To evaluate the features of tuberculous infection in HIV+ patients from our area we reviewed the 50 cases of seropositive patients admitted to a provincial hospital from 1985 to June 1989. TB was diagnosed in 18 of the 50 patients; mean patient age was 27 years, with a male predominance. PDA was the major risk practice. The most common localization were lymph nodes; 95% had fever and weight loss and 39% had nightly perspiration and
cough
; cervical lymphadenopathy and hepatosplenomegaly were found in more than 50%.
Tuberculin
skin test was negative in 75% of cases. The diagnosis of TB represented the diagnosis of AIDS for 66% of patients. 25 of the 50 seropositive patients met the criteria for AIDS, and 68% of these had TB. TB is a common infection in HIV+ patients from our area, and it is the first cause of the diagnosis of AIDS. The index of suspicion of TB in these patients should be high, as clinical features are often nonspecific. However, it is preventable and curable superinfection.

From January 1984 to October 1990, 140 of 392 (35.7%) patients with the acquired immunodeficiency syndrome (AIDS) were found to have had tuberculosis. One hundred and sixteen were intravenous drug abusers and 16 were homosexual men. Fever,

Tuberculin
skin tests were negative in 74% and 55% had intraabdominal lymphadenopathy. The chest radiographs showed hilar lymphadenopathy and lower lobe interstitial or alveolar infiltrates, but rarely cavitation. Forty-one of our patients had pulmonary tuberculosis, 38 had extra pulmonary and in 61 it was disseminated. In 80 cases tuberculosis was the presenting feature of AIDS. Tuberculosis usually responded well to chemotherapy.

Tuberculin
skin tests were performed on 1,146 patients out of an active patient population of 3,112 patients over a ten-year period in a rural western New York State family practice. There were 19 new positive tuberculin reactions and six cases of active tuberculosis discovered in the population. All but two of the patients with new positive tuberculin reactions and all of the patients with new cases of active tuberculosis were members of at least one of the following high-risk groups: (1) contract with an individual with active tuberculosis, or a positive family history of the disease; (2) immigrants to the United States; (3) a history of alcohol abuse; (4) having lived in an institutional setting; (5) health care personnel; and (6) having signs and symptoms of tuberculosis (

, anorexia, weight loss, positive chest roentgenogram). All new cases of active tuberculosis were diagnosed because of symptoms. No asymptomatic person with a positive tuberculin test developed active disease during the study period. The positive predictive value of using risk factors to prescreen for the tuberculin skin test was 16 percent. The negative predictive value of not screening people without risk factors (because they will have a negative tuberculin test) was 99.8 percent.

Immune response of infants vaccinated under Expanded Programme on Immunization (EPI) was evaluated for measles, poliomyelitis, and tuberculosis in Ifo/Otta area of Ogun State and Badagry area of Lagos State, Nigeria. In the prevaccination evaluation of measles antibody, 59 per cent were protected and 41 per cent were at risk in Ifo/Otta area, while 49 per cent were protected and 51 per cent were at risk in the Badagry area. After measles vaccination, 89 per cent of those evaluated seroconverted and 11 per cent did not in Ifo/Otta area, while in Badagry area, 86 per cent of those evaluated seroconverted and 14 per cent did not. For polio neutralizing antibody evaluated at post-immunization, 91 per cent seroconverted, while 9 per cent did not in Ifo/Otta area, while in Badagry area 66 per cent seroconverted and 34 per cent did not.

test was used to evaluate the cellular response to BCG vaccination against tuberculosis. 64 per cent were found protected, while 18 per cent were at risk in both areas examined and 18 per cent dropped out. Examination by thoracoscopy, using flexible fiberoptic bronchoscope, was performed in 43 patients with pleural effusion according to our reported method. In these patients, 14 cases were diagnosed as tuberculous pleuritis. These 14 cases were investigated with respect to clinical and thoracoscopic findings. Their mean age was 38.1 years, and 11 cases were younger than 50 years old (78.5%). The male:female ratio was 2/1. Clinical symptoms recognised were fever (100%),
cough
(64.2%), chest oppression (50%) and sputum (35.7%). In 5 cases, gastrointestinal symptoms were recognized such as vomiting, abdominal pain, and diarrhea.
Tuberculin
reaction was positive in all patients with tuberculous pleuritis on admission. With respect to the thoracoscopic findings of tuberculous pleuritis, yellow-white miliary granulomas were observed on the parietal pleura in 12 cases (85.7%), and this characteristic finding was especially observed at the apex. Biopsy specimens, obtained from these miliary granulomas on the parietal pleura showed histological findings of tubercles. In the other 2 cases, generalized reddening of the entire parietal pleura was observed, with no yellow-white miliary granulomas. Biopsy specimens obtained from the reddened lesions on the parietal pleura showed histological findings of tubercles, and these 2 cases were also diagnosed as tuberculous pleuritis. These results indicate that this method may be very useful for the diagnosis of tuberculous pleuritis in patients with pleural effusion.

