We present a 3-yr-old girl with coarctation of aorta and patent ductus arteriosus in whom mycotic aneurysm and bacterial
endarteritis
developed postoperatively and was diagnosed by two-dimensional and Doppler echocardiography. Five weeks after the operation of ligation of ductus and resection of coarctated segment, the patient was readmitted with complaints of vomiting, fever and
coughing
. Bacterial
endarteritis
, empyema and septic arthritis were diagnosed. Suprasternal echocardiographic examination demonstrated an aneurysmatic appearance 60 x 65 mm in size at the location of coarctation. The patient died, most probably due to aortic rupture, before surgical treatment could be undertaken. Autopsy study confirmed our diagnosis.

Since the introduction of surgical treatment for coarctation aortae, presentation of the disease with postcoarctation mycotic pseudoaneurysm has become rare. We report an 18-year-old male with
endarteritis
caused by Staphylococcus aureus treated with appropriate antibiotics. Later the patient developed an irritative
cough
and problems with swallowing. CT and MRI of the chest demonstrated a postcoarctation pseudoaneurysm. After acute surgery with resection of the coarctation and pseudoaneurysm and insertion of a tube graft, the patient had an uneventful postsurgical course.

A 46-year-old man presented with nonproductive
cough
and lower limb swelling. Chest radiograph showed a left lower lobe lung mass and multiple subpleural nodules. Other investigations revealed that he had nephrotic syndrome. Core biopsies of the left lower lobe lung mass showed features of inflammatory pseudotumor with
endarteritis
obliterans and a lymphoplasmacytic infiltrate. Immunohistochemical stain for
Treponema pallidum
was positive. Resolution of the lung mass and nephrotic syndrome was achieved after treatment with intramuscular benzathine benzylpenicillin. The differential diagnosis of pulmonary inflammatory pseudotumor, manifestations of pulmonary syphilis, and a literature review of secondary syphilis of the lung are discussed.
