We prospectively studied patients with Streptococcus bovis septicemia for the presence of gastrointestinal lesions. This study was prompted by our reported findings of the association of fecal carriage of S. bovis with carcinoma of the colon. We studied 29 patients with 30 episodes of S. bovis septicemia. Fifteen completed gastrointestinal evaluations that included colonscopy, surgery, or autopsy. Eight of these had carcinoma of the colon, three had adenomatous polyps of the colon without
carcinoma
, and two had
carcinoma of the esophagus
. The 14 patients who did not have complete evaluations included one each with carcinoma of the stomach, gastric lymphoma, and adenomatous polyp of the colon and three with colonic masses not further delineated. Nineteen patients had no gastrointestinal signs or symptoms or stools positive for occult blood at admission. The results of our study suggest that all patients with S. bovis septicemia need aggressive evaluation of the gastrointestinal tract, especially the colon.

tends to be a complication of cardiospasm in patients inadequately treated for the primary condition. Early diagnosis and an adequate Heller operation for cardiospasm will lessen the chance of cancer developing in megaesophagus with stasis. When finally diagnosed, this variety of esophageal

has an extremely poor prognosis. An even more important claim upon the clinician is to be alert to the hazards of continued stasis in megaesophagus and to devise earlier techniques of recognition, including more frequent recourse to biopsy during esophagoscopy.

A clinical series of 216 patients with
carcinoma of the oesophagus
or cardia, who underwent surgical resection of the oesophagus, is presented. There were 104 patients with oesophageal
carcinoma
and 112 patients with
carcinoma
of the cardia. Histologically, there were 102 squamous cell carcinomas, 98 adenocarcinomas, 11 anaplastic carcinomas and 7 non-differentiated carcinomas. Oesophago-gastrotomy was the procedure mostly used; colon interposition was done in only 13 cases. The hospital mortality was 21%. The 5-year survival rate for the whole series after oesophageal resection was 23%. The duration of symptoms, location of the tumour, age and sex of the patients, pre- or postoperative radiotherapy and the histological type of the tumour had only a minor bearing on survival. The two most important prognostic factors were the spread of the tumour at time of operation and a preceding lye stricture. The 5-year survival rate was 34% for the patients with a local tumour at operation and 44% for those in whom the
carcinoma
developed at the site of a previous lye stricture. The variance of the results in the literature is discussed. Surgical approach to the
carcinoma of the oesophagus
and cardia is recommended in all the cases in which the patient and tumour seem to be eligible for resection.

The carcinogenic activity of orally administered N-bis(2-hydroxypropyl)-nitrosamine (DHPN) in male Wistar rats was evaluated with respect to its dose. DHPN was administered at two doses, 100 ppm and 500 ppm, in the drinking water to rats for 25 to 52 weeks. Tumors developed in the lung, liver, and thyroid of rats receiving 100 ppm DHPN and in the lung, liver thyroid, esophagus, kidney, and urinary bladder of rats receiving 500 ppm DHPN. The principal target organ was the lung in rats receiving either 100 or 500 ppm DHPN, indicating that the carcinogenic action of these doses of DHPN was similar to that of higher doses previously reported.
