As a preliminary approach to an epidemiological investigation of Huntington's disease, the authors report the incidence of admissions of these patients to both Neurological and Psychiatric Departments in Genoa and surrounding region from 1930 to 1977. The number of patients (84 cases) and their family history (225 cases of Chorea in the ancestors) suggest a prevalence of Huntington's disease remarkably higher (about 28 cases out of 100,000 inhabitants) than it is usually estimated. Further studies are however required to assess the distribution of the disease in the whole population of the region.

We report a case of infectious mononucleosis in which central nervous system involvement was the presenting and sole manifestation of the disorder. The major symptomatology consisted of stupor, chorea, and signs of aseptic meningitis. We also discuss the clinical and laboratory features of the neurological manifestations of infectious mononucleosis and the theories as to its pathogenesis.

In the last few years both neuropathological and neurochemical data have contributed towards a classification of the pathogenesis of degenerative chorea of which Huntington's chorea shows clear evidence of heredity. The role of GABA, DA and Ach in determining chorea seems clear. In 7 patients with degenerative hypotonic-hyperkinetic chorea (the acid metabolites into the cerebrospinal fluid are not available) a neuropharmacological study was conducted. The symptomatology was evaluated both through clinical rating and through phasic and tonic performance tests. The neuropharmacological tests were both acute and chronic. In the acute tests the effect of modification of various neurotransmitters (Ach, DA, NA, GABA) was evaluated. In chronic treatment the effectiveness of haloperidol, PCPA and bromocryptine was evaluated. The results of the acute study were positive both as regards increment of DA (receptors stimulation) and GABA. The chronic study shows any effect of PCPA, positive results both with bromocryptine and with haloperidol. Our results confirm a DA/GABA interaction in the pathogenesis of degenerative chorea.

A univocal disorder of dopaminergic activity in the nerve structures responsible for extra-pyramidal motility does not take into account the total phenomena seen in psychomotor neurological studies. The hypothesis of two categories of dopaminergic receptors-respectively excitatory (DA) and inhibitory (DAi)--in the striatal and mesolimbic structures concerned, was derived from experimental and clinical anatomical cytological, and pharmacological data. This duality of the receptors could establish a relationship between dopaminergic mediating, mechanism and adrenergic mediation for with the existence of a least two types of distinct receptors, alpha and beta, has been demonstrated. The author discusses the physiopathology of neuromotor disorders wuch as Parkinson's disease, chorea, and various types of dyskinesias, in relation to this dual mediation. By separation of dopamine antagonists and agonists in this new context, it should be possible to have a more selective approach, which would be better adapted, therefore, for the treatment of these affections. The study of the effects of a new medication, tiapride, on dyskinesias provoked by levodopa in parkinsonian patients, is a step forwards in this new method exploration in neurology.

The therapeutic action of Tiapridal appeared to the authors particularly precious and constant in delirium tremens, senile agitation and turbulence, whatever the origin, and bucco-linguo-facial dyskinesia, whether the latter were linked to age or whether they formed part of a neuroleptic syndrome. Their experience does not permit them to have any opinion concerning the use of this drug in tremor and chorea, the patients seem to respond favourably to treatment but in an erratic manner. On the other hand the drug was totally inefficacious in patients suffering from spasmodic torticollis and writer's cramp. Finally, it seemed to them useful to emphasise the improvement in comfort in patients suffering from various pains when given Tiapridal. This justifies the place given to Tiapridal among drugs necessary for the daily practice of neurology.

It is the purpose of this communication to study statistical correlations between the different symptomes of MBD ; speech defect, dyslexia, hyperactivity, poor muscular control, give a school handicap for about 10 per cent of children, excluding cerebral palsy, sensorial trouble, or intellectual defect. The choreiforme syndrome is a systematic investigation in a group of 60 children selected with two symptomes of MBD. Choreiform movements are found in 48 per cent of our MBD patients (40 per cent of patients with hyperactivity and 50 per cent of patients with serious dyslexia).

Rheumatic fever has decreased in frequency and severity, but the disease has not been eradicated in this country, especially in low socioeconomic populations. Carditis is less common, and changes in the frequency of other rheumatic manifestations such as chorea has made the clinical diagnosis more difficult. Streptococcal antibody tests are still the most useful laboratory aid, but care must be taken against overinterpretation. The search for the answer to the pathogenesis puzzle continues. Until the nature of the disease is better understood, it is unlikely that rheumatic fever will be eradicated. In the meantime, however, the incidence of the disease can be reduced further by improved medical care for deprived populations.

The effects of the thyroid gland upon the nervous system with neurologic-psychiatric symptoms are discussed. With hyperthyreosis in younger patients hyperactive and psychotic signs occur, in older patients so-called apathic thyreotoxicosis. Neurologically there appear acute and chronic thyreotoxic myopathies, paroxysmal paralyses, the so-called hyperthyreotic chorea and a series of disturbances of eye muscles. In a thyreotoxic crisis psychotic preliminaries may proceed to a coma. With hyperthyreoses there occur intellectual impairment, showing but also psychoses as well as multiple neurological defect syndromes and epileptic seizures. Neurological defects as a consequence of thyreostatic therapy are rare, mostly these are disturbances of periphal nerves.

The clinical features of acute rheumatic fever as seen in Wairoa County 1962--76 are described. The types of cases seen comprised 107 definite first attacks, 78 definite recurrences, and 117 first attacks and 34 recurrences which did not satisfy strict diagnostic criteria. The incidence of the major manifestations, carditis, polyarthritis and chorea were 53 percent, 56 percent and 11 percent in definite first attacks, and 81 percent, 26 percent and 8 percent in definite recurrences, respectively. The ratio of recurrences to first attacks was high (58 percent). The Wairoa population exhibits factors thought to contribute to a high risk of recurrence--young age distribution, non-Caucasian ethnicity, avoidance of parenteral prophylaxis and frequent carditis in the first attack.

The diagnosis, natural history and management of this uncommon manifestation of CNS lupus is discussed, as well as the influence of pregnancy on disease activity in SLE.
