The author's cat started to develop the signs of pemphigus foliaceus one month after he returned home after six months absence. The initial signs included dry coughing and difficulty with purring and swallowing, followed by typical changes of the skin. The cat was treated by a combination of chrysotherapy and systemic glucocorticoid injections, and remained free of clinical signs for one and a half years. The cat then relapsed and showed the initial signs except that coughing was not observed. It was treated as before but after a second relapse and the same treatment it slowly developed a general weakness and was euthanased.

The diagnosis of dysphagia in the dog requires an evaluation of a variety of signs that can be caused not only by a 'swallowing disorder' but also by several other pathological conditions. Most owners mention coughing, vomiting, regurgitation and nasal discharge, and the clinician must decide whether these signs are related to dysphagia. In this study a standardised questionnaire for the diagnosis and localisation of dysphagia was evaluated for its accuracy by comparing the results with contrast videofluorography as the definitive standard. The purpose of the study was to optimise the selection of dogs for more expensive diagnostic procedures such as videofluorography and electromyography. In a group of 69 dogs with 'swallowing problems' the questionnaire had a sensitivity of 0.97 and a positive predictive value of 0.94 for dysphagia in general. The questionnaire was also useful for the exclusion of oral phase dysphagia, with a specificity of 0.70 and a negative predictive value of 0.97. Most dogs with pharyngeal phase dysphagia could be detected by using the questionnaire (sensitivity 0.91). The questionnaire was not of specific value for the detection or exclusion of oesophageal phase dysphagia, for which it had a sensitivity of 0.69, a specificity of 0.57 and predictive values for positive and negative tests of 0.79 and 0.44, respectively.

PMID:Evaluation of a standardised questionnaire for the detection of dysphagia in 69 dogs. 845 12

The clinical features of 18 cases of summer pasture associated obstructive pulmonary disease were reviewed. The horses had signs of obstructive pulmonary disease (expiratory dyspnoea, wheezing and crackling lung sounds and coughing) during the spring, summer or autumn while they were kept permanently at grass with no access to hay or straw, for at least two consecutive years. In nine cases there was a seasonal incidence with the disease occurring during April and May. Eleven of the horses were affected by bouts of severe dyspnoea. Eleven of the horses also suffered from chronic obstructive pulmonary disease and two were affected by idiopathic headshaking. Endoscopy revealed evidence of lower airway inflammation, and a cytological examination of tracheal aspirates and bronchoalveolar lavage fluid revealed a neutrophilia. Moving the horses into a stable controlled the clinical disease effectively in only two of the 16 cases. Oral clenbuterol was effective in only seven of 15 cases. Systemic dexamethasone or oral prednisolone, in combination with clenbuterol, was the most effective treatment.

Each of 10 set-stocked calves was given a fenbendazole sustained-release intraruminal bolus at turnout for the control of parasitic bronchitis while a group of 10 similar calves was left untreated. The respiratory rates of the control calves were not greatly increased during the grazing season, but persistent coughing was evident from early July when they all had patent lungworm infections. Only occasional coughing was reported from the bolus-treated calves except for a transient increase in its frequency in late September. In mid-August, one of the treated calves was passing lungworm larvae and when they were housed six of the 10 had patent infections. In August tracer calves picked up an average of 23.5 lungworms per day from the control paddock but only 2.1 from the paddock grazed by the treated calves. In September the corresponding figures were 7.6 and 19.2 lungworms per day, respectively, from the control and 'treated' paddocks. After housing, the post mortem worm counts after an experimental challenge with Dictyocaulus viviparus larvae were reduced by 99.2 and 98.7 per cent (P < 0.0001), respectively, for the control and bolus-treated calves in comparison with weight-matched parasite-naive calves. Thus, despite a relatively low level of challenge during the grazing season, the treated animals had developed a considerable degree of protective immunity.

A longitudinal study of respiratory disease in racehorses was carried out to assess its relative associations with different infectious agents and to examine any role that the environmental conditions might play. The relationships between coughing, nasal discharge, pyrexia and lower respiratory tract disease were also examined to provide information for improving clinical diagnosis, particularly of disease of the lower respiratory tract. Lower airway disease was closely associated with infection with Streptococcus zooepidemicus. It was also found that equine herpesvirus seroconversions and S pneumoniae infections were independently associated with the development of nasal discharge. Coughing was a specific, but insensitive measure of lower respiratory tract disease (specificity 84 per cent, sensitivity 38 per cent). However, horses that coughed were very likely to have had lower airway disease for more than one month. Horses housed on straw in loose boxes were twice as likely to suffer from lower airway disease as those kept on shredded paper in American barns. The study was not large enough to assess the significance of rarer infections but it did improve the definition of the problem of respiratory disease in racehorses and revealed some of the trends in the associations between viruses, bacteria and the environment in respiratory disease.

