Urinary continence is maintained by the smooth-muscled system of the "internal sphincter". The striated external sphincter is not primarily responsible for continence. One of the chief functions of the external sphincter is the initiation of voluntary micturition. Its other functions are: random interruption of the urinary stream, reflex control during an increase in intra-abdominal pressure (such as
coughing
,
sneezing
, larghing, lifting) complete emp;ying of the urethra after micturition and stabilization of the posterior urethra in the urogenital diaphragm. After prostatectomy an intact external sphincter is important in order to support the smooth-muscled system which continues to be primarily responsible for continence to function as efficiently as possible. In approximately 90% of all postprostatectomy incontinences the external sphincter is intact and this cames a good prognosis following our correcture surgery without the need for prostheses. Only in rare post-prostatectomy incontinence cases (aprox. 10%) is the external sphincter also injured. Incontinence surgery according to our method has not been satisfactory in these cases.

Intrapleural pressure, the tracheal air flow and tidal volume were recorded simultaneously in pentobarbital-anaesthetized dogs and changes occurring in them during defensive reflexes elicited by mechanical stimulation of the mucosa of different parts of the respiratory tract were evaluated quantitatively. The results show that, in addition to
coughing
and
sneezing
provoked by inserting a nylon fibre into the tracheobronchial region, the larynx and the nose, further respiratory reflexes described in other mammals also appear in these animals. Mechanical stimulation of the epipharynx with a fine polyvinylchloride catheter, for instance, also produces in dogs an aspiration reflex characterized by sniff-like inspiratory efforts without subsequent active expiration. Touching the vocal folds, however, produces an expiration reflex consisting of expiratory efforts without preceding inspiratory effort. The character of all these reflexes is typical and closely resembles their character in cats. Stimulation of the various parts of the respiratory tract sometimes evokes an apnoeic reaction instead of typical respiratory defensive reflexes.

Intranasal and intraperitoneal exposure of English ferrets (Mustela putorius furo L) to infectious bovine rhinotracheitis virus caused acute and chronic infections of the respiratory tract. The clinical syndrome was characterized by

, and anorexia from postexposure days (PED) 3 to 7. Mucopurulent exudate was observed in the posterior nares and pharyngeal area of ferrets euthanatized on PED 4 and 8. The virus was readily recovered from the turbinates, respiratory tract epithelium of the pharynx, retropharyngeal lymph nodes, trachea, lungs, and spleen of animals euthanatized on PED 4, but only from the respiratory tract epithelium of the pharynx in ferrets euthanatized on PED 8 and 12. Results of histopathologic studies revealed an acute suppurative pharyngitis in animals euthanatized on PED 4 and 8. Recrudescence of chronic infection could be elicited by daily intraperitoneal injections of 4.0 mg of dexamethasone. However, daily administration of 2.0 mg of dexamethasone intraperitoneally did not cause more severe clinical disease. Results of serologic studies revealed serum antibody profiles comparable with those expected in experimentally exposed cattle.

Common otolaryngologic symptoms such as

may not be manifestations of disease of the upper respiratory tract. Two cases are reported in which these symptoms were the first evidence of tic-like disorders. A short discussion of one such disorder, Gilles de la Tourette's syndrome, is presented. The entity of paroxysmal
sneezing
is also mentioned. It is pointed out that, in the absence of otolaryngologic disease, these disorders may first present to an otolaryngologist for diagnosis.

Intranasal administration of a 4% solution of cromolyn sodium for the treatment of ragweed hay fever was tested in an 8-week double-blind matched-pair study involving 66 patients. Patients on active drug received 5.2 mg into each nostril 6 times daily; control patients received a placebo spray. The treated group showed a significant reduction in mouth breathing (p less than 0.001), stuffy nose (p less than 0.002), runny nose (p less than 0.003), and postnasal drip (p less than 0.035). Patients receiving the active drug also reported fewer
sneezing
episodes (p less than 0.003) and nose blowing episodes (p less than 0.015). One patient using cromolyn solution developed nasal ulceration, tongue swelling,
coughing
, and wheezing. Other side effects were minimal and occurred with equal frequency in both groups. In the treated group relief of symptoms was most marked in patients with high preseasonal levels of IgE ragweed antibody. Intranasal 4% cromolyn solution appears to be an effective drug for the treatment of ragweed hay fever; measurement of the preseasonal level of IgE ragweed antibody is a useful screening test to identify patients most likely to achieve a maximal beneficial response to treatment.

To get an optimal function after prostatectomy an intact sphincter externus becomes important for the smooth urethral muscular system which is further responsible for the continence. About 90% of all patients with a postprostatectomy incontinence have an intact sphincter externus. In these cases we have a high incidence of incontinence cure. Only in 10% of incontinence after prostatectomy the sphincter externus is damaged. In those cases the incontinence operation is inadequate.

Intermittent claudication from peripheral vascular disease is sometimes difficult to distinguish from similar claudication due to degenerative disease of the lumbar spine. In the present study 26 patients with vascular disease were compared with 23 patients with lumbar degenerative disease. Assessment was by clinical and radiological examination. In the vascular group characteristic distinguishing features were: abnormal foot pulses, arterial bruits, relief of symptoms by standing, a constant claudicating distance and stocking sensory loss. Treatment with beclomethasone dipropionate aerosol (BDA), 50 mug four times daily in each nostril, was compared with placebo therapy in a double-blind non-crossover trial of 30 matched patients with allergic rhinitis induced by ragweed pollen. The trial was started at the beginning of the ragweed season and continued for 42 days. Response to treatment was assessed from information on daily diary cards, weekly objective measurements of nasal patency and measurement of total eosinophil count (TEC) before treatment and at week 4. Patients in the BDA group had significantly less (P less than 0.05)

, rhinorrhea and nasal stuffiness at 36 days,

at 10 days and antihistamine consumption at 17 days. There was no significant difference between the groups in eye symptoms, nasal airway inspiratory resistance, maximum inspiratory nasal flow or TEC. Overall comparison with previous pollen seasons by the patients indicated moderate to great improvement in 86% of the BDA group and in 13% of the placebo group (P less than 0.01). Minor side effects were noted by two patients in each group.
