A retrospective review of 150 patients with adenocarcinoma of the prostate was done to determine the morbidity associated with lymphadenectomy performed as an independent procedure. A total of 77 complications, 17 of which were considered major, was noted in 50 patients. Wound drainage and transection of the spermatic cord were associated with increased morbidity. Systemic antibiotic therapy afforded no protection against wound infections, while intraoperative irrigation was protective. Lymphoceles and lower extremity edema occurred infrequently in our series, although only a few patients underwent postoperative pelvic irradiation therapy.

Pelvic lymphocele is an infrequent complication of pelvic surgery, usually presenting shortly after surgery. We report a case of an infected pelvic lymphocele presenting more than 1 year after a staging pelvic lymphadenectomy for adenocarcinoma of the prostate. This case illustrates that late infection of pelvic lymphoceles can occur following a pelvic lymphadenectomy and radical prostatectomy.

Symptomatic lymphocele presented after pelvic lymphadenectomy for localized carcinoma of the prostate is a relatively rare complication. We treated a case of infected lymphocele presenting 4 months after a limited staging pelvic lymphadenectomy and a radical prostatectomy for adenocarcinoma of the prostate. The patient was a 70-year-old Japanese man having the chief complaint of fever, right lower abdominal pain, nausea and vomiting. Pelvic computed tomography showed a 14 cm cystic mass with a thick capsule on the right iliopsoas muscle. It was suspected to be an infected lymphocele. After percutaneous drainage, sclerotherapy (povidone iodine, minocycline and ethanol) and antibiotic drugs, the infected lymphocele was resolved.

Pelvic lymphocele is an infrequent complication of pelvic surgery, usually presenting shortly after surgery. We report 2 cases with a delayed infected pelvic lymphocele presenting after transperitoneal pelvic lymphadenectomy and robotic radical prostatectomy for adenocarcinoma of the prostate. These cases illustrate that late infection of pelvic lymphoceles may occur following radical prostatectomy and pelvic lymphadenectomy. The practicing urologist should be aware of this possibility and look for an infected lymphocele in postoperative pelvic lymphadenectomy patients presenting with fever and leukocytosis of uncertain etiology, regardless of the time elapsed since surgery. To date, there is a paucity of data in the literature on robotic- assisted laparoscopic resection of a lymphocele after radical prostatectomy. The minimally invasive technique can be considered as a possible alternative to lymphocele percutaneous drainage. It is effective, results in minimal patient morbidity and allows for rapid recovery.