Forty-five calves were assigned randomly to three groups of 15 at turnout (day 0) in the spring of their first grazing season on to separate paddocks on a pasture known to carry infective larvae of Dictyocaulus viviparus. Group 1 calves were left as untreated controls, group 2 were injected with doramectin at 200 micrograms/kg at turnout and again on day 56, and group 3 were treated with an intraruminal bolus containing ivermectin. From day 42 onwards group 1 calves developed parasitic bronchitis which required repeated treatment with levamisole. The two endectocide regimes controlled lungworm infection, although some calves in group 2 developed some coughing during the week before the second dose of doramectin. After the end of the grazing season and again in May of the following year, five cattle from each group were infected experimentally with lungworm larvae and slaughtered 28 days later for lungworm counts and measurements of length to be made. At both times group 1 calves were found to be largely resistant to reinfection; group 2 were slightly more susceptible although the differences from group 1 were not statistically significant. Group 3 calves were more susceptible with no significant difference in worm counts from naive infection controls.

The prevalence of diseases suffered by dogs within two weeks of their acquisition from a rescue shelter in Northern Ireland was investigated. A postal questionnaire was completed by 556 people who had purchased a dog from the Ulster Society for the Prevention of Cruelty to Animals (USPCA) to provide information on the diseases suffered by their dog. The majority of the dogs (53.7 per cent) had an ailment the most common being coughing and diarrhoea. Of the respondents who returned their dog to the USPCA, 92 per cent did so because the animal was unhealthy. Stray dogs were more likely to have an ailment than unwanted dogs, specifically coughing, and/or skin problems. More puppies suffered from parvovirus, vomiting, and/or diarrhoea than did juveniles or adults. Adult dogs were more likely to have a cough than juveniles or puppies. There was no association between the sex of the dogs and their ailments.

A cross-sectional survey of a convenience-sample of 740 cats was undertaken to obtain an estimate of the prevalence of Bordetella bronchiseptica infection, and to identify risk factors that might predispose them to the infection. Data on individual cats and household variables, including disease status and animal contacts were obtained by questionnaire. B bronchiseptica was isolated from 82 (11 per cent) of the cats sampled. The prevalence of B bronchiseptica varied with the type of household sampled, being 19.5 per cent in rescue catteries, 9 per cent in breeding catteries, 13.5 per cent in research colonies, and 0 per cent in household pets. On the basis of a univariable analysis, 19 of 29 predictor variables were found to be significantly associated with the isolation of B bronchiseptica, including an association with cats in rescue catteries, and with cats from premises with larger numbers of animals. Separate analysis of the rescue cattery subpopulation showed a highly significant association on multivariable analysis with current respiratory disease, suggesting that different risk factors may operate in this type of environment. In the whole sample there was also strong association with cats from households containing a dog with recent respiratory tract disease. The clinical signs observed in the B bronchiseptica-positive cats included sneezing, ocular and nasal discharges and coughing, although only the association with sneezing was statistically significant. There was no significant association between the isolation of B bronchiseptica and the isolation of respiratory viruses, suggesting that in some circumstances B bronchiseptica may be able to cause disease independently.

This paper describes the clinical features, and diagnostic findings of a chronic respiratory condition in 29 West Highland white terriers. Typically, the dogs were coughing chronically, had dyspnoea and tachypnoea of varying severity, and had deteriorated progressively over months to years. The mean (sem) survival time in months from the clinical signs being first noted by the owners was 17.9 (2.3). Most cases had a combination of respiratory signs, but coughing was the predominant sign in 18 cases. Inspiratory crackles were audible on chest auscultation in 28 cases, 10 of which were also wheezing. Rhonchi were the predominant sound in the remaining case. The main radiographic changes were mild to severe increased Interstitial markings in all cases, with additional bronchial markings in 14 of the dogs. Right-sided cardiomegaly (cor pulmonale) was recorded in 15. Bronchoscopic findings in 17 of the dogs were either normal or involved a mild airway mucoid reaction in eight. Chronic mucosal changes were observed in eight, but in two this finding was equivocal. Dynamic changes to the lumen of the airway were present in seven cases. No significant haematological or biochemical changes could be detected in 20 cases, but four cases were hypercholestrolaemic. A histopathological assessment of four cases revealed alveolar septal fibrosis to be the predominant change. Prednisolone, with or without bronchodilators, was the most commonly used therapy, and the response was variable. The condition appears to be associated with significant pulmonary interstitial fibrosis of unknown aetiology and has clinical similarities to idiopathic pulmonary fibrosis (cryptogenic fibrosing alveolitis) in human beings.

In 1998, equine influenza was diagnosed by serology and nucleoprotein enzyme-linked immunosorbent assay as the cause of acute respiratory disease in vaccinated and unvaccinated horses in the UK. The signs were generally milder in vaccinated horses and completely susceptible animals showed the most severe signs, including pyrexia, inappetence, coughing, mucopurulent nasal discharge and secondary bacterial pneumonia. In a detailed investigation of an outbreak among 52 vaccinated thoroughbreds in a flat racing yard, more than 60 per cent of the horses seroconverted on the evidence of paired serum samples tested by single radial haemolysis (SRH). Preliminary sequencing and characterisation of an isolate from this outbreak indicated that it was an 'American-like' strain. In addition, in this outbreak there was a larger proportion of horses with preinfection SRH titres greater than 140 mm2 that subsequently seroconverted than in other recent outbreaks from which 'European-like' strains have been isolated. This result suggested that the cross-protectivity between circulating 'American-like' strains and the 'European-like' strains of A/equine-2 viruses present in current vaccines may be decreasing.
