Propylthiouracil

) is usually the first choice for the treatment of hyperthyroidism, but it has serious side effects such as hepatitis, cholestatic jaundice, splenomegaly and lupus-like syndrome, in addition to mild and common side effects like granulocytopenia, pruritus, urticaria and maculopapular or papular eruption. Antineutrophil cytoplasmic antibody (ANCA)-positive vasculitis is another serious side effect. A 14-year-old female receiving

treatment for hyperthyroidism was referred to our clinic with fever,
cough
and dyspnea. The

dosage was first decreased but pericardial, dermal and joint involvement ascribed to
PTU
developed later and the drug was discontinued. ANCA-positive vasculitis due to
PTU
was considered when tests revealed an ANCA-positive state. We suggest that severe multisystemic vasculitis due to
PTU
should be considered during
PTU
usage.

-induced interstitial pneumonia is rare--only three cases have been reported--and

-induced nonspecific interstitial pneumonia (NSIP) has not been reported. We report a patient who developed NSIP after taking
PTU
for 1 year. She developed dyspnea,
cough
, and mild fever lasting 1 month, and a chest CT scan showed multifocal patchy consolidation in both lungs. She underwent a surgical lung biopsy, and NSIP was confirmed pathologically. The symptoms and abnormalities seen in the chest radiograph improved after withdrawal of
PTU
. To our knowledge, this is the first documented case of pathologically proven
PTU
-induced NSIP.

out of blood since 1 day. Patient was taking

(100 mg per day) since 4 years for hyperthyroidism. Patient was immediately intubated for type-II respiratory failure. Diagnosed to be having
PTU
-induced autoimmune disease.
PTU
was stopped and treated with methylprednisolone and cyclophosphamide. Clinical features improved over a period of 8 days and discharged home successfully. Having a high suspicion for the onset of autoimmune disease in hyperthyroidism patients who are on
PTU
therapy and timely treatment with immunosuppressants and supportive care along with the withdrawal of the drug can make a difference in morbidity and mortality.
