21 patients with gastroenterological disease and indication for the use of intravenous nutrition received an elemental diet (ED) for 5-44 days. In 6 out of 8 patients with exacerbation of
Crohn's disease
remissions were achieved, apart from 3 persistent fistulas. In 5 out of 9 cases with various primary diseases and postoperative intestinal fistulas, spontaneous healing was observed. Furthermore, 2 patients with ulcerative colitis, 1 with radiation enteritis and 1 with pancreatitis were treated with ED. On ED, hemoglobin increased from 11.3 +/- 0.4 (m +/- SEM) to 12.0 +/- 0.5 g% (p less than 0.01) and
serum albumin
from 2.7 +/- 0.1 to 3.4 +/- 0.1 g% (p less than 0.001). Nitrogen requirements were studied in 11 patients receiving various quantities of ED. Nitrogen balance was found to be in equilibrium or positive in 7 patients, and negative in 4. In one patient with severe ulcerative colitis, fecal nitrogen losses were higher than urinary nitrogen losses. The unpleasant taste of ED resulting from free amino acids limited the ED supply in 3 patients and led to premature ending of ED administration in 3 other patients. In such cases ED may be given by nasogastric tube feeding. From the results presented it appears that ED is indicated in
Crohn's disease
and intestinal fistulas. However, the results obtained require confirmation by further observations and comparison with an intravenously fed control group.

with granulomas were prepared as snap-frozen or fresh and were injected into the ascending colonic walls of New Zealand white rabbits. Control animals were injected with 1 per cent bovine

alone. The rabbit bowel was examined after 1 year, and lesions were noted in each of the rabbits injected with
Crohn's disease
homogenate, irrespective of the type of tissue preparation. The observed lesions were diffuse and occurred both at the injection site and in the terminal ileum. These changes were not noted in the control group. This work confirms earlier results in the same animal model and suggests that either fresh or snap-frozen homogenates will produce the intestinal lesion but that bovine albumin alone will not.

Nineteen children and 160 adults with
Crohn's disease
were examined. The children had been under treatment between 1972 and 1974. There were 11 boys and 8 girls. Their main symptoms were general malaise, diarrhea, abdominal pains, anemia and weight loss. Clinical examination showed pallor, malnutrition, and abdominal tenderness on palpation. In 7 of 19 cases, rectosigmoidoscopy showed pathological changes. Radiologically, the most frequent site of the disease was observed to be in the ileocecal area. Since conservative therapy is associated with the risk of grave local and general complications, resection of the affected intestinal segment was the method of choice. Resection must be carried out through healthy tissues. Although the intestinal absorption of B12, fat and bile acids deteriorated after resection of the ileum, the general state of health of the patient improved. Body weight,

, serum iron concentration and the total iron binding capacity increased and the patient developed normally. Relapses occurred in 8 of the 19 cases. From the large number of adult patients who were studied, it could be shown that the risk of a relapse was greater in the ileal and ileocolic disease than in disease restricted to colon.

(86%), 5 with untreated celiac disease and in 12 of 28 normal subjects (43%). In patients with inflammatory bowel disease, antibodies to BSA were present in greater amounts in those with severe and moderate disease than in those with mild disease. We reviewed 46 consecutive patients with

predominantly affecting the ileum who specifically underwent right-sided ileocolectomy with primary anastomosis. All had a primary ileocolic anastomosis done by suture in a single-layer closed fashion. Of these 46 patients, 19.6% had overt intra-abdominal sepsis, 30.4% had prior surgery, and 95.7% were taking corticosteroid drugs just before their operation.
