Breast cancer is a disease affecting millions of women worldwide. In the United States, the institution of screening mammography protocols has increased the number of suspicious breast abnormalities requiring diagnostic intervention. Up to 80% of these lesions are benign, forcing the medical community to devise minimally invasive techniques for tissue sampling. A reduction in the number of needle-localized open breast biopsies reduces the morbidity and cost associated with image-detected breast masses. Ultrasound, stereotaxis, and MRI are excellent modalities for detection of breast cancers. Image-guided, large-core biopsy systems have been developed for each of these imaging modalities, enabling successful and accurate tissue sampling and, ultimately, diagnosis of a suspicious lesion. Care must be taken to ensure correlation between imaging findings and pathologic diagnosis; if the two are discordant, further investigation is mandatory. There remains a role for needle-localized open breast biopsy, although is has been reduced significantly. Some patients prefer this method of diagnosis, and in others further investigation in required because of discordant findings. When the documented pathology of the breast abnormality is ADH, ALH, or LCIS, the patient should undergo surgical excision because of the possibility of DCIS or invasive disease in the same area. Some lesions are inaccessible with the current imaging modalities and biopsy systems available. Minimally invasive, image-guided biopsy for breast masses promises to continue to evolve, enabling physicians to diagnose breast cancer with a high degree of accuracy without significant morbidity.

When considering typical features of malignant lesions, the radiologist must differentiate between invasive cancers consisting of mass lesions and ductal carcinoma in situ, typically appearing as microcalcifications. Common malignant features of invasive cancers include irregular shape and indistinct or spiculated margins. In microcalcifications, segmental distribution and pleomorphic shape are the features with the highest predictive value of malignancy. However, there is a broad spectrum of findings that confound the reliable differentiation between benign and malignant lesions. The American College of Radiology has established the Breast Imaging Reporting and Data System (BI-RADS) for standardizing radiological terms and reports in mammography screening. The Breast Imaging Reporting and Data System provides diagnostic categories that have implications for guidance regarding follow-up or biopsy of mammographic breast lesions. BI-RADS 3 lesions are considered probably benign with a malignancy risk < 2%. These findings can be followed up at predetermined intervals according to current recommendations. Suspicious lesions with a substantial probability, but without the classic appearance of malignancy, are classified as BI-RADS 4. Minimal invasive biopsy should be considered in patients with these lesions. BI-RADS 5 lesions are highly suggestive of malignancy. It is recommended that appropriate action should be taken for these most suspicious lesions. The accuracy of the mammography as the primary diagnostic tool can be increased by the use of ultrasound and physical examination. In some situations, MRI is helpful for further evaluation. However, classifying the lesions with precision is not trivial since overlap exists between malignant and benign features.

We herein present a case of non-palpable ductal carcinoma in situ (DCIS) treated with successful breast-conserving surgery (BCS) using MR markers made with commercially available materials in the supine position. The patient was a 40-year-old woman found to have a non-mass lesion by routine screening ultrasonography (US). The margin of the lesion was unclear by the US examination, whereas contrast-enhanced MR mammography (MRM) revealed a clear margin of the lesion. After the resection line was determined by under US guidance, the area was marked by original markers for MRI using the pills of Breath Care inside a Penrose drain. MRM was performed in the supine position (supine MRM), which is similar to the posture used for surgery. The lesion was located within the resection area, but it was not at the center of the resection area; thus, the resection lines were modified using information obtained by MRM. The histological evaluation of the BCS specimen demonstrated a wide extension of the DCIS. The lesion was located at the center of the resection specimen, and the surgical margin was free. In conclusion, since our procedure can be performed easily, it is useful for evaluating the extent of the lesion as determined by the US examination in difficult cases.

Recently, the number of radial scars (RS)/complex sclerosing lesions (CSL) of the breast has been increasingly detected by mammography screening. Six RS/CSL cases encountered clinicopathologically in the last 2 years are presented. All patients were pre-menopausal. Three cases were detected by ultrasonography (US) screening, and the others were detected by mammography (MG) screening. Partial mastectomy was carried out for both diagnosis and treatment, since it was difficult to discriminate whether RS/CSL accompanied breast cancer even by US, MG, MRI, aspiration cytology, and spring-loaded core needle biopsy (CNB). RS/CSL was histologically confirmed in all cases, and atypical ductal hyperplasia (ADH) or ductal carcinoma in situ (DCIS) accompanied RS/CSL in each case. At present, the clinical diagnosis of complicated breast cancer is difficult. Therefore, we selected partial mastectomy that resects a wider area than surgical biopsy to adequately diagnose breast cancer and to achieve a resected margin that is free from breast cancer. But it may be that partial resection should be performed in case of older age with larger RS/CSL, since it is over-surgery for RS/CSL without breast cancer. Further studies where complicated breast cancer is certainly identified are necessary.

In younger women at high risk for developing breast cancer the value of mammography is limited by a higher prevalence of breast tissue density, low rate of DCIS in gene carriers, faster growing tumours and concerns over radiation exposure. We report on our experience of MR screening in high risk patients over a three year period. Women at high risk of developing breast cancer were offered an MRI scan and an Ultrasound in addition to their annual mammogram for two years. The following MR-protocol was used: pre-contrast T2 STIR sequence and pre contrast 3D FLASH sequence, post contrast axial dynamic 3D FLASH sequence. Seventy two women consented to participate in this study. One hundred thirty nine breast MRI examinations were performed. Two pre-cancerous lesions and an axillary lymph node metastasis were found, but the majority of the lesions were benign. Difficulties in screening young women at high risk are discussed in this paper. In our study three lesions of significance were detected. Two lesions were precancerous thus curable. The recall rates show the difficult nature of screening younger breasts. MRI generated more findings judged as uncertain, so short term-term follow up studies or MR-guided biopsy techniques are required.

