A 12 year old boy was admitted to hospital with fever, general malaise,
cough
and peripheral edema. The patient who have had rheumatic heart diseases-mitral insufficiency was found to be in congestive cardiac failure. In blood cultures Staphylococcus aureus and Alpha-hemolytic streptococcus grew. The regimens of Cephalothin-Gentamicin,
Methicillin
-Tobramicin, to which the organism were sensitive were given intravenously. On these therapy the patient continued to have fever. He was put on Trimethoprim-Sulfomethoxazole intramuscularly. He became afebril for the first time. After two weeks fever recurred. In spite of medical treatment, the infection persisted and the indication for surgery was considered. Mitral valve replacement with a Starr-Edwards prosthesis was carried out. Postoperatively, the patient was treated with TMP-SMZ. For the past 10 months the patient has remained afebril and without evidence of congestive heart failure.

In January 1999, an outbreak of type A(H3N2) influenza occurred in a psychiatric hospital. Among 273 inpatients, 59 male and 40 female patients, age between 24-84 year old, developed high fever(38 degrees C), joint pain,

and nasal discharge, and eleven male and one female patients died. In that hospital,

-resistant Staphylococcus aureus(MRSA), with production of toxic shock syndrome toxin-1 (TSST-1) and staphylococcal enterotoxin(SE) type C(SEC), were isolated from inpatients, medical stuffs and hospital environments. Under investigation, a 59-year-old male inpatient developed respiratory failure following influenza infection. The titer of serum TSST-1 antibody and SEC antibody of this patient was significantly elevated in his clinical course. It is highly suspected that influenza outbreak occurred among MRSA carriers and TSS was complicated with fatal cases.

The aim of the study was to analyze epidemiological and microbiological aspects of oral colonization by methicillin-resistant Staphylococcus of health care workers in a cancer hospital. Interview and saliva sampling were performed with 149 health care workers. Antimicrobial resistance was determined by disk diffusion and minimum inhibitory concentration. Polymerase Chain Reaction, Internal Transcribed Spacer-Polymerase Chain Reaction and Pulsed Field Gel Electrophoresis were performed for genotypic characterization of methicillin-resistant Staphylococcus. Risk factors were determined by logistic regression.

-resistant Staphylococcus colonization prevalence was 19.5%, denture wearing (p = 0.03), habit of nail biting (p = 0.04) and preparation and administration of antimicrobial (p = 0.04) were risk factors identified. All methicillin-resistant Staphylococcus were S. epidermidis, 94.4% of them had mecA gene. Closely related and indistinguishable methicillin-resistant S. epidermidis were detected. These results highlight that HCWs which have contact with patient at high risk for developing infections were identified as colonized by MRSE in the oral cavity, reinforcing this cavity as a reservoir of these bacteria and the risk to themselves and patients safety, because these microorganisms may be spread by
coughing
and talking.
