The case of a 36-year-old women found to have a hepatic tumor is reported. The patient complained a malaise, weakness,
dyspnea
, and ankle edema and had been aware of a slowly growing abdominal swelling for 3 years. She had been taking Gynovlar 21 (3 mg norethinsterone acetate with 50 mcg ethinyl estradiol) for 6 years. Laparotomy revealed a solid, vascularized tumor arising from the left lobe of the liver and from part of the right lobe. A 2800 gm mass was excised along with a 40 gm mass from the
celiac
axis that involved lymphatic tissue. This is the 1st case report of a hepatic malignancy associated with an oral contraceptive that showed histological evidence of secondary spread.

Precipitin responses to different avian serum antigens occur in bird fancier's lung (BFL) and
coeliac disease
. Failure to distinguish between them could encourage an erroneous diagnosis of BFL in patients with
coeliac disease
, and the recent suggestion that these two disorders are strongly associated may be questioned, partly for this reason. In the present study small bowel biopsy specimens were obtained from 12 of a series of 14 patients proved to have BFL by inhalation provocation tests. None was suggestive of
coeliac disease
. Of a further 61 patients with biopsy-proved
coeliac disease
, seven were found to be exposed currently and 33 formerly to birds. As a result of clinical evaluation, BFL was considered a possible cause of undue
breathlessness
reported by three of the current bird fanciers only, and all underwent inhalation provocation tests. One alone gave positive results. We conclude that if a real association does exist between these two disorders, its clinical importance has been greatly exaggerated.

trunk associated with hepatic steal syndrome. We suggest that the marked, effort-induced lactic acidosis may be a metabolic consequence of Dunbar syndrome (stenosis of the

trunk due to compression by the crura diaphragmatica).

We report the development of lymphocytic interstitial pneumonia followed later by abdominal lymphoma in a 62-year-old woman with
celiac sprue
. She presented with
dyspnea
, cough, weight loss, and bibasilar pulmonary infiltrates. Lung biopsy demonstrated lymphocytic interstitial pneumonia and corticosteroid therapy resulted in clinical and radiological improvement. She remained well for just over a year until abdominal pain developed and investigation revealed an abdominal lymphoma. Chemotherapy effectively controlled the lymphoma while the lymphocytic interstitial pneumonia was satisfactorily managed by corticosteroid therapy. Although lymphoma is a well-recognized complication of
celiac sprue
, it is not associated with lymphocytic interstitial pneumonia, despite a number of other reports describing the occurrence of pulmonary disease in this disorder.

A 13-year-old boy suffering from
celiac disease
(CD) developed shortness of breath at exercise and episodes of malaise, fever and acute
dyspnea
following contact with pigeons. Lung function testing and chest X-ray suggested interstitial lung disease. Serum precipitins and a combined systemic and pulmonary reaction 4 h after challenge with an extract of pigeon droppings confirmed bird-fancier's lung. This case exhibits two points of interest. Extrinsic allergic alveolitis was found in childhood in combination with CD and it may be considered that both these diseases are based on one common immunologic disorder; HLA-typing of the boy's family showed that he carried HLA-DR 3 in a double dose and was therefore homozygous for HLA-DR 3. This is of some interest because extrinsic allergic alveolitis and CD are both associated with HLA-DR 3.

Neurolytic
celiac
plexus block is an established, well-developed procedure and the most accepted and applied in visceral pain; recognized by the WHO and the IASP, it is very good in palliative management of cancer pain in visceral of superior hemiabdomen. However, conventional techniques in
celiac
plexus have not been successful in patients with organomegaly and/or anatomic abnormalities, except when splanchnic nerve neurolytic blockade is used. On the other hand, conventional techniques in splanchnic nerves are highly associated with complications such as paraplegia, pneumothorax and liver or renal punction. For these reasons an alternative option has ben designed, termed transdiscal percutaneous approach of splanchnic nerves under tomographic control; this technique affords the option of improving accuracy and performance with minimum risks, particularly lung puncture and its consequences. Under this technique, 64 superior hemi-abdomen cancer patients initiated such a study (four without morphine treatment quit the study), 55% females and 45% males, visceral pain syndrome 65%, and mixed, 35%. Side effects were
dyspnea
5%, hypotension 26.7%, nausea 31.7%, diarrhea 83.3% in which diarrhea means increased peristalsis showing adequate sympathetic inhibition via splanchnic nerves), vomiting 28.3%, punction-site pain 46.7%, aorta punction 6.7%, anal pleural punction 5%. All these incidents were dealt with by conservative treatment. Student t test showed that pain intensity in all measurements after procedure was different in comparison to basal pain intensity prior to procedure (p<0.05), emphasizing that at the 12th, 18th and 24th months, there was noticeable reduction in participants number with eight, five and four participants, respectively. Morphine intake at week 1, and 1, 2, 3, 6 and 12 months after procedure was different from basal intake prior to procedure (p<0.05) with same noticeable reduction in participant numbers at last stages. Butylhioscine intake at week 1, 1, 2, 3 and 6 months after procedure was different from basal intake prior to procedure (p<0.05). NSAIDs consumption was likely during 2 months after procedure (p<0.05). Linear regression showed that butylhioscine and morphine explained low percentage of pain intensity variance, controlling statistically that effect over pain. There were no differences in pain pathophysiology with regard to cancer type. Transdiscal percutaneous approach of splanchnic nerves guided by CAT is an alternative with minimal risks, as with lung punction, confirming that inhibiting splanchnic nerves has advantages in pain release, reducing and/or eliminating morphine consumption.

