A correlated morphological study, employing endocrine cell stains, immunofluorescence, and electron microscopy, was performed on biopsy specimens taken from a pancreatic tumor and liver metastases in a woman with hypoglycemic symptoms and high fasting insulin levels. The study revealed the tumor to be composed of two different endocrine cell populations, irrespective of the primary or metastatic growth. The first cell type fulfilled all the morphological characteristics of the islet B cells. The second was argyrophil (with the Grimelius silver method) and showed the morphological pattern of polypeptide-hormone-producing cells. With the lack of a detectable symptomatology, normal blood levels of the hormones other than insulin, and the negative results of a large number of immunofluorescence tests, we were unable to indetify the specific nature of the second type of cells.

A periodic acid-chromic acid-silver methenamine techniqe for visualizing glycoproteins at the electron microscope level was applied to colonic mucosa taken from areas adjacent to and remote from carcinoma. Normal control mucosa was obtained by biopsy of patients with no known gastrointestinal disease. Non-oxidized control sections were run in parallel. Quantitative and qualitative differences in glycoproteins were detected in the mucosa adjacent to carcinoma ('transitional' mucosa, as we call it) as compared with the normal. Furthermore, the vesicles in both the 'intermediate' and absorptive cells elaborate a glycoprotein product and it seems that a direct relationship exists between the increased vesiculation and the markedly developed 'fuzzy coat' in the 'transitional' mucosa. It is suggested that these findings may represent one of the features of an early stage of carcinogenesis. Histochemical and ultrastructural techniques of the kind used in this study may thus be of value in identifying or predicting malignancy in the colonic epithelium.

Six cases of peripheral ganglioneuroblastoma are described and analyzed; in general these contained neuroblasts, mature ganglion cells and cells of intermediate degrees of maturity. Portions of the tumor resembled the glial tissues of the central nervous system, but this is spurious, no glial cells or fibers being present. Such tissues contained cells of intermediate maturity, immature axons which characteristically stain poorly with silver stains for axons, and a paucity of connective tissue fibers. These tumors exhibit a tendency for continued maturation with decreased malignancy, and for anaplastic change with increased malignancy. These opposing tendencies make it difficult to predict the outcome with complete certainty, except possibly in those cases in which maturation is complete. Schwann cells are present in those tissues which have matured to form neurons with axonal processes. The Schwann cells are probably reactive, not neoplastic. The presence of Schwann cells in such mature tissues is most readily explained on the assumption that Schwann cells are mesenchymal in character, and have been formed by differentiation of multipotential primitive reticular cells in response to the formation of axons by the maturing neurons.

With the aid of a simple silver-staining procedure, large numbers and unusual arrays of nucleolar argyrophilic granules were found in Novikoff hepatoma, KB, and HeLa cells. Some of these arrays consisted of linearly arranged discrete granules, and others were in two to three rows each containing three to five granules. Corresponding formations were not found in either the normal or regenerating liver nucleoli which contained an argyrophilic network in which the dark granules were apparently associated with the less dark argyrophilic fibrils of a reticulum. The nucleolar argyrophilic granules were readily identifiable in the separated daughter nuclei of the tumor cells in telophase, suggesting that the increased nucleolar activity of the G1 phase begins in these cells even before cell division has been completed.

An ependymoma was induced in the brain of a BD-IX rat by repeated doses of MNU. In vitro grown cells of the tumor were fibroblasts and stellate cells considered to be glial. Reimplantation of these cells into the brain of another BD-IX rat resulted a tumor growing in the meninges. This tumor was then repeatedly transplanted over several passages. Its gross morphology resembled that of a sarcoma; however, glial in cells could be demonstrated with silver methods. A cell line derived from the first transplanted tumor group proved to contain S-100 protein in traces in the first passages in vitro. Explanted cells of the primary ependymoma were cloned. Two clones were established, RGL 1 and RGL 2. The cells of both clones resembled morphologically the stellate elements of the primary explant. RGL 1 contained S-100 protein. It was cloned again in the 16th subpassage. Four clones were established. Only 1 contained S-100 protein in traces. The clone RGL 1 had a subdiploid karyotype with a modal number of 39 and a large submetacentric marker. It was subcloned in the 25th passage with two clones, both having a karyotype of 41 chromosomes.

