A report in 1984 on the success of zinc gluconate against common cold symptoms could not be confirmed in three subsequent studies, which are now known to have used formulations that inactivated zinc. A non-chelating formulation including glycine, which releases 93% of contained zinc into saliva, was tested in a randomized, placebo-controlled, double-blind trial in 73 young adults. Efficacy was recorded in symptom diaries using a symptom severity rating. Patients' symptoms first appeared 1.34 days prior to entry to the study in both groups. Disappearance of symptoms occurred after an additional 4.9 days for zinc-treated patients versus 6.1 days for placebo-treated patients. A difference was noted in the efficacy of treatment if it was started 1 day after symptom onset: cold duration was an additional 4.3 days in zinc-treated patients compared with 9.2 days for placebo-treated patients. Cough, nasal drainage and congestion were the symptoms most affected, and only mild side-effects were noted.

A smelter exposed to zinc fumes reported severe recurrent episodes of cough, dyspnea and fever. Bronchoalveolar lavage showed a marked increase in lymphocytes count with predominance of CD8 T-lymphocytes. Presence of zinc in alveolar macrophages was assessed by analytic transmission electron microscopy. This is the first case of recurrent bronchoalveolitis related to zinc exposure in which the clinical picture and BAL results indicate a probable hypersensitivity pneumonitis.

To determine whether orally administered zinc supplements could correct the abnormal humoral and cell-mediated immunity of Down syndrome, we randomly assigned 64 children with Down syndrome, aged 1 to 19 years and living at home, to receive either zinc gluconate or placebo daily for 6-month periods with crossover from one regimen to another. Control subjects were siblings and age-matched, unrelated children. Serum zinc, copper, and measures of immune system competence were tested at 3- or 6-month intervals. Parents kept daily logs of clinical symptoms such as cough and diarrhea and of physician visits. Mean serum zinc concentrations increased to about 150% of baseline during zinc supplementation, but we found no effect on serum levels of copper, immunoglobulins, or complement; on lymphocyte number or subset distribution; or on in vitro response to mitogens. Children with Down syndrome who were receiving zinc had a trend toward fewer days or episodes of cough and fever but no change in other clinical variables. Long-term, low-dose oral zinc supplementation to improve depressed immune response or to decrease infections in children with Down syndrome cannot be recommended.

Since their introduction in clinical practice in 1980, ACE inhibitors have been found useful in the treatment of hypertension and CHF. In hypertension, they are effective as monotherapy in 40% to 50% of the patients, and in combination with diuretics or calcium antagonists, they are effective in up to 85% of the patients. They are well tolerated, are not associated with depression, impotence, bronchospasm or metabolic derangements such as hypokalemia, hyperuricemia or hyperglycemia, and do not have adverse effects on the quality of life. As a result, they are preferred in hypertensive patients with CHF, left ventricular dysfunction, mental depression, older age, coronary artery disease, metabolic disorders, chronic destructive pulmonary disease, and peripheral vascular disease. In CHF they cause long-lasting hemodynamic and symptomatic improvement, improve exercise tolerance, and may lower mortality in certain patient subsets. Evolving new indications for ACE inhibitors include the diagnosis of renovascular hypertension, the prediction of surgical success, the treatment of scleroderma renal crisis, the reduction of proteinuria, renal protection, cardioprotection, the improvement of arterial compliance, in Bartter's syndrome and idiopathic edema, etc. ACE inhibitors are usually well tolerated but in some instances they may cause class-specific side effects such as hypotension; usually reversible azotemia or renal failure, especially in patients with renal artery stenosis or with CHF with low blood pressure; cough; angioedema; and hyperkalemia. Differences among ACE inhibitors are emerging and include chemical class (e.g., zinc ligand), biotransformation, potency, pharmacokinetics, prodrugs, tissue effects, additional pharmacologic properties, and drug interactions.

Five soldiers were injured by inhalation of hexite smoke (ZnCl2) during military training. Two soldiers, not wearing gas masks breathed hexite for 1 or 2 min, they slowly developed severe adult respiratory distress syndrome (ARDS) over the ensuing 2 weeks. This slow, progressive clinical course has not been previously described. In both patients, an increased plasma zinc concentration was measured 3 weeks after the incident. Intravenous and nebulized acetylcysteine increased the urinary excretion of zinc, and briefly decreased the plasma levels. In an attempt to arrest collagen deposition in the lungs, L-3,4 dehydroproline was administered. Both patients died of severe respiratory failure (25 and 32 days after inhalation). At autopsy diffuse microvascular obliteration, widespread occlusion of the pulmonary arteries and extensive interstitial and intra-alveolar fibrosis was observed. Three soldiers wearing ill fitting gas masks, immediately developed severe coughing and dyspnea. They improved, and 12 months after exposure their lung function tests were nearly normal, but they still had slight dyspnea on exercise.