The prevalence of respiratory disease in a Midwest Hispanic (mostly Mexican) migrant farm worker population was investigated. Chronic respiratory symptoms (
cough
, wheezing, sputum production) in adult workers (n = 354) were elevated (8.5%, 6.2%, 6.5%, respectively) and were accompanied by physiologic abnormalities as determined by pulmonary function testing. Over 15% of the adult cohort exhibited a FEV1/FVC < 75, and over 14% had FEF25-75 values which were less than 60% of predicted. The observed airflow obstruction of both large and small airways was not explained by cigarette usage (43%) in the adult cohort (current/past smokers).
Tuberculin
skin tests (TST) were positive (> or = 10 mm) in 55/195 men and 35/123 women for a total prevalence of 28.3%. No case of active tuberculosis (TB) was identified by either chest X-ray (CXR) or sputum cultures (in selected cases). In contrast to adult farm workers, who were predominantly born in Mexico (70%), only 36% of adolescent workers (age 11-18 years, n = 107) were born in Mexico with only 7.5% exhibiting TST positivity. Airflow obstruction of large airways (5.8%) and small airways (12.9%) were also less common in adolescents than adults. In summary, these studies document respiratory dysfunction in Hispanic migrant farm workers in Indiana and highlight the need to closely monitor the respiratory health of this high-risk population.

In summary, despite the major benefits conferred by aerosol therapy, the risk of transmission of M tuberculosis remains a concern. The most effective approach to preventing nosocomial transmission of respiratory infections such as tuberculosis is to prevent disease in patients at risk, particularly those receiving aerosol therapy.
Tuberculin
skin tests should be applied to all HIV-infected persons and chemoprophylaxis administered to those with greater than or equal to 5mm reaction. Early detection and treatment of tuberculosis are essential to prevent transmission both inside and outside of health-care settings.
Cough
-reduction measures should be taken when giving pentamidine aerosol, and patients who do
cough
should cover their mouths. Proper ventilation of treatment areas with negative air pressure and at least 10 air changes per hour, and more if feasible, is essential. HEPA filters should be installed to cleanse exhaust air from treatment rooms, and air should not be recirculated. The use of UV light to disinfect air is recommended only for special situations. Finally, personal protective devices should be worn by health-care workers who are exposed to patients receiving aerosol therapy when other measures do not offer adequate protection.

We describe a 26 year old woman treated for near a year because of deafness, purulent otitis,

and purulent skin lesions. She was treated by otolaryngologist. Sputum examination for acid fast bacilli and chest x-ray were not done a that time. In hospital chest x-ray revealed a cava in left lung and tuberculous bacilli were found in sputum and in content from ear.

Tuberculosis in patients undergoing maintenance hemodialysis therapy presents a number of diagnostic and therapeutic challenges. This study was designed to assess the specific diagnostic and therapeutic features of dialysis-associated tuberculosis in a clinical setting. A total of 55 patients were enrolled in this retrospective study over the 5-year period from 1996 to 2000. Diagnostic techniques included questionnaire, clinical examination, chest X- ray, tuberculin skin test, Mycobacterium tuberculosis (MT) screening on biological fluids and tissue biopsy. skin testing was negative in 5 cases. Tuberculosis was located in the lung in 2 cases, pleura in 2 cases, peritoneum in 1, and lymph node in 1. Isolation of the MT and confirmation by tissue biopsy was performed in only one case. Appropriate polychemotherapy was successful in 5 of 6 cases. Morbidity and mortality of tuberculosis in dialysis patients is closely related to early detection and treatment. Therapy is often based on strong presumptive evidence without definitive diagnosis.
