Fourteen previously healthy young patients with unusual community-acquired opportunistic infections were seen over a period of three years. They differ from patients previously described in that 11 were heterosexual drug abusers (including two women) and only three were homosexual men. There were eight Puerto Ricans, five blacks, and one white. Infections included Pneumocystis carinii pneumonia (seven), disseminated Mycobacterium intracellulare infection, histoplasmosis,

, and cytomegalovirus infection (one each), oral thrush (13), and Candida esophagitis (two). All patients had impaired cellular immunity manifested by cutaneous anergy and lymphopenia, and all 11 tested had a markedly decreased ratio of T helper/inducer cells to T suppressor/cytotoxic cells. Twelve had evidence of associated viral infection (Epstein-Barr virus in nine, cytomegalovirus in five, Herpes simplex type 2 in two). We report a case of primary pulmonary
cryptococcosis
. A 20-year-old woman was admitted to the hospital complaining of
coughing
, fever, and dyspnea on exertion. She had no underling disease or immunological abnormality. Chest X-ray film revealed bilateral diffuse infiltrative shadows, which were first believed to have been caused by a community-acquired pneumonia. Pulmonary cryptococcosis was diagnosed from the results of a transbronchial lung biopsy. After 2.5 years of anti-mycotic chemotherapy with amphotericin B and flucytosine, pneumothorax occurred in the left lung. Thoracotomy and open lung biopsy were done. Histological findings of the open lung biopsy specimens showed numerous broken cryptococcal organisms within alveolar macrophages. Diffuse fibrosis accompanied by multiple bullae may have punctured bullae or blebs and thus led to pneumothorax.

We report a 37-year-old female patient with pulmonary
cryptococcosis
who presented with a one week old history of

and fever. Within a week of admission, she developed cryptococcal meningitis and 5 months later cryptococcomas of the brain were detected. She was successfully treated with 2 courses of amphotericin-B at an interval of 5 months. 5-flucytosine was also administered initially along with amphotericin-B. The pulmonary lesions resolved completely whereas the cryptococcomas reduced in number.

Seventeen patients with pulmonary
cryptococcosis
collected at VGH-Taipei from 1967 to 1991 were analysed. Their ages ranged from 16 to 70 years with a mean of 45.2. Twelve were men and five were female. The chief characteristic symptoms and signs were

(58.8%), fever (29.4%) and dyspnea (23.5%). Nearly one third of cases were asymptomatic and presented with abnormal chest roentgenogram. The chest roentgenogram included single nodule or mass (41.2%), multiple nodules (35.3%), pneumonic patch or consolidation (23.5%). In addition, 5 cases (29.4%) and 4 cases (23.5%) were combined with cavitary formation and pleural effusion respectively. Only two cases had associated immunologic deficiency. No patient was found to have history of breeding pigeon. Five cases didn't receive any treatment but were followed from 6 months to 5 years. The remaining 12 cases underwent medical treatment, surgical resection or a combination of both. Prognosis was good in all except one patient who died of multiple myeloma.
