Campylobacter jejuni is a common cause of enteritis, and has been isolated from patients with bacteremia, meningitis, and cholecystitis. We describe here an unusual case of a chronically inflamed bursitis infected with C. jejuni.

A review was made of the histories of 125 adult patients admitted for acute gastroenteritis (GEA) due to Salmonella no typhi. The complications that appeared in the series are analyzed. A total of 16 patients (12.8%) presented complications: the most common was bacteremia, 3 had renal tubular necrosis, 2 alithiasic cholecystitis that required surgery, 2 toxic megacolon, 2 rectal hemorrhage, 1 erythema nodosum and 1 intestinal perforation. We reviewed the hospital admissions of 168 patients with acute leukemia to determine the incidence of persistent fever following recovery from chemotherapy-induced granulocytopenia. This phenomenon was observed during 26 (15.5%) hospital admissions. The microbiologically and/or clinically documented causes identified in 23 instances included viral infection (two patients), perirectal abscess (two patients), Hickman catheter-related bacteremia (two patients), intraabdominal infection (four patients), and nine fungal infections (five resolving pneumonia, one disseminated candidiasis, three focal hepatic and/or splenic mycosis). One patient had both cholecystitis and a pneumonia of uncertain origin and three patients had drug reactions. Although overall the source of fever was usually readily apparent, focal hepatic and/or splenic mycosis produced protracted fevers that were difficult to diagnose. Visceral fungal infection should be a leading diagnostic consideration in patients with leukemia who remain persistently febrile following recovery from chemotherapy-induced granulocytopenia.

In a five-year period, 29 cases of bacteremia and/or meningitis in adults caused by Haemophilus influenzae were seen in our large community hospital. There were 17 cases of bacteremic pneumonia and 12 cases of serious extrapulmonary infections. The extrapulmonary infections included cases of endocarditis, meningitis, cholecystitis, epiglottitis, tubo-ovarian abscess, and cellulitis. In contrast with the pediatric experience, H influenzae type B was the causative pathogen in only 45% of patients and only one isolate was ampicillin resistant.

Eleven of 20 patients with chronic granulomatous disease (55%) developed serious gastrointestinal complications requiring surgical consultation or operation over a 10-year period with an average of 2.2 complications per patient. The mean age of onset of symptoms was 12 years (range 2.5 months to 25 years), and 91% of the patients were male. Admission for gastrointestinal complications constituted 18% of all admissions for these patients; the mean hospitalization time was 27.8 +/- 3.5 (SEM) days. Hepatic abscess, the most common complication, occurred in 16 instances. Perirectal abscess developed in three patients and gastric outlet obstruction developed in two patients. Other complications included appendicitis, acalculous cholecystitis, and Salmonella enteritis. Open hepatic debridement and external drainage combined with long-term intravenous antibiotics (mean 25.2 +/- 4.8 days) were curative in every case, but operative morbidity was frequent and severe. Twelve major complications accompanied open hepatic drainage in 14 cases including wound disruption, prolonged febrile course, subhepatic abscess, and recurrent hepatic abscess. Five secondary operations were required for treatment of these complications. Gastric outlet obstruction, by contrast, was successfully managed nonoperatively. Staphylococcus aureus was an etiologic agent in 66% of the cases, but many other aerobic gram-positive and gram-negative organisms were isolated. Anaerobic bacteria were unusual. Bacteremia occurred only once.

The clinical course of patients with hematological disease, especially after treatment, is often complicated by gastrointestinal infections. Between 1986 and 1990 a total of 18 patients affected with hematologic disease and presenting with an acute abdomen were admitted to the surgery department at the University of Rome "La Sapienza". Most patients were affected with acute or chronic myeloid leukemia (61%) and lymphoma. Five patients with acute appendicitis, three with necrotizing enterocolitis, three with spontaneous hemoperitoneum, three with cholecystitis, two splenic infarctions and two intestinal occlusions were diagnosed. Symptoms were often vague and non specific and blood counts revealed neutropenia in all but two patients, while anemia was characteristic in spontaneous hemoperitoneum and in neutropenic enterocolitis. Fungemia occurred in only two cases while bacteremia was present in seven. The most critical patients were those affected by neutropenic enterocolitis and acute cholecystitis. Sonography was meaningful in the diagnosis of hemoperitoneum, splenic infarct and acute cholecystitis. All patients underwent surgical procedures within 48 hours of admission to the department. In all cases peritoneal washing was performed and at least one peritoneal drainage was left. In all cases of necrotizing enterocolitis, intestinal resections, either ileal or colonic, were followed by an immediate anastomosis in two layers. Intensive hematological and antibiotic post surgical care was performed in all patients. Seven patients presented minor complications (38.8%), and only one died (5.5%). Emergency surgical treatment may be safely carried out in patients with hematological diseases presenting with an acute abdomen. Intensive postsurgical care is mandatory for the recovery of patients and the patient's critical condition should not be a deterrent to surgical intervention.

