We reviewed 69 patients who underwent surgery for primary malignant tumors of the small bowel over a 15-year period. Patients with periampullary lesions were excluded, as were those with known adenocarcinomas in other sites. There were 46 males (67%) and 23 females (33%) with an average age at presentation of 52.6 years (range 4-92). Of the 69 patients, four (6%) were asymptomatic. The mean duration of symptoms before diagnosis, in the remaining 65 symptomatic patients, was 3.6 months (range 1 day 1.2 years). The most frequent symptom was abdominal pain (83%), followed by nausea and/or vomiting (54%), and weight loss (43%). Abdominal mass was the most common finding on physical examination (29%), followed by abdominal distension (24%) and abdominal guarding and rigidity (14%). Laboratory examinations were normal in 26 patients (40%). Radiographic study of the duodenum and small bowel was abnormal in 87%. The diagnosis was suspected preoperatively in 51%. Lymphoma was the most common tumor (42%), followed by adenocarcinoma (38%),
carcinoid
(10%) and leyomiosarcomas (10%). In 41% the neoplasm was located in the jejunum, in 33% in the ileum, in 22% in the duodenum and in 4% multiple sites were found.
Lymph node metastases
were found in 45% and vascular invasion was found in 19%. Of the 65 symptomatic patients 43% presented as surgical emergencies, the rest had elective surgery. 61% had a curative resection, the rest were palliative or a bypass. The operative mortality rate was 10% and the morbidity rate was 24%. Ten patients underwent emergency surgery, developed a complication, giving a morbidity rate of 32% in this group. tumors was 60% while the 5-year survival for small bowel malignant lymphoma and adenocarcinoma was 40 and 34.28% respectively.

Carcinoid tumors
occur most frequently in the gastrointestinal tract. Despite their ability to produce hormones, most of the midgut and hindgut carcinoids covered in this study are clinically silent, and the diagnosis is often not made before emergency surgery or evaluation for liver metastases. Because the rate of lymph node involvement and the prognosis of
carcinoid
tumors depend on their site and size, surgery refers to these two factors too.
Lymph node metastases
are most commonly found with small bowel carcinoids (20-45%), providing the rationale for an extended resection including the adjacent lymph node drainage area.
Carcinoid tumors
of the appendix < 1 cm in diameter rarely metastasize, simply requiring appendectomy for treatment. Lesions > 2 cm should be treated by right hemicolectomy because of their approximately 30% risk of lymph node metastases. Resection should always be done for
carcinoid
tumors of the colon resection as for adenocarcinomas. Rectal carcinoids < 2 cm rarely metastasize, directing the conclusion that for these smaller lesions local excision is sufficient; for lesions >2 cm a standard cancer resection should be performed provided distant metastases are absent. In general, the younger the patient or the larger the primary tumor, the more aggressive the treatment should be.

Results of radical surgical treatment of 86 patients with early cancer of the stomach are analyzed. Among revealed macroscopic types of early cancer of the stomach (B0), type I (elevated) was seen in 26 (31%) patients, type IIa (superficial raised) - in 8 (9%), type IIb (superficial plane) - in 7 (8%), type IIc (superficial excavated) - in 20 (23%), type III (ulcerated) - in 25 (29%) patients. Adenocarcinoma was the prevalent tumor - 84 (98%) patients, in 1 (1%) case undifferentiated cancer was revealed and in 1 (1%) -
carcinoid
tumor. Tumor invasion was limited by mucosa in 49 (57%) patients, in submucosa - in 37 (43%).
Metastases to lymph nodes
were diagnosed in 4 (5%) patients. Subtotal proximal resection performed in 6 patients, subtotal distal resection - in 60, gastrectomy - in 20 patients. Lymphadenectomy corresponded to D1 in 30 cases, to D2 - in 51, to D3 - in 5 cases. Complications after surgery were seen in 21 (24%) patients. Cancer in the gastric stump was diagnosed in 3 (4%) patients 2, 4 and 6 years after primary subtotal distal resection of the stomach with D1 lymphadenectomy. All these patients underwent extirpation of gastric stump. Radical surgery for early cancer of the stomach must include extended lymphadenectomy D2 or more.

Two cases of multiple
carcinoid
tumors of the rectum with numerous micronests of
carcinoid
tumors are reported. The patients were 51- and 58-year-old males. Many
carcinoid
tumors and numerous
carcinoid
micronests were found in the resected rectum; the total number of
carcinoid
tumors, groups of micronests, and solitary micronests was 69 in the first case and 62 in the second case. The micronests, consisting of a few to many endocrine cells, were observed in the lamina propria, muscularis mucosa, and/or submucosa. Micronests increased in number, gathered and formed
carcinoid
tumors, which were up to 8 mm in diameter. It was found that a nest of the
carcinoid
tumors in the lamina propria showed continuity with the endocrine cells of a crypt in the different
carcinoid
tumors in both cases. The
carcinoid
tumor and micronest infiltrated the nerves and ganglions in the muscularis mucosa and submucosa. Nests of the
carcinoid
tumors and micronests were surrounded by S-100-positive cells.
Lymph node metastases
of the
carcinoid
tumor were found in both cases. Rectal
carcinoid
tumors may originate from endocrine cells of the crypts, and multiple
carcinoid
tumors may occur heterogeneously.
