A 33-year-old woman had been on hemodialysis since she was 25 years old. In September, 1993, she had cough, which gradually increased in severity, and was accompanied by slight fever. No abnormality was observed on the chest radiography taken on November 2. However, bilateral diffuse miliary shadows were observed on the radiograph taken on November 30. Tubercle bacilli were detected by the examination of sputum smear. As she was pregnant 20 weeks, therapeutic abortion was done on December 8. A case of miliary tuberculosis occurring during pregnancy in a patient on hemodialysis has not been documented in Japan to date, and the first case in Japan was reported with a review of the literature.

In Egypt, 400 randomly selected pregnant women attending prenatal clinics affiliated with El-Shatby Maternity Hospital, Dar El-Welada Hospital, Gamal Abdel Nasser Hospital, Boharram Bay Maternal and Child Health Center, and Bacous Maternal and Child Health Center were interviewed to determine their knowledge of and practices related to drug intake during pregnancy. 47.7% had adequate knowledge (i.e., 75% correct answers) of drug use during pregnancy. Only 14% did not use any drugs during pregnancy. The remaining 86% used drugs without a prescription. The most common drugs used were vitamins and tonics (78.8%), antacids (66.5%), analgesics (41.8%), and antiemetics (35.5%). The leading reasons for drug use included general weakness (78.8%), heart burn and indigestion (66.5%), headaches (41.8%), vomiting (35.5%), and cough and insomnia (27.5%). Factors associated with poor knowledge of drug use during pregnancy were: younger than 30, illiteracy, being a housewife, primigravidity, and history of abortion (p 0.05 for all factors). These findings indicate a need to inform pregnant women about the dangers of drug use during pregnancy, especially during the first 12 weeks of pregnancy. Nurses should play a key role in communicating these risks.

The purpose of this study was to investigate the three-year incidence risk of various diseases of adult sheep and goats through on-farm herd health monitoring. The study was conducted, between April 1989 and March 1992, in three sheep and three goat herds with a rolling average total population of 704 adult animals. A system for on-farm data recording was established at the beginning of the study. The data were recorded on a specially-designed form by a veterinary stock assistant stationed on the farms, and collected by the investigating veterinarian during weekly visits. The results were used to determine the three-year incidence risk, three-year recurrence probability and time to first occurrence of various health problems. The associations between occurrences of various health problems were also determined. In the study herds, the three-year incidence risk was highest for diarrhoea, enterotoxaemia, cough/nasal discharge, laboured breathing with elevated body temperature, and abortion. Significant and strong associations were recorded between digestive disorders and death, and between respiratory disorders and death. Temporal occurrence of these health problems was recorded to determine the seasonality of occurrence. Areas for further research are discussed.

Success of intraocular surgery depends in part on low intraocular pressure, sufficient muscle relaxation during the operative period, and avoidance of coughing during emergence from anaesthesia. Isoflurane (ISO) guarantees deep anaesthesia and Laryngeal Mask Airway (LM) makes emergence smoother than when using conventional endotracheal intubation. Occupational exposure to isoflurane appears unavoidable near the potential main source of leakage, the "patient's mouth". Due to the short distance between the patient's mouth and the working area, the risk of exposure for the surgeon could be high. Traces of anesthetic gases may cause various negative effects on the health of the personnel. Higher abortion rates are reported on in recent literature. The aim of this study was to quantify trace amounts of ISO and then to compare these values with international threshold limits. In this study, trace concentrations of ISO were measured directly by means of a high sensitive photoacoustic infrared spectrometry analyser. Measurements were done continuously every 120 seconds at the working places of the surgeon, anaesthetist and the assisting nurse. One possible leakage source, the patient's mouth, was also analysed. 10 cataract operations under general anaesthesia were included in the study. All the measured values were low, the majority under 2 ppm isoflurane. The national threshold values (10 ppm) were not exceeded. We conclude that the use of the LMA in ventilated patients is not associated with high concentrations of isoflurane in the working environment of the operating theatre personnel during eye surgery. Trace concentrations of isoflurane using the LMA are comparable with those using ET under these working conditions. Under other working conditions, higher concentrations may be expected should therefore be controlled.

Equine herpesvirus-1 (EHV-1) causes respiratory disease, neonatal death, abortion and neurologic disease. The main purpose of this study was to identify viral antigen in respiratory tract samples by immunoperoxidase staining. Six pony foals were selected on the basis of demonstrating seronegativity to EHV-1 by virus neutralization and housed in isolation. They were infected experimentally by administering EHV-1 nebulized ultrasonically through a face mask. Successful infection was clinically apparent as each of the foals had febrile responses, nasal discharge, and enlarged submandibular lymph nodes. Sporadic coughing was also heard. EHV-1 was isolated from nasopharyngeal swabs of 4/6 ponies and seroconversion was demonstrated in all foals. Bronchoscopic examination of the large airways revealed hyperemia. The incidence of recovery of Actinobacillus suis from nasopharyngeal swabs increased initially, with recovery of Streptococcus zooepidemicus isolates predominating at 3 wk post-infection. Cytology brushes were used to sequentially sample the respiratory tract of the infected ponies at the nasopharynx, mid-trachea and the mainstem bronchus. Bronchoalveolar lavage provided lung cells. Immunocytochemistry techniques were applied to both types of samples to locate EHV-1 antigen. Indirect immunoperoxidase staining of samples utilizing monoclonal antibodies specific for EHV-1 demonstrated viral antigen associated with cellular debris, primarily in the nasopharyngeal samples on days 3-9 post-infection.

