A case of endobronchial minute leiomyoma successfully treated by bronchoscopically directed forceps biopsy is described.

-year-old male with a 20 pack-year smoking history was admitted for dry
cough
occurring at night. Chest X-ray showed no abnormal shadows. The tumor, measuring 2 by 2 mm, was located in the right B7. The clinical characteristics of the 66 cases of endotracheal and endobronchial leiomyoma reported in Japan are also discussed. The male to female ratio of this disease was 2:3. Middle-aged people were most, commonly affected. Usually, obstructive pneumonia or atelectasis, which develops distal to the lesion, causes respiratory symptoms and chest X-ray abnormality. However, 10% of cases were asymptomatic and 30% had a negative chest X-ray. There were 7 cases of endobronchial minute leiomyoma, measuring less than 10 mm in diameter. Of these, three cases had only hemoptysis and had no chest X-ray abnormality. In such cases fiberoptic bronchoscopy is may be the only useful procedure for the diagnosis of this disease.

, sputum, and fever. A chest roentgenogram revealed a nodular density in the left upper lung field with satellite lesions compatible with tuberculoma. Mycobacterium tuberculosis was detected from sputum. Five weeks after starting the treatment with 0.4 g/day of isoniazid, 0.45 g/day of rifampicin, and 0.75 g/day of streptomycin, she showed itching erythema in the trunk. The white blood cell count was 4,500/mm3 with 14% eosinophils, and serum transaminases were slightly increased (GOT 101 U/L, GPT 74 U/L). Although isoniazid and rifampicin were stopped, the erythema with exfoliation spread to her extremities, suggesting exfoliative dermatitis. The white blood cell count reached 15,990/mm3 with 68% eosinophils (10,810/mm3). Stimulation indices measured with the lymphocyte stimulation test (LST) were 109% with rifampicin, 140% with isoniazid, and 275% with streptomycin, suggesting streptomycin-induced allergy. After cessation of streptomycin, the symptoms gradually improved. After the reaction had subsided, the treatment with isoniazid, rifampicin, and ethambutol was resumed, but she showed no further adverse reactions. LST seems to be very useful to identify the drug or drugs responsible for the reactions occurred during the treatment by antituberculosis drugs.

and was found to have a 15 cm mass on the left side of his chest. Carcinoma was suspected but not proved, and a course of chemotherapy was administered. Two years later he was referred to Duke University Medical Center after he experienced a thromboembolic event in his left upper extremity. A CT scan and angiogram of the chest showed a large false aneurysm located 5 cm from the origin of the left common carotid artery and filling the anterior mediastinum. Additional history was obtained of multiple stab wounds to the left side of the chest 7 years before admission. The aneurysm was treated with sequential saphenous vein bypass grafting of right common carotid to left common carotid to left subclavian artery, followed by aneurysm thrombosis induced with wire coils. The differential diagnosis of a chest mass should include aneurysms of the great vessels.

for 3 weeks, was dull at the left base clinically, and had left lower zone consolidation on chest radiography. The pneumonia spread despite oral ampicillin and cloxacillin. Blood culture grew Listeria monocytogenes and white cell count showed a monocytosis. He responded to intravenous penicillin and gentamicin with complete X-ray clearance.

-year-old man with one year history of

and chest pain due to right upper lung cancer was scheduled for radical surgery. An echocardiogram and a lung scan showed a tumor mass in the left atrium, which was originating from carcinoma of the right upper lobe. Right pneumonectomy and atrial tumor extirpation were done successfully under the state of cardiac arrest using extracorporeal circulation with topical cooling by crushed ice in order not to spread the tumor cells into systemic circulation. He was extubated on the 1st postoperative day. But, thereafter he developed moderate dyspnea. On 7, 10 and 13th postoperative days he fell into the state of CO2 narcosis. Decreased capacity of the lung after the pneumonectomy and left phrenic nerve injury during the operation were considered factors causing the respiratory insufficiency. Patient was discharged 6 weeks later and continued to have consultations at the outpatient clinic.

The diagnosis of hypersensitivity pneumonitis was confirmed immunologically by detecting a precipitin to spores of Pholiota nameko but not to other antigens. After separation from the antigen along with an addition of corticosteroid therapy, the symptoms, inflammatory findings and a reduced level of PaO2 quickly subsided.

, chest pain and dyspnea. The chest roentgenogram revealed a large mass shadow in the right upper and middle lobes with atelectasis and pleural effusion. Massive polypoid tumor extending into the left atrium was diagnosed by computed tomography and two dimensional echocardiography. In order to prevent sudden death and cardiac failure, surgery was performed. At first, the polypoid tumor in the left atrium was removed with a partial resection of the left atrial wall under cardiac arrest using cardiopulmonary bypass. Then, a right pneumonectomy was performed. Episodes of embolism were not observed during surgery. His postoperative course was almost favorable. The size of the tumor in the right lung was 18 x 15 x 8 cm and the one in the left atrium was 6.5 x 4.5 x 3 cm, respectively. Pathological examination of the resected specimen revealed the evidences of large cell carcinoma extending into the left atrium. Local recurrence with S9 metastasis of the left lung were detected 6 months after surgery, and he died 6 months later. It is emphasized that the extended surgery using cardiopulmonary bypass was useful for both prevention of embolism and improvement of quality of life.

-year-old woman with diabetes mellitus was admitted to our hospital because of fever,

, and dyspnea. Coarse crackles were audible and respiratory sounds were weak in the right lung field. Laboratory examination revealed a high erythrocyte sedimentation rate, a high level of serum C-reactive protein, a high blood sugar level, and hypoxemia. A chest roentgenogram revealed cystic lesions with fluid levels, and an infiltration shadow in the right lung field. A chest computed tomographic scan revealed many cystic lesions with fluid levels and an infiltration shadow. Our diagnosis was infected emphysematous bullae. A tube was inserted percutaneously for drainage and to allow injection of antibiotics into the cystic lesion. The cystic lesion then vanished. Percutaneous drainage and washing with antibiotics can be used to treat infected emphysematous bulla that have thick closed cystic walls.

-year-old male was admitted to our hospital because barium esophagograpm showed an esophago-bronchial fistula with an esophageal deverticulum. He has had frequent episodes of
cough
at drinking water since childhood. A chest CT scan showed mild inflammatory change and bronchiectasis in the right S6. Division of the fistula by video-assisted thoracoscopic surgery (VATS) was performed. There was no evidence of inflammation and adherent lymph nodes around the fistula. This case was diagnosed as a congenital esophago-bronchial fistula by operative findings and clinical course. The fistula was dissected easily and divided by an auto-suturing instrument. The affected lung could be preserved. The patient was discharged on the 10th postoperative day. VATS is an effective treatment for the patients of congenital esophago-bronchial fistula (Braimbrige type I and II), if they are not accompanied with pulmonary abscess or pleural empyema.
