A
cold
kit has been developed for the instant preparation of 99mTc-(V)-
DMSA
. The radiochemical purity of the radiopharmaceutical was determined by chromatography. A study of the effect of pH and stannous chloride concentrations on its labelling efficiency demonstrated that pH 8.5-9.0 and 0.2 mg stannous chloride for 1.1 mg
DMSA
provides the optimum formulation (98%). The addition of ascorbic acid stabilized the kit for up to 24 weeks under refrigerated temperatures. The radiopharmaceutical was found to concentrate in the bone and muscles in contrast to trivalent
DMSA
which accumulates in the liver and kidneys in normal mice. Studies in patients have shown its usefulness in the scintigraphy of thyroid medullary carcinoma and in the brain metastasis of breast carcinoma.

From 1984 to 1990, 99mTc-
DMSA
renal scintigraphy was performed before and after nephrolithotomy (15 cases), pyelolithotomy (15 cases), percutaneous nephrolithotripsy (PNL: 15 cases) and extracorporeal shock wave lithotripsy (ESWL: 16 cases, 17 kidneys) in order to evaluate of influences of renal stone surgeries on split renal function.
DMSA
renal uptake change ratio of treated kidneys of nephrolithotomy (-24.94 +/- 5.60%) was significantly lower than that of PNL (-0.06 +/- 3.92%), pyelolithotomy (-4.08 +/- 4.79%) (p less than 0.01) and ESWL (-7.72 +/- 3.87%) (p less than 0.05). The average change ratios of contralateral kidneys were as follows: PNL 4.80 +/- 4.21% nephrolithotomy 4.67 +/- 4.73%, pyelolithotomy -1.46 +/- 5.39% and ESWL -2.02 +/- 4.44%. One to 3 weeks after PNL, the
cold
area on the renal image was found in 10 (66.7%) of 15 cases. In cases of ESWL,
DMSA
renal uptake decreased even 4-10 weeks (mean 7 weeks) after treatment. In conclusion, possivility of deterioration of renal function after ESWL was suggested.

99mTc-
DMSA
renal scintigraphy was carried out in 54 patients with unilateral renal stones before and after PNL. Four to 8 weeks after PNL the
DMSA
renal uptake significantly decreased to 17.2 +/- 6.0% from 18.2 +/- 6.7% before PNL.
DMSA
renal uptake did not change in the contralateral side. Since in some patients changes in the
DMSA
renal uptake of 5-7% were observed after PNL not only in the PNL side but also in the contralateral side, the renal function was assessed by the formula:
DMSA
renal uptake in the PNL side/
DMSA
renal uptake in the contralateral side, and the change of this ratio was evaluated in 44 patients, in whom the renal
DMSA
uptake in the PNL side was less than two times that in the contralateral side. The
DMSA
renal uptake ratio decreased to 95.6 +/- 8.7% from the base line 4-8 weeks after PNL. This change was statistically significant. Some functional risks such as massive bleeding with PNL, the fever after PNL and the number of nephrostomy tract did not affect the decrease in the renal function. In 29 patients in whom renal function was reevaluated one year after PNL, the
DMSA
renal uptake ratio significantly decreased to 94.2 +/- 9.6% from the base line 4-8 weeks after PNL. But the ratio significantly improved to 99.6 +/- 11.6% about one year after PNL. In two patients with a
cold
area on the renal image, the renal function of the operated side still remained at about 80% levels from the base line even one year after PNL.(ABSTRACT TRUNCATED AT 250 WORDS)

Twenty-five patients with solitary
cold
thyroid nodules (SCN) were studied by fine needle aspiration (FNA) cytology, radionuclide perfusion scanning (RPS) and pentavalent technetium dimercaptosuccinic acid (99m Tc
DMSA
) scanning, with an aim to distinguish neoplastic from non-neoplastic nodules. All the patients were operated upon and the nodules were subjected to histopathological examination. 13 of the 25 nodules (52%) were malignant (7 papillary carcinomas, 3 medullary thyroid carcinomas and 1 each of follicular carcinoma, insular carcinoma and clinically anaplastic thyroid tumour). The specificity of
DMSA
scanning and FNA cytology were 100% and that of RPS 60%. The sensitivity of RPS and
DMSA
scanning were 80% and 20% and that of FNA cytology (using a broad definition of test-positivity), 100%. Additionally, FNA cytology could morphologically type the majority of neoplasms.

A sixty year old female referred for thyroid and liver scintigraphy had a clinical history of progressive swelling in the neck with hepatomegaly. A large
cold
area was detected in the right thyroid lobe using 99mTc pertechnetate and in the right lobe of liver using 99mTc phytate. Subsequent whole body scan with 99mTC
DMSA
(V) showed avid tracer uptake in right lobe of thyroid and liver. Aspiration cytology of thyroid and liver showed medullary carcinoma of thyroid with its metastasis in liver. Histopathology following thyroidectomy confirmed the diagnosis. Thus 99mTc pentavalent
DMSA
contributes specificity to diagnose medullary carcinoma of thyroid and metastatic lesions.

Current therapeutic approaches in neuroendocrine tumours include surgery, radiotherapy and polychemotherapy. Different metabolic patterns of neuroendocrine tumours allow the use of a wide range of diagnostic options in nuclear medicine, due to the presence of a wide spectrum of radiotracers electively concentrating in these neoplasms. Nuclear medicine, and in particular 111In Octreotide (OCT) scintigraphy, 123I Methaiodobenzylguanidine (MIBG) and pentavalent 99mTc-
DMSA
(V-
DMSA
), together with biohumoral markers, are currently able to locate tumours also not detectable using traditional diagnostic techniques. Somatostatin analogs, such as octreotide have become increasingly important over the years in the treatment of patients with neuroendocrine tumours. At present the therapeutic use of somatostatin analogs can be schematised as 1) pharmacological treatment (with
cold
octreotide); 2) surgical treatment (radioguided surgery); 3) radiometabolic treatment (with marked octreotide). The development of new synthetic molecules and new radiocompounds will probably open up interesting scenarios in the near future.

Since December 1995, pediatric renal transplant recipients in our unit have received a
DMSA
scan as soon as possible post-transplant in order to provide a baseline for comparison in the event of subsequent complications. We retrospectively reviewed the case notes and
DMSA
scans of the 45 patients who underwent a scan within 9 wk of their transplant to see if pre or peri-transplant factors or post-transplant complications were associated with defects on scanning. Forty percentage of scans had defects. The presence of defects was not associated with potential predisposing factors such as patient or donor age, cadaveric or live donation,
cold
ischemia time, multiple donor vessels, the use of non-heart beating donors, the mean time to scan, the serum creatinine, or the presence of structural renal tract anomalies predisposing to UTI. However, 87% of patients had complications before the scan, including UTI, rejection, acute tubular necrosis, transplant biopsy and drug toxicity. Children with no clinical complications had a significantly reduced risk of a defect (p = 0.035), while biopsy was associated with the presence of defects (p = 0.0034). Twenty patients had one or more follow up
DMSA
scans: one patient developed a new focal defect. In conclusion, renal transplant defects are frequently found on
DMSA
scanning even early after transplantation and are non-specifically associated with many different complications.
