The incidence of
NTM
pulmonary infections increasingly is recognized in patients with CF. This may reflect the increasing longevity of this population with increased environmental exposure time, a high index of suspicion, and/or some as of yet unidentified predisposing factor(s). The most common species of
NTM
in CF is MAC, followed by M. abscessus. The authors recommend that adult patients with CF be screened for the presence of
NTM
pulmonary secretions on a regular basis. Positive cultures are likely to indicate disease if they are multiple or if a patient has clinical evidence of pulmonary disease exacerbation (increased
cough
, increased purulence of secretions, or systemic manifestations such as fever and weight loss) that is not responding to conventional antibiotic therapy. CF patients who do not respond to treatment for the usual organisms should be re-evaluated for the presence of
NTM
and treated with a macrolide-containing regimen directed against the identified
NTM
if diagnostic criteria are met. Novel treatments with cytokines and intermittent dosing of antibiotics are currently under investigation.

HIV is a major health problem in Thailand. These patients are vulnerable to opportunistic infections, especially Mycobacterium tuberculosis and MAC infection. However,
NTM
was considered a rare disease in Thailand before the AIDS era. In this study, there were 38 HIV seropositive patients with
NTM
(other than MAC) identified from clinical specimens during the 3 year period 1998-2000 at Siriraj Hospital, which has a higher prevalence than the previous report. Among these patients, 29 cases were likely to have had definite infection from
NTM
, 5 cases possibly had
NTM
as a pathogen, and 4 cases had
NTM
as colonization. The most common site of infection was the lung (87%) and most common symptoms were
cough
(62.2%), fever (34.2%), weight loss (42.1%), and lymphadenopathy (5.3%). The outcome was poor because many
NTM
are not susceptible to standard medication for tuberculosis which is the empirical treatment for the majority of HIV seropositive patients with a clinical finding suspected of mycobacterial infection. The fatality rate was as high as 58.6 per cent. Awareness of
NTM
as a potential pathogen in HIV seropositive patientsand adjustment of medications even before the availability of culture results may improve the outcome of treatment of
NTM
infection in HIV seropositive patients.

We present a case of hypersensitivity pneumonitis in a 46-year-old female working at a yacht manufacturing company. She reported a 2-month history of progressive dyspnea, chest tightness, and daytime, nocturnal, and exertional
cough
in temporal relationship to work where she was exposed to chemicals involved in the manufacture of yachts. Treatment with systemic antibiotic therapy, inhaled bronchodilators, and inhaled corticosteroids provided minimal relief of symptoms. Spirometry revealed a restrictive defect and a chest x-ray demonstrated a diffuse interstitial pattern. She improved on oral corticosteroids and with avoidance of her work environment had resolution of her symptoms and normalization of her spirometry. Among the various chemicals the patient was exposed to, the most likely causative agents for her symptoms were
dimethyl phthalate
and styrene. Although the specific chemical or antigen could not be determined, the history and objective findings are consistent with occupational hypersensitivity pneumonitis. This represents a case of hypersensitivity pneumonitis related to the manufacture of yachts.

We herein report our experience with patients who had nontuberculous mycobacterial lung disease (
NTM
disease) accompanied by organizing pneumonia (OP). Out of 98
NTM
disease patients who had undergone a biopsy or surgical resection, 11 patients had OP that was revealed histologically. After excluding six patients who had OP-related diseases (idiopathic interstitial pneumonia, rheumatoid arthritis, etc.), the remaining five patients were studied. Two of them (a 73-year-old man and a 66-year-old woman) showed common clinical feature: acute-onset symptoms of
cough
and fever, infiltrating shadows and dramatic improvement following treatment with a corticosteroid and anti-mycobacterial therapy. Our cases demonstrate that
NTM
disease is sometimes accompanied by OP histologically, and some such cases show common clinical features.

Patients with nontuberculous mycobacterial lung disease (NTM-LD) often have significant exercise intolerance and poorer health-related quality of life (HRQL). The goals of treatment for
NTM
-LD should include reducing the severity of symptoms, improving HRQL, and reducing acute exacerbations. Nonpharmacological treatment, including pulmonary rehabilitation program and optimal nutritional strategy, should be one part of treatment for
NTM
-LD. A pulmonary rehabilitation (PR) program can comprise education, airway clearance techniques instruction, exercise training program, and inspiratory muscle training (IMT). Airway clearance techniques can improve the volume of sputum expectorated,
cough
symptom, breathlessness, and HRQL. Exercise training can improve exercise capacity and HRQL, and reduce acute exacerbations and dyspnea. Clinical benefits of IMT remain controversial but high-intensity IMT has been shown to be effective in increasing respiratory muscle strength with concurrent improvement of HRQL and exercise capacity. Body weight and muscle mass loss are common in patients with
NTM
-LD. An adequate protein and caloric diet combined with antioxidant nutrients might be the most appropriate dietary strategy. Comprehensive treatment for
NTM
-LD should include the combination of both pharmacological and nonpharmacological treatments. The management programs should be tailored to the individual's condition.

The objective of this study was to administer commonly used tools, the Center for Epidemiological Studies Depression Scale (CES-D) and the Hospital Anxiety and Depression Scale - Depression subscale (HADS-D), to screen for depressive symptoms in patients with nontuberculous mycobacterial pulmonary disease (NTM-PD). In addition, we sought to identify whether differences existed in the prevalence of depressive symptoms as assessed by CES-D and HADS-D, and by various predictors of depression. The presence of depressive symptoms in 95 patients with
NTM
-PD was assessed using the CES-D and HADS-D. Data regarding age, body mass index, pulmonary function, dyspnea,
cough
, and exercise capacity were obtained to examine their independent contribution as predictors of depressive symptoms. The prevalence of depressive symptoms was 37.9% based on CES-D and 26.3% based on HADS-D. The prevalence of depressive symptoms based on CES-D and HADS-D revealed significant differences between the two instruments. Analysis suggested that the presence of
cough
is a significant predictor of depressive symptoms as assessed by both CES-D and HADS-D. Countermeasures are necessary because some patients with
NTM
-PD disease have depressive symptoms. It is possible that assessment of the prevalence of depressive symptoms differs in accordance with the screening tool used.
