We report three patients with metastases to the ENT-region mimicking a primary malignant tumour. A 36-year-old woman presented with vertigo, sudden hearing loss, partial facial palsy and headaches. CT scan suggested a meningioma or an acoustic neuroma. Histological examination of the neoplasm removed surgically showed a metastasis from an amelanotic melanoma. A 38-year-old woman with nodules in the tongue had
dysphagia
. The history revealed that she had been treated successfully with chemotherapy for a carcinoma of the
uterine cervix
one year ago. Histological examination of a tongue biopsy showed a metastasis from the uterine carcinoma. The primary tumour was in complete remission. The third patient was treated for recurrent epistaxis. Physical examination showed a tumour in the right nasal cavity. A CT scan showed a tumour of the ethmoid cells and of the maxillary sinus, protruding into the nose. Histology and immunohistology proved a metastasis from a primary carcinoma of the liver. Ultrasound and CT scan of the liver confirmed the diagnosis.

Esophageal material for cytologic smears was obtained by the gel-foam technique from an 84-year-old man with
dysphagia
and symptoms of obstruction in whom initial attempts at endoscopic biopsy were nondiagnostic. The smears contained cells with koilocytotic atypia, similar to those described in smears from
uterine cervix
lesions produced by human papilloma virus. The cytologic diagnosis of an esophageal papilloma of viral origin was confirmed by the second biopsy. A third biopsy, from a lower level of the esophagus following dilatation procedures, revealed an epidermoid carcinoma in situ contiguous with the papilloma.

Extremely well-differentiated adenocarcinoma (EWDA) is an unusual gastric cancer that is histologically too bland to be diagnosed as malignant neoplasm, particularly using biopsy. EWDA may be a gastric counterpart of 'adenoma malignum' or minimal deviation adenocarcinoma (MDA) in the
uterine cervix
; however, the clinicopathological features of EWDA remain less apparent than those of MDA. A 60-year-old male was complaining of
dysphagia
. He had been made aware of a small submucosal tumor in the cardia 2 years before the onset of this symptom. Endoscopic ultrasonographic examination revealed a large cardiac tumor consisting of thickened layers, as observed in Borrmann type IV. Three mucosal biopsies suggested only benign changes including adenoma and hyperplastic polyps. At the fourth biopsy, cytologically bland columnar cells were located in the submucosa along with stromal fibrosis and laminated stones. The possibility that non-neoplastic aberrant pancreas with lithiasis formed the tumor was denied at laparotomy by a frozen section that revealed benign-looking glands invading the diaphragm. Immunohistochemically the cancer glands were positive for CA19-9 and human gastric mucin, but not for p53 or MUC2. To our knowledge, this is a previously unknown combination of EWDA and psammomatous calcification in the stomach.