We report the case of a 29-year-old man with hemoptysis. The patient came to the emergency department, where a laboratory test and chest radiograph were reported as normal. The following day the patient again had hemoptysis, though less than previously. He reported no chest pain,

, fever, catarrh, changes in urine or feces, contact with patients with bacillus disease or constitutional symptoms. Doppler ultrasound of the chest showed right basal parenchymatous condensation containing a vessel with arterial flow (in the opposite direction to the aortic flow) compatible with an aberrant vessel, possibly a sequestration, leaving the aorta above the
celiac
trunk. Because of the findings of the chest echogram and magnetic resonance study, thoracoabdominal computed tomography angiography was undertaken; this showed right basal condensation and an anomalous vessel originating 1 cm above the
celiac
trunk, supplying the right lower lobe. An aortic and pulmonary arteriogram via an arterial and right femoral vein approach confirmed the findings. The patient was treated successfully with percutaneous embolization with coils. The relevant literature is reviewed.

Diffuse alveolar hemorrhage (DAH) is a rare and life-threatening condition characterized by hemoptysis,
dyspnoea
, alveolar infiltrates on chest radiograph and various degrees of anemia. It may occur either as a primary disease of the lungs or a secondary condition due to cardiac, systemic vascular, collagen or renal diseases. Idiopathic pulmonary hemosiderosis (IPH) is a separate form of DAH of unknown origin, associated in some cases with
celiac disease
. The estimated incidence of IPH in children is 0.24-1.23 cases per million, with a mortality rate as high as 50%. Only about 500 cases of this disease have been described in medical literature. We present a case of a 9-year-old girl diagnosed with IPH, which was confirmed by the presence of many hemosiderin-laden macrophages in bronchoalveolar lavage obtained by bronchofiberoscopy. Therapy with glucocorticoids was initiated with a partial and transient response. Azathioprine and a gluten-free diet were subsequently introduced. However, the girl still suffers from recurrent episodes of hemoptysis,
dyspnea
and anemia.

Poorly differentiated neuroendocrine carcinoma has a poor prognosis, especially when associated with distant metastasis. A 60-year-old man was admitted to a private hospital because of
dyspnea
at work in 2007. Computed tomography revealed lung infarction and a right adrenal tumor sized 12 cm in diameter that was tightly compressed against the inferior vena cava (IVC). Moreover, multiple lymph node metastases around the
celiac
axis and a solitary liver metastasis at the lateral segment were observed. Thus, we planned chemotherapy without surgery. We selected a combination therapy of irinotecan (CPT-11) and cisplatin (CDDP) (i.e., IP therapy): administration of CDDP [60 mg/m(2) body surface area (BSA)] on day 1 plus CPT-11 (80 mg/m(2)) BSA on days 1 and 8. Thereafter, this protocol was repeated at 3-week intervals. After 15 months of this chemotherapy strategy, the whole lesions showed a partial response by RECIST. The primary tumor had shrunk to 4.2 cm in diameter. In November 2008, we planned surgery to perform resection of the whole lesions. Histological diagnosis of the specimen was a poorly differentiated neuroendocrine carcinoma based on the immunostaining features, i.e., synaptophysin- and chromogranin positive. There were no viable tumor cells at the dissected lymph nodes or at the liver tumor. After surgery, CPT-11 administration was continued. The patient has remained well for 9 months without recurrence.
