Studies with azidomorphine derivatives have revealed that some of them, particularly N-cyclopropylmethylnorazidomorphine (CAM), stimulate some opiate receptors, while inhibit the others. The opiate receptors stimulated by CAM are called opiate A receptors, while those antagonized by CAM are called opiate B receptors. Opiate receptors are located at nerve terminals and upon stimulation decrease the release of a neurotransmitter. Opiate A receptors are most probably located at cholinergic nerve terminals, are present in the guinea pig ileum, mouse vas deferens and in the brain. Their stimulation leads to constipation and mental clouding. Opiate B receptors located on adrenergic nerve terminals are present in the cat nictitating membrane and in the brain. Their stimulation produces analgesia, depression of coughing and respiration, catalepsy, and mental clouding.

A 40-year-old man, referred for treatment of syncope due to cough, was found to have generalized tracheobronchomalacia. The diagnosis was based on an abnormal collapsibility of the trachea and bronchi on bronchoscopic examination. The detailed investigations confirmed the diagnosis of cough-syncope syndrome due to tracheobronchomalacia. Based on this case the causes, symptoms and possible treatment of those clinical entities are discussed.

A case of 55 years old woman with "hot" right lobe toxic thyroid nodule, presenting with paroxysmal atrial fibrillation, and therefore treated with 131I 666MBq (18 mCi) is described. After six years she became pyrexic and suffered of severe cough proxyisms. The fine needle biopsy of the above nodule showed the presence of anaplastic thyroid carcinoma. Strumectomy followed by local radiotherapy resulted in complete disappearence of all symptoms. The microscopic of the removed thyroid tissue confirmed the above diagnosis. After 22 months' observation the patient remained in good general condition. The possible reasons for the development of the thyroid carcinoma in this case are discussed.

A role of nutrients in the onset of migraine and other gastrointestinal symptoms (vomiting, nausea, diarrhoea), skin reactions (rush, atopic dermatitis, Quincke'a edema), respiratory symptoms (bronchial asthma, cough, allergic rhinitis, polyps, congestion of the nasal mucosa), motion system disorders (jointache and edema), gynecological disorders (chronic and recurrent adnexitis), and sleep disorders together with emotional tension and behavioral disturbances has been assessed in 17 patients with atopy. Migraine attacks have been produced most frequently by cow milk (in 10 out of 17 patients), cabbage, flour and eggs in 5 patients, preservatives, cottage and Swiss cheese, porcine meat in 4 patients, colorants and chocolate in 3 patients, beef, strawberries, lemons and butter in 2 patients. Other nutrients produced headache in single patients. Migraine and other symptoms have diminished after an individual elimination diet. Recurrence has been noted after each consumption of allergen except one female patient with EEG abnormalities. Immunoglobulins E have been involved in headache-producing mechanism in 3 patients.

In all of 11 observed patients with bakers' asthma, coughing preceded the development of dyspnea. Seventeen coughing atopic patients working in a bakery were observed for 2 years and treated with ketotifen and disodium cromoglycate (DSCG). Four of the patients developed asthma. Ketotifen prevented coughing attacks but did not prevent asthma development. Bronchodilators interrupted cough attacks. DSCG was ineffective.

Ambroxol (Bronchopront-Mack) was administered to 73 patients with the bronchial asthma and spastic bronchitis with expectoration disorders. The drug was given in the dosage forms of slow release capsules, syrup, drops, intramuscular injections and inhalations for about 14 days. All patients have been carefully examined clinically, PEF has been measured, and sputum physico-chemical properties have been tested prior to and 10 days after treatment. Complete disappearance of cough, liquefaction of sputum and clearance of airways have been achieved in 47% of patients. A significant improvement has been noted in 38.4% of cases. The treatment failed in 2.7% of patients. Ambroxol has been well tolerated. No adverse effects on laboratory findings have been noted. Results suggest, that ambroxol is valuable drug in the combined treatment of patients with the bronchial asthma and spastic bronchitis.

A case of a 34 year old female with clinical and histological confirmed eosinophilic granuloma causing osteolytic lesions of the mandible and skull and diabetes insipidus. Dyspnea, cough, exercise intolerance with ventilatory disturbances and radiological evidence of interstitial lung disease were believed to be in the course of eosinophilic granuloma. The patient refused lung biopsy and BAL. After starting therapy with corticosteroids all respiratory symptoms improved.

The author diagnosed cough, emphysema and the symptoms characteristic for the bronchial asthma in 43% of patients with seasonal allergy (rhinitis, conjunctivitis) to pollens. Such symptoms were more frequent (51% of cases) in patients allergic to the grass pollens with coexisting hypersensitivity to Compositae family. Asthmatic symptoms in patients allergic only to grass pollens were seen in 38%. The author suggests that prolonged exposition in the inhalatory allergens (from two to four-five months) plays an important role in asthma onset in such patients. It rather delays than accelerates admittance to allergic clinics.

Many reports describe an increase in the incidence of allergies in recent years. Thus the epidemiological studies are necessary for efficacious prophylaxis. The aim of our study was to estimate the prevalence of allergic diseases in schoolchildren. Allergic rhinitis and/or conjunctivitis was observed in 16.7%. Atopic dermatitis occurred in 12.9% cases. We showed the discrepancy between the number of children with symptoms suggestive for asthma (wheezing--11.1%, breathlessness--19.4%, nocturnal cough--4.9%, exercise-induced cough--9.8%) and number of cases diagnosed as asthma (3.2%). Family history of allergy increased the risk of allergic diseases in studied population.