Metal fume fever (MFF) is an acute industrial disease caused by the inhalation of a variety of heavy metal oxides. MFF occurs most commonly during welding operations, particularly those involving zinc oxide. The illness is of short duration and produces symptoms of cough, fever, chills, malaise, and myalgias. Its etiology is uncertain, and its diagnosis is difficult because symptoms resemble a number of pulmonary illnesses. Supportive treatment, with bed rest, analgesics, and fever control is used for symptomatic relief. Emergency medicine physicians must differentiate the clinical picture from other common respiratory illnesses. The mainstay of therapy for MFF consists of recognizing the disease and preventing subsequent exposure to harmful metals.

Clinical signs and haematological findings during the acute and convalescent phases of an infection with bovine respiratory syncytial virus (RSV) were studied under field conditions. The study was carried out in 139 cattle less than 16 months of age in 16 herds with serologically proven bovine RSV infections. Blood was collected for serological and haematological examination. Repeated clinical examinations were carried out until 35 days after appearance of disease. Signs of general disease such as reduced appetite and a body temperature of 40 degrees C or higher lasted less than three days in most cases. Signs of upper respiratory disease, such as coughing, nasal discharge and conjunctivitis were predominant and persisted in 10 to 30 per cent of the animals till the end of the observation period. Signs of lower respiratory disease such as abdominal breathing, bronchial and bronchovesicular sounds on auscultation and a high respiration rate were valuable in diagnosing the disease. These symptoms were present in about 50 per cent of the animals in the early stages of the disease. They lasted for about six days and disappeared in most animals after about 10 days. On haematological examination statistically significant blood changes were observed. A rise in zinc and iron between day 0 and day 10 and a reduction in copper content between day 10 and day 21 were found.

Addition of digested sewage sludge at concentrations of 2% and 10% (v/v) to the water increased coughing rate in big head and tilapia (P < 0.05). Ventilation rate was significantly decreased (P < 0.05) in big head and tilapia at sludge concentrations of 6% and 2% (v/v) respectively. Copper (Cu), cadmium (Cd) and zinc (Zn) are trace metals which are commonly found in sludge. Cu caused a significant increase (P < 0.05) in coughing rate in both tilapia and big head at concentrations of 0.3 and 0.2 microgram/ml respectively. Zn caused significant increase (P < 0.05) in coughing rate only in big head at 2 microgram/ml. Neither fish responded to Cd of up to 2 micrograms/ml in the water. However, when the levels of these trace metals in the digested sludge were measured, they were below that which can cause significant changes in the respiratory movements. Therefore, the changes in ventilation and coughing rates after addition of sludge may be due to the presence of substances other than these metals. The results of this experiment provides a guideline to control the level of sludge that can be used in rearing these fresh water fish in ponds.

An 8-month-old Labrador Retriever was examined because of a 1-month history of productive coughing unresponsive to ampicillin treatment. Larvae of Crenosoma vulpis were found in fecal samples examined by zinc sulfate centrifugation and Baermann technique. Physical examination abnormalities or larvae in fecal samples were not detected 6 weeks after treatment with prednisone (1 mg/kg, PO, q 24 h, for 7 days, then 0.5 mg/kg, PO, q 48 h, for 8 days) and fenbendazole granules (50 mg/kg, PO, q 24 h, for 3 days). This report suggests that fenbendazole may be effective for treating Crenosoma vulpis infection in dogs.

An unexpected mortality of more than 300 cattle was investigated near a metal recovery factory located in a rural area of the Thane district of India. The factory was engaged in reclaiming lead, aluminum, tin, and zinc from discarded lead storage batteries and soft drink cans. The environmental samples (soil, leaves, grass, slag, water, and sediment), human blood and hair and animal samples (blood, urine, peritoneal fluid, liver, kidney, cow dung, ribs, and femur), collected for analysis revealed toxic levels of lead, cadmium, and chromium. Clinical examination of factory workers and school children revealed cough, fever, gastric problems, abdominal pain, skin lesions (scabies), and blue line on gums. Histopathological examination of animal tissues revealed chronic pathology with lead inclusion bodies in hepatocytes and renal tubules. Based on environmental, clinical, analytical, and histopathological observations, the mortality has been attributed to toxic levels of metals in the body and the malnourished status of the animals.
