We hereby report a rare case of a mature type
teratoma
that perforated the lung and combined with aspergilloma in the tumor cyst. The patient was a 48-year-old female, who complained of
coughing
and sputum. The chest roentogenography and chest CT suggested aspergilloma. The patient was operated upon. From the intraoperative and pathological findings, a mature type mediastinal
teratoma
was found to have perforated the bronchus and lung, and combined with aspergilloma in the cyst of the tumor. The tumor was resected, the drainage bronchus was closed. The mediastinal
teratoma
and aspergilloma have not recurred for two years and four months since the operation.

of the lung (3 cases were presented and 17 reviewed) were analyzed. There were 13 males and 7 females, the age ranging from 16-47 years. The primary symptoms were:
cough
(19 cases, 95%), blood sputum or hemoptysis (16 cases, 80%), chest pain (12 cases, 60%), fever (7 cases, 35%) and expectoration of hair (4 cases, 20%) etc. The histogenesis, diagnosis and treatment of the disease were discussed.

Approximately 8% of all mediastinal tumors are benign teratomas. We reviewed 86 cases of benign
teratoma
seen at the Mayo Clinic from 1930 through 1981. The mean age of the patients was 28 years and the sex distribution was approximately equal. The most common symptoms were chest, back, or shoulder pain, dyspnea, and
cough
, but 36% were asymptomatic at the time of presentation. Chest roentgenograms showed a well-circumscribed anterior mediastinal mass which often protruded into one lung field. Detectable calcification was observed in 22 patients: a calcified tumor wall in seven, bone or teeth in the mediastinum of seven, and nonspecific calcifications in eight. Surgical excision remains the best means of diagnosing and treating this benign tumor. Though the tumors are histologically benign, they may present difficult surgical problems because of the vital structures involved. Since 1952 there has been a change in the clinical presentation of patients with this entity: More patients are asymptomatic and have smaller tumors and fewer complications than prior to 1952.

and hemoptysis was found to have a metastatic trophoblastic tumor in her lungs and in a supraclavicular lymph node. A regressed primary
teratoma
of the ovary was considered the most likely origin of the tumor. Sputum cytology showed malignant cells, many of which were in elongated fiberlike forms and some of which had orange-staining cytoplasm. These cytologic appearances of choriocarcinoma are described and compared with the brief descriptions available in the literature. The possibility of misdiagnosis as squamous carcinoma is emphasized. The correct diagnosis is of particular importance because choriocarcinoma of gestational origin, which is similar cytologically to that of teratomatous origin, responds well to chemotherapy.

During the last 8 years, 70 cases of testicular tumor were treated in our department. Of them 2 cases had brain metastasis. Case 1; A 37-year-old male was admitted with the chief complaint of

. Retroperitoneal lymph node and lung metastases were discovered (stage IIIB, pT4aN1M1). Resected right testis was diagnosed as embryonal carcinoma,

and STGC histopathologically. After 3 courses of PVB (cisplatinum, vincristine, bleomycin) chemotherapy right hemiplegia occurred and computerized tomographic (CT) scan revealed brain metastasis. His general condition degraded rapidly and he died of brain herniation 3 months after orchiectomy. Case 2; A 32-year-old male was admitted because of right testicular swelling and lung metastases (stage IIIB, pTiN0M1). Pathological examination revealed embryonal carcinoma, yolk sac tumor and
teratoma
. After 4 courses of chemotherapy with cisplatinum (CDDP), vincristine, methotrexate, peplomycin, and etoposide all lung metastases were disappeared. A few months later left hemiplegia by brain metastasis appeared suddenly. Four additional courses of high dose chemotherapy and resection of brain metastasis was performed. Complete remission continued for 13 months. The prognosis of testicular tumor with brain metastasis was very poor. During the last 8 years, 21 cases of testicular tumor with brain metastasis were reported in the Japanese literature. A follow-up study of prognosis in the literature was performed and discussed.

with elevated serum CA19-9 level is reported. A 50-year-old woman admitted to our hospital complaining of
cough
and chest pain. Chest X-ray showed an abnormal mass shadow with retention of the right pleural effusion. Her serum CA19-9 level was high (204.4 U/ml), while serum AFP, CEA, HCG levels were normal. On lateral thoracotomy, an anterior mediastinal tumor perforating into the right lung was revealed and the total resection of the tumor with adherent part of the right lung was performed. Postoperatively, her serum CA19-9 level returned to normal. Histological examination disclosed a mature
teratoma
consisting of skin, pancreatic tissue, cartilage, bronchial epithelium, etc. Immunohistochemical staining were positive in the bronchial epithelium and bronchial gland but was negative in the pancreatic tissue of the tumor. This is a rare case of mediastinal mature

9 patients (8 males 1 female, middle age 24.3 years, range 20-41) affected by primary mediastinal germ cell tumor were surgically treated in our department. They were 4 seminomas, 2 embryonal carcinomas, 1 malignant
teratoma
and 2 benign teratomas. Three patients were asymptomatic;
cough
, dyspnea and chest pain were the most frequently observed symptoms. The staging work-up did not show signs of metastatic disease in the malignant types. Benign teratomas underwent complete excision. In one patient suffering from seminoma radiotherapy and chemotherapy caused complete remission of the disease. In the others cases remission of the neoplasm was obtained by resection and adjuvant therapy in 3 cases, by neoadjuvant treatment and excision of the residual mass in 3 cases. Among the patients suffering from seminoma, 2 are dead at 60 months since initial treatment and 2 are alive at 132 and 120 months respectively. Among the patients with malignant nonseminomatous tumours, 2 are alive at 60 and 36 months and 1 patient is dead at 13 months. Two patients with benign
teratoma
are alive at 189 and 168 months respectively. At present a multimodality treatment including surgery, radiotherapy and cisplatin-based combination chemotherapy, give the most satisfactory results in the treatment of malignant mediastinal germ cell tumours.

, the specimen extirpated by thoracotomy revealed macroscopically and microscopically foreign body granuloma originated from gauzes. He had undergone segmentectomy of right upper lobe for tuberculosis 28 years ago, so the gauzes were thought to be left in right thoracic cavity at the operation. The foreign body granuloma originated from gauzes are iatrogenic and few cases were reported previously, however, whirl like pattern demonstrated by MRI is retrospectively considered to be an important diagnostic finding in this case.