The main causes of death in rural areas of the Faritany of Toamasina during 1986 are identified and classified by order of importance for the entire population as well as for vulnerable groups such as preschool children and reproductive-aged women. The 10 leading causes of death of infants and children under age 5 years are coughs and fevers, as well as thoracic pains; vomiting and diarrhea; age factors; high, intermittent fevers and chills; protein-calorie malnutrition; convulsions; other high fevers; cough of long duration; sudden death; and measles. Leading causes of death for women aged 15-49 years include coughs and fevers, as well as thoracic pains; high, intermittent fevers and chills; vomiting and diarrhea; other high fevers; delivery complications; cough of long duration; malnutrition; abortion or miscarriage; sudden death; and postpartum illnesses. Over 60% of deaths reported for children aged 0-5 years could have been prevented through a broader vaccination program, oral rehydration therapy, nutrition education and growth monitoring, and the preventive treatment of malaria. Priority focus should be given to respiratory infections.

Canine herpesvirus-1 (CHV-1) is presumed to be enzootic in the dog population and is associated with fertility disorders and neonatal mortality. In this study we screened for risk factors affecting CHV-1 antibody titers and investigated the association between antibody titers and reproductive disorders. Therefore, serum from 545 dogs used for reproduction was analysed with an ELISA. Using a forward stepwise procedure and retaining significant risk factors (P<0.05), best fitting multifactorial generalized linear model (glm) procedures were built for males and females. The effect of antibody titers on reproductive disorders was analysed with logistic regression analysis. The association between reproductive disorders and seroprevalence was analysed in chi-square analyses using contingency tables. In both sexes, kennel cough and breeding management were found to have an impact on the CHV-1 antibody titer. Also, the influence of kennel cough on the antibody titer was correlated to the hygienic status of the kennel. In females, age, kennel size and cycle stage had an effect on CHV-1 antibody titers. Furthermore, kennel size and hygiene were found to be correlated. In males, mating experience had an impact on CHV-1 antibody titers. An association was observed between serological status and a history of abortion in bitches. In conclusion, this study suggests CHV-1 antibody titers may be affected by many factors, both on an environmental and host level. Therefore, interpretation of the serological status requires precaution. Furthermore, oronasal and venereal transmission seem to play a role in the spreading of infection.

We report a case of respiratory distress in the post anesthesia care unit following general anaesthesia for a dilatation and curettage related to miscarriage in a 32-year-old woman. The preoperative physical examination showed no abnormalities except for the presence of dry cough during the preceding two or three days. A few minutes after her arrival in the PACU, the patient developed hyperthermia till 40.6 degrees C, cough, polypnea and oxygen desaturation (SpO(2): 82% on FiO(2): 40%). A thoraco-abdominal angioscanner showed pulmonary basal condensations and a thrombosis of the right ovarian vein. The patient had to be transferred to the intensive care unit where she remained intubated and ventilated during 13 days because of a Haemophilus influenzae pneumonia.

Brucellosis remains a major zoonotic disease worldwide. It has never been reported at King Chulalongkorn Memorial Hospital (KCMH). The authors describe the first case of brucellosis in KCMH, and also review all previous reports in Thailand. The presented case was a 52-year-old Thai man, living in Phetchabun Province, who was diagnosed with idiopathic pulmonary fibrosis two years prior to admission. He presented with prolonged fever, dry cough, weight loss of eight kg over three months, hepatosplenomegaly, and pancytopenia. Blood and bone marrow cultures grew Brucella melitensis at 72 hours of incubation. A slide agglutination (Rose Bengal) test was also positive for Brucella antibody. He had been exposed to contaminated placenta of his goats that had spontaneous abortion in the past few months before his illness. The patient was successfully treated with gentamicin, doxycycline, and rifampicin. Clinicians should have a high index of suspicion when evaluating patients presenting with prolonged fever and having an exposure risk of brucellosis.

Canid herpesvirus 1 (CaHV-1) is a well-known cause of fatal hepatic and renal necrosis in neonatal puppies. In adult dogs infected with CaHV-1, papulovesicular genital lesions may be observed. CaHV-1 infection during pregnancy can lead to embryonic resorption, abortion, and stillbirth. In high-density dog populations, CaHV-1 can also contribute to kennel cough. Furthermore, recent literature has clearly documented that CaHV-1 can induce ocular disease in immature and adult dogs. The current study describes a case of fatal CaHV-1 infection in a 9-year-old spayed female Bichon Frise dog. Following a history of vomiting and diarrhea, the dog deteriorated and subsequently died. The main lesions were multifocal areas of necrosis with intranuclear inclusion bodies in the liver, adrenal gland, and small intestine, similar to the lesions observed in CaHV-1-infected puppies. Infection with CaHV-1 was confirmed on samples of liver by polymerase chain reaction, immunohistochemistry, and in situ hybridization. There was no indication of immunosuppression in this dog. Based on the results presented herein, CaHV-1 should be included in the list of differential diagnoses of hepatic necrosis in adult dogs.