Explants of nine infiltrating duct carcinomas of the human female breast, maintained in organ culture, were exposed to the glycoprotein precursors, L-[3H]furcose and [3H]glucosamine, in order to determine the cellular distribution of newly synthesized glycoprotein as revealed by autoradiography with the light and electron microscopes. Explants were incubated with a single isotope for 2 hr, at which time some of the labeled explants were removed for autoradiographic analysis while the rest were transferred to nonradioactive medium for an additional 24 hr. After exposure to label for 24 hr, autoradiography with each isotope was similar and showed strong reactions over most tumor cells. The reactions were due to clumps of silver grains over intracytoplasmic lumina within single tumor cells and silver grains over Golgi saccules, cytoplasmic vesicles, lysosome-like bodies, lateral and basal plasma membranes, and microvilli. Extracellular ductular structures were also heavily labeled. At the later sampling time, Golgi saccules often showed a reduced reaction while the reactions over other organelles and intracellular and extracellular ductular structures remained strong. The observations suggest that in our in vitro system the tumor cells are metabolically active and complete the synthesis of the carbohydrate side chains of glycoproteins within the Golgi apparatus. From there, some of the newly synthesized glycoprotein appears to migrate to plasma membranes and lysosome-like bodies. Furthermore, our data support the notion that many duct carcinomas of the breast exhibit secretory activity by showing that some newly synthesized glycoprotein also appears to become products that are secreted into intracellular and extracellular ductular structures.

Twenty cases of pulmonary tumorlet are presented. Approximately one-third of the tumors occurred in lungs severely scarred by bronchiectasis or other inflammatory processes; the other two-thirds were found in lungs with little to no scarring. In the former instances, tumorlets were found in large scars in which identifying architectural features had been destroyed. In the latter, the tumorlets were seen in minute fibrous nodules, surrounding or obliterating small bronchi or bronchioles. Argyrophilic granules were demonstrated in 14 of 15 cases; in the electron microscope these were consistent with neurosecretory granules. Their location, silver staining properties, and ultrastructure suggest that pulmonary tumorlets are minute peripheral carcinoid tumors.

Twenty-six cases of so-called "minute pulmonary chemodectoma" are presented. The patient population showed a marked female preponderance, and there appeared to be an association of the lesion with pulmonary injury from a variety of causes including cardiac failure, chronic bronchitis and emphysema, and thromboemboli. Half the cases had multiple tumors. Microscopically, the tumors consisted of nests of cells in the interstitial tissue near small veins. Argentaffin and argyrophil stains failed to demonstrate cytoplasmic granules in any case. By electron microscopy, the nests were composed of large cells with broadly interdigitating processes connected by many well-formed desmosomes. The cytoplasm was filled with numerous 60-A filaments. The Golgi apparatus was prominent, while other organelles were sparse. No secretory granules were identified. It is concluded that the fine structure and lack of silver-positive granules are inconsistent with the morphology of previously reported paragangliomas, but that there is a resemblance at the light and electron microscopic level to meningeal arachnoed cells and the cells of meningiomas.

Sixty-nine sheep were infected with bovine leukemia virus from bovine lymphosarcoma materials. Twenty-four developed lymphosarcoma and died from 13 to 66 (average, 29) months later. Circulating lymphocytes were increased to leukemia levels (70,000 to 403,000/cu mm blood) in only eight sheep within 2 to 3 months of death. Various lymph nodes and visceral organs including heart, abomasum, uterus, kidneys, and urinary tract were commonly affected as in cattle with the adult form of lymphosarcoma. In one sheep the skin was involved. The liver was involved in only one case. This was in contrast to more frequent involvement reported in literature for naturally occurring lymphosarcoma. The neoplasms in experimental sheep are regarded as a mixture of reticulum or histiocytic cells and lymphoid cells with transitional forms supported by a usually sparse and diffuse fibroplasia and a web of silver-staining reticulin fibers.

A case of a primary thymic carcinoid tumour of a 32 year old male is reported, and the previously published 25 cases are reviewed. The neoplasms occur in adults, predominantly in males: they are located in the anterior or antero-superior mediastinum. The only sufficient treatment is the surgical excision; through radiotherapy may be indicated in some cases, too. The tumour is characterized by the formation of rosettes, ribbons and garlands; sheet-like or medullary areas, and large clusters with central necrosis. The tumour is positive for the Grimelius argyrophil silver stain. Ultrastructurally numerous dense-core "neurosecretory" granules are observed. The formal pathogenesis is similar to that of carcinoid tumours of other locations; particularly of those arriving from derivates of the foregut, respectively from the APUD cell system. At present it cannot be decided whether this particular tumour of the thymus represents an autochthonic thymic carcinoid or simply a teratoma with a differentiation in one direction in the sence of a simplified teratoma.
