treatment. Another patient died 11 months from his initial treatment due to a cardiac arrhythmia. During the 11 months after his initial treatment he continued to eat a regular diet.

Two case reports are given of patients suffering from malignant melanoma of the esophagus, which occurs very rarely. In both patients the leading clinical symptom was dysphagia. Diagnosis was confirmed by endoscopy and biopsy, and surgery was done. One patient died within 3 weeks from pneumonia, the second patient is alive 3 months after resection of the tumor and is free of symptoms.

We report herein a rare case of primary esophageal malignant melanoma in a 49-year-old Japanese woman who presented with a 3-week history of dysphagia. Esophagogram and esophagoscopy demonstrated a polypoid tumor in the upper to mid-thoracic portion of the esophagus, and a radical subtotal esophagectomy was performed through a right thoracotomy. Histological and immunohistochemical studies proved this tumor to be a primary esophageal malignant melanoma. She received no postoperative adjuvant therapy and no signs of recurrence have been clinically observed for the 22 months since her operation.

A case of multiple primary malignant melanoma of the esophagus was reported. A 66-year-old male was referred to the hospital with complaints of appetite loss and dysphagia. A barium swallow revealed two polypoidal tumors of approximate equal size, one in the lower third of esophagus and the other in the cardiac region of stomach. The tumors were shown blackish at endoscopy and biopsy yielded a histological diagnosis of malignant melanoma. Abdominal CT, abdominal ultrasonography and hepatic arteriography showed metastasis to liver and rt paracardiac lymph nodes. However, no primary lesion of malignant melanoma but esophagus was found through the preoperative clinical examination. Partial resection of the esophagus along with the proximal of stomach and the paracardiac lymph nodes was performed. Microscopically, the typical finding of junctional activity adjacent to the tumor mass was not obtained but we diagnosed as primary esophageal tumor because melanocytes were present along the basal layer of normal esophageal mucosa and groups of melanophages were found in the submucosal layer. However, we could not conclude which polypoidal tumor was the primary lesion. The patient received systemic chemotherapy postoperatively and is alive 5 months after surgery.

Malignant melanoma metastatic to the esophagus, as the only evidence of disease, is rare. We present the only known case treated at our institution and review 10 other cases reported in the world's literature. Of 11 patients (including ours) with melanoma metastasizing to the esophagus and no other known disease, no patient survived 1 year. With this dismal prognosis, the goal of therapy in these patients must be palliation of dysphagia.

A 75 year old man with an eight month history of dysphagia and weight loss underwent pericardiocentesis for a massive pericardial effusion. The echocardiogram showed a mass in the left atrium, and computed tomography also showed distal oesophageal narrowing, which was found to be due to a malignant melanoma. The left atrial mass, investigated by immunoscintigraphy with technetium-99m labelled monoclonal antibody, was diagnosed as metastatic melanoma. This represents a rare case of primary oesophageal melanoma with left atrial metastasis.

A 48-year-old woman presented with a 6-month history of dysphagia, often associated with retrosternal chest pain. Upper endoscopy revealed an unusual pigmented lesion within the middle portion of the esophagus, and multiple biopsies were obtained. The histopathology and immunohistochemical profile of the tissue specimens were diagnostic of malignant melanoma. A thorough clinical and radiographic evaluation was performed, providing no additional findings or alternative primary source. Of approximately 11,500 melanoma patients entered in the Duke University melanoma database since 1970, this represents the only case of primary esophageal melanoma. The case is described and a review of the literature is presented.

Primary malignant melanoma of the esophagus is extremely rare. Less than 200 cases have been published worldwide up to now. We report a case of a 65-years old male suffering from dysphagia for four months. The endoscopy showed a polypoidal tumor of black colour in the distal part of the esophagus. The histological diagnosis of the initial biopsy specimen was melanoma. Partial esophagectomy and lymph node sampling were performed. The resected specimen showed a polypoid tumor with black pigmentation of 8 x 5 x 3 centimeter. Histopathology revealed a primary malignant melanoma of the esophagus stage pT2 pN1 M0 and grading three. The patient died 8 months after resection from general metastatic disease. Age, sex, symptoms, duration of symptoms and time of survival are similar as described in former cases.

Primary malignant melanoma of the esophagus is an extremely rare non-epithelial neoplasm with a fatal prognosis. At present, approximately 184 cases have been published in the world literature. Of all published cases, 10 (5.4%) occurred in Spain. A 76 year-old man with malignant melanoma of the esophagus presented with dysphagia and weight loss, and treated by subtotal esophageal resection is reported. There was no history of previous cutaneous melanoma and ophthalmologic examination had been normal. At esophagoscopy a polypoid and pigmented mass (diameter 5 cm) almost completely occluding the lumen in the lower third of the esophagus was found. Multiple tumor biopsies were obtained. Histological diagnoses were made by examining endoscopic biopsy specimens and confirmed with resected specimens. The purpose of this communication is to present a case of primary esophageal malignant melanoma proved preoperatively by histological and immunohistochemical studies (positive reaction to the HMB-45 and S-100 antigen), followed by a review of the literature of these exceptional tumors.

A pleomorphic (giant cell) carcinoma of the esophagus is reported in a 52-year-old man who had dysphagia and weakness. The 8-cm-high vegetating tumor consisted of solid sheets of poorly cohesive epithelioid cells broken into clusters by strands of stroma. Numerous giant cells showing phagocytic phenomenon were present. Immunochemical analyses demonstrated the epithelial origin of the neoplasm, although most of the tumor cells strongly expressed vimentin. Numerous tumor cells expressed synaptophysin. Neurosecretory granules were detected in some tumor cells on electron microscopic examination. The patient died 4 months after he became symptomatic. As far as we can ascertain, this is the first case report describing a pleomorphic carcinoma arising in the esophagus. This poorly differentiated carcinoma might be of neuroendocrine differentiation. In the esophagus, pleomorphic carcinoma must be distinguished from polypoid tumors such as carcinosarcoma and malignant melanoma.