Dynamic contrast-enhanced MRI (DCE-MRI) is an evolving tool for determining breast disease, which benefits from the move to imaging at 3 T. It has major capabilities for the diagnosis, detection and monitoring of malignancy. It benefits from being non-invasive and three-dimensional, allowing visualisation of the extent of disease and its angiogenic properties, visualisation of lesion heterogeneity, detection of changes in angiogenic properties before morphological alterations, and the potential to predict the overall response either before the start of therapy or early during treatment. In addition, DCE-MRI is emerging as a powerful tool for screening high-risk patients and for detecting high-grade ductal carcinoma in situ. However, there are also a number of limitations, including the overlap in enhancement patterns between malignant and benign disease, the failure to resolve microscopic disease particularly in the neoadjuvant setting, and the inconsistent predictive value of the enhancement pattern for clinical outcome. Careful consideration should be given to the technical requirements of individual examinations and the need for automation of post-processing techniques to appropriately handle the growing volume of data acquired. Research continues, focusing on the use of higher field strengths with improved spatial and temporal resolution data, improving understanding of the mechanism of contrast enhancement at the cellular level, and developing macromolecular and targeted contrast agents.

Improving the prevention and detection of preinvasive ductal carcinoma in situ (DCIS) is expected to lower both morbidity and mortality from breast cancer. Transgenic mouse models can be used as a 'test bed' to develop new imaging methods and to evaluate the efficacy of candidate preventive therapies. We hypothesized that despite its microscopic size, early murine mammary cancer, including DCIS, might be accurately detected by MRI. C3(1) SV40 TAg female mice (n=23) between 10 and 18 weeks of age were selected for study. Eleven mice were subjected to in vitro imaging using a T(2)-weighted spin echo sequence and 12 mice were selected for in vivo imaging using a T(1)-weighted gradient echo, a T(2)-weighted spin echo and high spectral and spatial resolution imaging sequences. The imaged glands were carefully dissected, formalin fixed and paraffin embedded, and then H&E stained sections were obtained. The ratio of image-detected versus histologically detected cancers was obtained by reviewing the MR images and H&E sections independently and using histology as the gold standard. MR images were able to detect 12/12 intramammary lymph nodes, 1/1 relatively large (approximately 5 mm) tumor, 17/18 small (approximately 1 mm) tumors and 13/16 ducts distended with DCIS greater than 300 microm. Significantly, there were no false positives--i.e., image detection always corresponded to a histologically detectable cancer in this model. These results indicate that MR imaging can reliably detect both preinvasive in situ and early invasive mammary cancers in mice with high sensitivity. This technology is an important step toward the more effective use of non-invasive imaging in pre-clinical studies of breast cancer prevention, detection and treatment.

A 56-year-old woman underwent FDG-PET screening, which demonstrated delayed-phase uptake in the lower part of the left breast. The findings of mammography, ultrasonography, MRI and cytological examination were compatible with ductal carcinoma in situ (DCIS), but core needle biopsy showed no evidence of malignancy. Therefore, partial resection of the left breast with sentinel lymph node biopsy was performed to make a definite diagnosis. Histological examination showed that this tumor was low grade DCIS. FDG-PET is a very useful examination to detect malignant diseases, but it is quite difficult to distinguish them from benign ones. It is suggested that delayed-phase uptake of FDG-PET is useful for diagnosis of DCIS.

Complete excision is key to successful breast-conserving treatment for patients diagnosed with ductal carcinoma in situ (DCIS). Patients with extensive DCIS should be considered the choice of mastectomy (followed by reconstruction), while for smaller DCIS breast-conserving treatment should be performed by complete excision. This review reports on the effect of margins on the clinical management of patients with a diagnosis of DCIS. The role of preoperative imaging including MRI, surgical procedures, and pathology are described.

The American Cancer Society now recommends annual MRI screening for women at 20-25% or greater lifetime risk of breast cancer. The role of MRI screening in other risk subgroups is unproved because of insufficient data. Our study comprised 209 breast MRI scans carried out in 171 asymptomatic patients (age range 22-67 years, mean 46 years), referred between January 2005 and June 2008. Targeted ultrasound was carried out in 32 episodes (15%) and biopsies were taken in 23 patients (13%). In four patients, MR-guided procedures were required to establish a diagnosis, two using hook-wire localization and two by means of vacuum-assisted biopsy. Seven cancers were detected by MRI in the 171 patients, with a yield of 4.1%. Only one of the seven cancers was also shown by x-ray mammography. Four patients had invasive ductal cancer (all axillary node negative) and three had high-grade ductal carcinoma in situ or pleomorphic lobular carcinoma in situ. The three women with in situ disease were all potentially high risk, based on the National Breast and Ovarian Cancer Centre (NBOCC) criteria. Three women with invasive breast cancer were at only average risk based on NBOCC criteria, but two of these had extremely dense breasts. A fourth patient, found to have multifocal invasive cancer, had a personal history of contralateral breast cancer, but no relevant family history. Our findings suggest that breast MRI could be used to screen a larger Australian population at increased risk of developing breast cancer.