The Streptococcus milleri group is associated with a spectrum of serious suppurative infections that have not been well defined. The purposes of this study were to ascertain the clinical significance of Streptococcus milleri bacteremia and to determine the epidemiological, clinical, and microbiological features of these infections compared to those caused by other viridans streptococci. All cases of streptococcal bacteremia observed in a Spanish hospital in the period from January 1988 to December 1994 were reviewed. Of 137 cases of Streptococcus milleri infection, 33 (24%) were documented cases of bacteremia. Twenty-four patients were men (mean age 57.8 +/- 17.4 years). The majority of infections were abdominal in origin (20/33), the most frequent diagnoses being cholangitis/cholecystitis (18%) and appendicitis (12%). The origin of infection could not be established in three cases. Nine cases of bacteremia (27%) were polymicrobial. Six patients (18%) had septic shock; in four the infection was polymicrobial, and in two the infection was of abdominal origin. Eighteen of the 33 patients (54%) required surgery. Five patients died. All 33 Streptococcus milleri isolates were susceptible to penicillin. Twenty-two cases of bacteremia caused by other viridans streptococci were observed during the same period. There were no statistically significant differences between the two groups in terms of age, sex, mortality, rate of polymicrobial infection, rate of nosocomial acquisition of bacteremia, or the occurrence of shock. An abdominal origin of infection was more frequent in Streptococcus milleri bacteremia (p = 0.0001); a cardiovascular origin was more frequent in the viridans group (p = 0.01), as was a diagnosis of endocarditis (p = 0.004). Four patients with viridans streptococci bacteremia required surgery versus 18 patients with Streptococcus milleri bacteremia (p = 0.01). Viridans streptococci were notably less susceptible to penicillin (89%), clindamycin (79%), and erythromycin (79%).

The goal of this work was to study the microbiology and clinical characteristics of patients with infections associated with malignancy treated over a period of 17 years. A total of 668 specimens were obtained from 605 patients. The malignancies include 224 hematological malignancies and 381 nonhematogenic malignancies. Anaerobic bacteria only were isolated in 201 (30%) specimens, aerobic bacteria in 226 (34%), mixed aerobic-anaerobic bacteria in 231 (35%) and Candida spp. in 10 (1%). A total of 683 anaerobic (1.0 isolates per specimens) and 592 aerobic or facultative (0.9 per specimen) organisms were recovered. The predominant anaerobic bacteria included Bacteroides fragilis group isolates (181), Peptostreptococcus spp. (166), Prevotella spp. (106), Clostridium spp. (70), and Fusobacterium spp. (43). The predominant aerobic bacteria included Escherichia coli (133), Staphylococcus aureus (100), Klebsiella pneumoniae (48), and Pseudomonas aeroginosa (45). The type of infections included abscesses (221), bacteremia (198), wounds (175), including 61 cellulitis, 24 post-surgical wounds and 23 decubitus ulcers), peritonitis (48), empyema (12), cholecystitis (6) and thrombophlebitis (5). S. aureus and Peptostreptococcus spp. were isolated from all sites. However, organisms of the oropharyngeal flora (Prevotella and Fusobacterium spp.) predominated in local infections and bacteremia that originated from this site (head and neck wounds and abscesses and pulmonary infections), and organisms of the gastrointestinal tract flora predominated in infections that originated from this site (peritonitis, abdominal abscesses and decubitus ulcers). This retrospective study demonstrates polymicrobial features of many infections associated with malignancies.

Achromobacter xylosoxidans is a gram-negative bacterium whose natural habitat has not been clearly defined. It has been isolated from ear discharge and the large intestine of humans and from various hospital or environmental water sources. Infection with A. xylosoxidans in humans has been documented, and resulting illnesses include meningitis, pneumonia, cholecystitis, peritonitis and urinary tract infection. Bacteremia due to A. xylosoxidans is rare, and little information on treatment is available. Two cases of bacteremia due to A. xylosoxidans in patients with hemapoietic malignancies are reported herein. Case 1 involved a 70-yr. male whose clinical diagnosis was IgA lambda-type plasmacytoma. Case 2 involved 72-yr. male whose clinical diagnosis was acute lymphatic leukemia (L2). Both patients had been catheterized. Neutropenia was noted and the white blood cell counts were 20/microliter in case 1 and 35/microliter in case 2 when A. xylosoxidans was isolated from the blood culture. We suggest that bacteremia due to A. xylosoxidans may have been related to the presence of the catheter and neutropenia.

Over a 7-year period, we isolated 294 Actinomyces-like organisms (ALOs) which were not clearly identifiable. Using well-defined probes coding for sequences specific for recently described Actinomyces species (A. turicensis, A. radingae, and A. europaeus), we were able to identify 128 strains. The majority belonged to the A. turicensis species. A. radingae was found only in patients with skin-related pathologies. A. europaeus was also detected in patients with urinary tract infections. The main sources of A. turicensis were genital infections, followed by skin-related and urinary tract infections. Additional clinical pictures were appendicitis, cholecystitis, ear, nose, and throat infections, and bacteremia. In a small number of patients these ALOs were found as the only pathogen. Strains of the three species were tested by two widely used biochemical identification methods. A. turicensis was easily identifiable by both these methods. We conclude that these ALOs are not infrequent pathogens and are found in a wide range of human infections. At least A. turicensis is easily identifiable by clinical diagnostic laboratories.
