Children with human immunodeficiency virus (HIV) frequently have recurrent otitis media, chronic rhinorrhea,
parotitis
,
cough
and other common pediatric otolaryngologic problems. As these complaints often occur before more unusual opportunistic infections or pulmonary conditions prompt a diagnosis of acquired immunodeficiency syndrome (AIDS), members of our specialty are liable to see HIV-positive children before infection with the virus has been recognized. Children with HIV infection are also likely to be referred to us after diagnosis, as is any immunosuppressed child with otolaryngologic infections. These children may require procedures such as bronchoscopy, sinus irrigations or tympanocentesis. The subject of this review is the natural history of pediatric HIV infection with special emphasis on otolaryngologic manifestations and recommendations for safe techniques of examination and treatment.

Within three years we have observed three patients with
parotitis
after measles-mumps-vaccination, one child with idiopathic thrombocytopenic purpura, one with meningitis, and one patient with a preexisting severe cerebral damage died of central vasomotor and breath regulation dysfunction 20 days after the vaccination. Their case histories are described here, and the literature dealing with this question is reviewed. The following incidences of side effects may have to be accepted: "vaccination measles" (fever, rash, conjunctivitis,
coughing
) 5%, parotid swelling 1%. Furthermore, there are reports and observations on 38 patients who have developed ITP after vaccination, and on 8 other children who developed meningitis. From their lumbar liquor, mumps- or vaccine mumps viruses have been cultured. There are, as yet, no markers that would permit a typing of these viruses. Parents will have to be informed of these possible side effects and observations. In addition to that further information is not necessary on other very rare observations with as yet not established causal relationship with the vaccination. The measles-mumps-vaccination correctly is regarded as to be of low risk. Its usefulness is apparent when the frequency of side effects discussed here is compared to the incidence of grave complications of wild measles and mumps infections.

Fundamental and clinical studies on cefpiramide (CPM), a new semisynthetic cephalosporin were performed and the following results were obtained. Antibacterial activity The antibacterial activity of CPM was investigated in comparison with those of CTT, CPZ, CEZ, LMOX and CFS. Against clinical isolates of S. aureus, CPM was superior to CTT and LMOX, but almost similar to CPZ and inferior to CEZ. Against E. coli, K. pneumoniae, P. mirabilis and S. marcescens, CPM showed the activity almost similar to that of CEZ, but inferior to those of the others. On the contrary, the activity of CPM against P. aeruginosa was satisfactory and was superior to those of CTT, CPZ and LMOX, but slightly inferior to that of CFS. Blood level and urinary recovery Twenty mg/kg of CPM was given intravenously at one shot to 3 patients. The mean serum levels of CPM were 116.9 micrograms/ml at 30 minutes, 90.5 micrograms/ml at 1 hour, 71.1 micrograms/ml at 2 hours, 55.8 micrograms/ml at 4 hours, 24.9 micrograms/ml at 6 hours, 19.3 micrograms/ml at 9 hours and 12.1 micrograms/ml at 12 hours after administration, respectively. The mean half-life was very long and the value was 3.85 hours. The urinary recovery rates in 2 cases were 18.31 and 21.47% respectively up to 12 hours after administration. Clinical results and side effects CPM was given intravenously to 30 diseases including 11 cases of bronchopneumonia, 3 cases of bronchopneumonia and pleurisy, 2 cases of bronchitis, 4 cases of purulent tonsillitis, 5 cases of pyelonephritis and each one case of pyothorax,
parotitis
, cellulitis, otitis media and salmonellosis. CPM was effective in 29 out of 30 cases, and the effective rate was 96.7%. As side effects, 2 cases of fever and 1 case of
cough
were observed, but no abnormality in clinical laboratory findings was observed.

Swine origin influenza was first recognized in the border area of Mexico and United States in April 2009 and during a short span of two months became the first pandemic. The currently circulating strain of swine origin influenza virus of the H1N1 strain has undergone triple reassortment and contains genes from the avian, swine and human viruses. It is transmitted by droplets or fomites. Incubation period is 2 to 7 days. Common clinical symptoms are indistinguishable by any viral respiratory illness, and include fever,

, sore throat and myalgia. A feature seen more frequently with swine origin influenza is GI upset. Less than 10% of patients require hospitalization. Patients at risk of developing severe disease are - younger than five years, elderly, pregnant women, with chronic systemic illnesses, adolescents on aspirin. Of the severe manifestations of swine origin influenza, pneumonia and respiratory failure are the most common. Unusual symptoms reported are conjunctivitis,
parotitis
, hemophagocytic syndrome. Infants may present with fever and lethargy with no respiratory symptoms. Diagnosis is based on RT PCR, Viral culture or increasing neutralizing antibodies. Principle of treatment consist of isolation, universal precautions, good infection control practices, supportive care and use of antiviral drugs. Antiviral drugs effective against H1N1 virus include: oseltamivir and zamanavir. With good supportive care case fatality is less than 1%. Preventive measures include: social distancing, practicing respiratory etiquette, hand hygiene and use of chemoprohylaxis with antiviral drugs. Vaccine against H1N1 is not available at present, but will be available in near future.

Garcinia dulcis (Roxb.) Kurz is a tropical fruit tree native to Southeast Asia where it has a long history of use as a traditional medicine for the treatment of ailments such as lymphatitis,

, and sore throat. Despite its medicinal values, this plant is not well known and rarely found nowadays. Research on the phytochemical constituents and biological activities of G. dulcis have demonstrated that various parts of the plant contain an. abundance of bioactive compounds mainly xanthones and flavonoids, with significant pharmacological properties such as anti-atherosclerosis, anti-bacterial, anti-cancer, anti-hypertension, and anti-malarial. In the present review, current knowledge of the phytochemistry of G. dulcis and biological activities of its active constituents based on the available literature are summarized in order to explore application potentials and prospective research works on this plant.

Coronavirus disease 2019 (COVID-19) has spread exponentially across the world. The typical manifestations of COVID-19 include fever, dry
cough
, headache and fatigue. However, atypical presentations of COVID-19 are being increasingly reported. Recently, a number of studies have recognized various mucocutaneous manifestations associated with COVID-19. This study sought to summarize the available literature and provide an overview of the potential orofacial manifestations of COVID-19. An online literature search in the PubMed and Scopus databases was conducted to retrieve the relevant studies published up to July 2020. Original studies published in English that reported orofacial manifestations in patients with laboratory-confirmed COVID-19 were included; this yielded 16 articles involving 25 COVID-19-positive patients. The results showed a marked heterogeneity in COVID-19-associated orofacial manifestations. The most common orofacial manifestations were ulcerative lesions, vesiculobullous/macular lesions, and acute sialadentitis of the parotid gland (
parotitis
). In four cases, oral manifestations were the first signs of COVID-19. In summary, COVID-19 may cause orofacial manifestations that might be the initial features in several cases. However, the occurrence of orofacial manifestations in COVID-19 seems to be underreported, mainly due to the lack of oral examination of patients with suspected and/or confirmed COVID-19. Oral examination of all suspected and confirmed COVID-19 cases is crucial for better understanding and documenting COVID-19-associated orofacial manifestations.
