Abnormalities of trace elements such as aluminum, silicon, zinc, manganese and nickel were found in chronic hemodialysis (HD) patients. The relationship between serum concentration of trace elements and complications such as anemia bone disease, neurological disorder and nutritional disorder in 100 chronic HD patients was examined. Serum Al and Si levels were very high in these patients, and were negatively correlated with indicators of anemia such as RBC, hemoglobin and hematocrit values. Serum Al and Si levels in the HD patients with peripheral neurological disorder (excluding encephalopathy) and with bone disease were significantly high. The levels of serum Zn, Mn and Ni were low, and were correlated with total serum protein levels. In summary, in order to prevent some complications in chronic HD patients, it is very important to regulate levels of trace elements.

22 patients with chronic renal failure, maintained on chronic maintenance dialysis with a dialysate calcium of 1.5 mmol/l, were randomly selected for measurement of pre- and postdialysis plasma calcium fractions. Total, ionized and protein-bound calcium fractions in plasma showed significant increases following dialysis, but with correction for hemoconcentration due to ultrafiltration from dialysis, only protein-bound calcium showed a significant increase. Change of in vitro pH from 7.35 to 7.44 did not influence calcium binding. Similarly, in vitro addition of calcium to pre- and postdialysis blood samples did not result in significant differences for protein-bound calcium fractions. Comparison of dissociation constants for protein-bound calcium between pre- and postdialysis samples showed a significant change. Our results indicate that dialysis alters the affinity of serum protein by some unexplained mechanism and may contribute to the genesis of dialysis-bone disease.

A 67-year-old man underwent total resection of the left lung because of adenocarcinoma, with no evidence of extra-thoracic spread of the cancer. On follow-up, metastatic bone disease was documented and chemotherapy (CMT) which included cisplatin, docetaxel, vinorelbine, and topotecan was administered. Six months after completion of CMT, a rise in total serum protein was noted. Bone marrow biopsy revealed the diagnosis of an IgG lambda myeloma. We report a case of a man with adenocarcinoma of the lung who presented multiple myeloma after completion of chemotherapy.
