A total of 71 patients has been reported in the English literature to have developed
carcinoid
tumors in the fundus of the stomach in association with chronic atrophic gastritis secondary to pernicious anemia. The tumors appear to develop from argyrophilic cells in response to hypergastrinemia produced by hyperplastic G cells in the antrum of the stomach. We report a similar patient who, in addition, had a neuroendocrine tumor develop in the neck of the pancreas, which obstructed the pancreatic duct and resulted in severe
chronic pancreatitis
in the body and tail of the gland. There was an associated splenic vein thrombosis with left-sided portal hypertension. The pancreatic neoplasm was treated by excision, including the caudal 85 per cent of the pancreas and spleen. The gastric carcinoids appeared to diminish in size in response to a octreotide acetate (Sandostatin, Sandoz Pharmaceuticals; Hanover, NJ) administered to promote closure of a low-volume postoperative pancreatic fistula. The gastric lesions may require surgical treatment in the future. We were unable to find other examples of pancreatic neuroendocrine tumor occurring in association with pernicious anemia in a search of the English literature.

A 60-year-old female with a history of
chronic pancreatitis
and previous pancreaticojejunostomy has been submitted to distal gastric resection for recurrent perforating ulcers. A
carcinoid
tumour was identified in the surgical specimen. Hypergastrinaemia was subsequently diagnosed and tumour localization was carried out by a new scintigraphic technique with 111In-Pentetreotide. The tumour was found to be a metastasis confined to a peripancreatic lymph node. Surgery was successful, and 6 months later the patient is still eugastrinaemic. The different imaging methods for the identification of gastrinomas are discussed together with the current therapeutic options.

This is a case report of a patient with an initial diagnosis of
chronic pancreatitis
who actually had metastatic
carcinoid
tumor. His symptoms of abdominal pain, weight loss, and diarrhea were manifestations of the large tumor bulk within the liver as well as carcinoid syndrome. Although abdominal CT scans showed multiple liver lesions, the primary lesion was not identified by conventional imaging studies. However, the mid-gut primary lesion was visualized on in-111 labeled octreotide scintigraphy; where the liver lesions were better delineated and seen to be separate from the normal pancreas when the Tc-99m sulfur colloid images were compared to the octreotide images.

carcinoid
tumor arising in the Vater's papilla with repeated episods of pancreatitis. The patient is a 28 year-old-woman who had repeated abdominal pain with elevated serum amylase and had been treated as
chronic pancreatitis
. Computed tomography (CT) revealed a slight dilatation of the main pancreatic duct from the pancreatic head to the tail, and mild swelling of the pancreas. A submucosal tumor measuring 1.3 cm in diameter was detected in the ampulla of Vater by esophagogastroduodenscopy (EGD), and total papillectomy was performed under the suspicious of
carcinoid
tumor. The tumor was not encapsulated, 1.0 cm in diameter, undefined, and whitish in color. Histologically monomorphic tumor cells with lightly eosinophlic cytoplasm and round nuclei proliferate in trabecular and solid patterns. Immunohistochemically tumor cells were positive for neuron-specific enolase, chromogranin A and synaptophysin, and the tumor was diagnosed as

Standard Whipple procedure performed in 91 patients, pyloro-saving PDR - in 32 cases and total pancreatoduodenectomy in one patient. 3 types of pancreatic stump preparation were used. Pancreatojejunal anastomosis (PEA) (n=63) considered to be the first, pancreatogastroanastomosis (PGA) (n=46 ) - the second and external pancreaticostomy (n=15) - the third. Overall anastomosis failure rate was 12,1% (n=15). By PEA, failure rate was 27,5% and was noticed predominately in cases of connecting all area of pancreatic slice with the gut (43,5%). PGA failure rate was 2,1% (n=1). Therefore, PDA should be considered to be a reliable pancreatodigestive anastomosis, allowing a decrease of postoperative complication rate to 26,1% and lethality rate to 2,1%.
