A case of late
cardiac tamponade
seen with progressive
dysphagia
15 days after aortic valve replacement is reported. The diagnosis was confirmed echocardiographically and successful pericardiocentesis was followed by immediate alleviation of the
dysphagia
.

A 41-year-old man was admitted to a hospital elsewhere because of tonsillitis with high grade fever. On the 9th day of hospitalization, the patient complained of
dysphagia
and dyspnea. A chest X-ray film and a CT scan showed right pleural effusion and pericardial effusion, and he was referred to our hospital. Immediately after admission, he underwent pericardiotomy to relieve
cardiac tamponade
, and a right thoracic tube was inserted for pyothorax. Next day, mediastinal drainage was accomplished through a cervical incision and a right thoracotomy. Eight drainage tubes were left in place. Cultures revealed alpha-Streptococcus, Neisseria and group F Streptococci. Continuous closed irrigation with diluted Isodine (povidone iodine) solution was performed. The last extubation of the drainage tube was done on the 140th day after operation. He was cured and discharged on the 162nd day after operation. In patients with extensive acute mediastinitis secondary to deep cervical infection, early complete mediastinal drainage via a cervical and a transthoracic incision is essential.

Pneumomediastinum is the presence of air in the mediastinum. Spontaneous pneumomediastinum (SPM) is an infrequent, benign, and self-limiting condition that predominantly affects young males and pregnant females. It is important to distinguish pneumomediastinum symptoms from similar clinical findings that require immediate treatment, such as
cardiac tamponade
, angina pectoris, dissecting aortic aneurysm, mediastinitis, and pulmonary embolism. This report describes 2 cases of SPM managed at University Hospital Hamburg-Eppendorf during the period 2000 to 2001. Spontaneous pneumomediastinum should be considered whenever there are anamnestic data for retrosternal chest pain that radiates to the neck or back accompanied by
dysphagia
, dysphonia, dyspnea, and a positive Hamman's sign.

Primary cardiac lymphomas (PCL) are rare cardiac neoplasms that carry an ominous prognosis. They occur more frequently in immunocompromised patients. We report on an immunocompetent 67-year-old who presented with dyspnea and
dysphagia
. Echocardiographic evidence of impending
cardiac tamponade
and obstruction of the inferior vena cava (IVC) with the tumor was seen. The deteriorating hemodynamics of our patient prompted an urgent surgical intervention. Pathohistological diagnosis showed diffuse large B-cell lymphoma of centroblastic subtype. Chemotherapy remains the standard treatment of PCL, with surgery reserved for relieving life-threatening complications of the neoplasm.

Pericardial cysts are rare benign intrathoracic lesions, more often located in the right rather than in the left cardiophrenic angle. At echocardiography, they appear as echolucent unilocular structures, which are in contact with the pericardium. They contain a clear water-like fluid and, thus, are referred to as "spring water cysts". They are usually discovered incidentally in asymptomatic patients. Sometimes they can cause symptoms (dyspnea, chest pain,
dysphagia
, cough), depending on their dimensions and location, or severe complications such as
cardiac tamponade
. Asymptomatic cases are managed conservatively with a close follow-up, being surgical excision recommended only in symptomatic patients. Actually, the treatment options include excision via thoracotomy, video-assisted thoracoscopic surgery, and percutaneous echo-guided aspiration. We describe the case of a patient for whom the echocardiographic follow-up allowed to disclose intracystic hemorrhage, leading to surgical treatment before the patient became symptomatic.

Mediastinal haematoma is a rare complication of laparoscopic paraoesophageal hernia repair with few documented cases in the current literature. Presentation of this unique clinical problem can range from extrinsic oesophageal obstruction to life-hreatening
cardiac tamponade
and therefore, warrants further discussion of at-risk population aetiology, diagnosis and successful management strategies. We present the case of a 71-year-old woman who underwent laparoscopic paraoesophageal hernia repair with nissen fundoplication complicated by severe
dysphagia
on postoperative day 12. Further evaluation with oesophagram and CT imaging revealed a large mediastinal haematoma with near obstruction of the distal oesophagus. This was managed successfully with laparoscopic transhiatal washout and drainage.
