A reversed-phase high-performance liquid chromatography (HPLC) method with dual optical rotation/UV absorbance detection has been developed for the determination of enantiomeric purity of ephedrine hydrochloride and pseudoephedrine hydrochloride using an achiral column. The method gave a correlation coefficient of 0.9997 for the plot of log(optical rotation response) versus log (concentration) over the range of 0.06-10 mg ml(-1) of (+)-ephedrine hydrochloride (20 microliters injection). The limit of detection was 1.0 micrograms. Enantiomeric purity is shown to be most readily determined by measuring optical rotation, alpha, and absorbance, A, responses for standard and unknown samples, and using the equation (alpha/A)u/(alpha/A)s = (2xu - 1)/(2xs - 1), where x is the

nevus
syndrome (BRBNS) lesions. BRNBS is a rare syndrome characterized with cavernous hemangiomas involving the skin and GI tract. We report the case of a 37-year-old woman, with known BRBNS, who acquired an intractable
cough
with a radiograph revealing multiple pulmonary nodules. A bronchoscopy demonstrated various bluish, raised, and hypervascular lesions characteristic for BRBNS involving the right mainstem bronchus and segmental bronchi. To our knowledge, this is the first report of endobronchial involvement with BRBNS.

We report a well-documented case of a 28-year-old woman, gravida 3, para 1, undergoing termination at 22 weeks for fetal congenital malformations with concurrent partial hydatidiform mole and choriocarcinoma. A fetal ultrasound showed spina bifida, and an elective termination was performed. One month later, the patient presented with vaginal bleeding,

, and shortness of breath. A chest computed tomographic scan revealed multiple lung nodules. A pelvic ultrasound was consistent with retained products of conception, and endometrial sampling showed an atypical trophoblastic proliferation consistent with choriocarcinoma. The serum beta-human chorionic gonadotropin level was greater than 100,000 IU/mL. The placental pathology reviewed at our institution showed microscopic foci of choriocarcinoma arising in a partial hydatidiform mole. Flow cytometry performed on paraffin-embedded tissue showed a triploid peak, which confirmed the partial molar nature of this gestation. Immunohistochemistry showed nuclear p57 expression in the partial molar villous trophoblastic and stromal cells, but not in the severely atypical trophoblasts, further supporting the diagnosis of intraplacental choriocarcinoma distinct from the partial
mole
. Although the molecular pathogenesis remains to be elucidated, this report provides additional evidence that choriocarcinoma may arise directly from partial molar gestations. Moreover, it emphasizes the importance of thorough sampling of placentas with partial hydatidiform mole in search of minute foci of choriocarcinoma because they might represent a potential source of metastatic gestational trophoblastic disease.

We examined the use of telemedicine for improving access to care in a work-site clinic. A prospective study of 100 patients was conducted over a four-month period in a work site that housed 700 employees. Sinusitis (10 visits), upper respiratory tract infections (9 visits), otitis media (9 visits), hypertension (9 visits) and back pain (8 visits) were the most common reasons for the visits. In 99 visits, clinicians were of the opinion that the telemedicine visit felt similar to a face-to-face visit. For most of the visits (67), patients strongly agreed or agreed that telemedicine had a positive effect on their relationship with the health-care provider. The otoscope, microscope and stethoscope telemedicine peripherals were important in aiding diagnosis (and ruling out other causes) in about 55% of the visits (upper respiratory tract infection, sinusitis, otitis media,

syndrome (BRBNS) and gave amitriptyline for her pain. There are only 200 cases of BRBNS in the literature, and central nervous system involvement is rarer still. The syndrome involves multiple cutaneous and visceral venous malformations. Most appear to be sporadic though a few have autosomal dominant inheritance. Although rare, BRBNS represents an important differential diagnosis for patients presenting with multiple and/or multisystem vascular malformations.

Air curtain is an efficient device for cutting off airflow and confining contaminants. Inspired by the ability, a circulated air curtain composed of end-to-end plane jets generated by a relay of air pillars is proposed to confine exhaled contaminants in this study. Furthermore, the optimization study of computational fluid dynamics (CFD) is conducted to explore cutting-off performance and find better design parameters under different conditions, i.e., human-curtain distance, enclosure shape, jet velocity from air pillar, and exhalation modes. The multidirectional blockage and vortex-like rotative transmission routes of exhaled airflow are observed when air curtain exists. Results indicate that contaminants are concentrated around the source. The average

fraction of exhaled contaminants outside air curtain under different human-curtain distance decreases 4.3%-19.6% compared to mixing ventilation with same flux. Shortening the human-curtain distance can improve the performance of air curtain and may change the direction of exhaled airflow. Moreover, It has better performance when the enclosure shape is close to a circle. Higher jet velocity is better for improving the confinement performance, but the trend is not very obvious as velocity increases. For exhalation modes, it is more challenging to control exhaled contaminants for intense exhalation activity (such as
coughing
) in steady simulation, but results in transient simulation show better performance when
coughing
only once. These results can provide a reference for the subsequent design and improvement in applying air curtain in hospital wards or other places, especially during the period of flu outbreak.
