This review article focused on the current status of
mercury
(Hg) contamination in different ecological compartments in China, and their possible environmental and health impacts, focusing on some major cities.
Mercury
emission from non-ferrous metals smelting (especially zinc smelting), coal combustion and miscellaneous activities (of which battery and fluorescent lamp production and cement production are the largest), contributed about 45%, 38% and 17%, respectively, to the total Hg emission based on the data of 1999.
Mercury
contamination is widespread in different ecological compartments such as atmosphere, soil and water. There is evidence showing bioaccumulation and biomagnification of Hg in aquatic food chains, with higher concentrations detected in carnivorous fish. In terms of human exposure to Hg, fish consumption is the major exposure pathway for residents living in coastal cities such as Hong Kong, but inhalation may be another major source, affecting human health in areas with severe atmospheric Hg, such as Guiyang City (Guizhou Province). The first case study indicated that after closure of the acetic acid plant 20 years at Songyuan City (Jilin Province), 16.7% of residents' hair still contained Hg concentration in excess of 1 mg/kg (the reference dosage value, RfD set by USEPA). The second case study indicated that the male residents of Hong Kong who consumed more than four or more meals of fish per week tended to contain higher Hg in their hair, which was linked to their subfertility. There is also increasing evidence showing that skin disorders and
autism
in Hong Kong children are related to their high Hg body loadings (hair, blood and urine), through prenatal methyl Hg exposure. There seems to be an urgent need to identify the sources of Hg, speciation and concentrations in different ecological compartments, which may lead to high body loadings in human beings. Adverse health effects of residents living in places with a higher background level of Hg, due to long-term exposure to chronic levels of Hg through oral intake should not be overlooked.

In the case of SARS, the analysis explores the age old practice of isolation and quarantine, an important preventive measure with clear civil liberty implications. In reference to childhood immunization the paper considers the American controversy involving the refusal of parents to have their children vaccinated, fearing that

. The analysis explores the US Supreme Court case of Jacobson v. Massachusetts which 100 years ago established a four part test for evaluating claims of individuals that government public health authorities were infringing on their liberty interests. The paper endorses the four elements of necessity, reasonableness, proportionality, and harm avoidance identified in Jacobson, but calls for a more rigorous scientific evaluation to accompany this traditional test.

Autism
was recently associated with a urinary porphyrin pattern indicative of
mercury
toxicity in a large cohort of French children. The IRB of the Institute for Chronic Illnesses approved the present study. A total of 37 consecutive American patients (> or = 7 years-old) with
autism
spectrum disorders (ASDs) (Diagnostic and Statistical Manual of Mental Disorders, Fourth Edition-DSM IV), born from 1983-1998, that presented to the Genetic Centers of America for outpatient genetic evaluations were prospectively examined for urinary prophryin levels (LabCorp, Inc.) from June 2005-June 2006. Imaging and laboratory testing were conducted on each patient to rule-out other causal factors for their ASDs. As controls, age-, sex-, and race-matched neurotypical ASD siblings were examined. An apparent dose-response effect was observed between
autism
severity and increased urinary coproporphyrins. Patients with non-chelated
autism
(2.25-fold, 83% had levels > 2 SD above the control mean) and non-chelated ASDs (2-fold, 58% had levels > 2 SD above the control mean), but not patients with non-chelated pervasive developmental delay-not otherwise specified (PDD-NOS) or Asperger's disorder (1.4-fold, 46% had levels > 2 SD above the control mean), had significantly increased median coproporphyrin levels versus controls. A significant increase (1.7-fold) in median coproporphyrin levels was observed among non-chelated ASD patients versus chelated ASD patients. Porphyrins should be routinely clinically measured in ASDs, and potential ASD treatments should consider monitoring porphyrin levels. Additional research should be conducted to evaluate the potential role for
mercury
exposure in some ASDs.

Metallothionein (MT) proteins are widespread in bacteria, fungi, plants, and eukaryotic species. They are of low molecular weight (6-7 kDa) and of the 60+ amino acid residues, 20 are cysteines. Functions attributed to MTs include the sequestration and dispersal of metal ions, primarily in zinc and copper homeostasis; regulation of the biosynthesis and activity of zinc metalloproteins, most notably zinc-dependent transcription factors; and cellular cytoprotection from reactive oxygen species, ionizing radiation, electrophilic anticancer drugs and mutagens, and metals. Observations on the abundance of MTs within the central nervous system (CNS) and the identification of a brain-specific isoform, MT-III, suggest that it might have important neurophysiological and neuromodulatory functions. Reinforced by the potential involvement of MT-III in a number of neurodegenerative disorders, the role of MTs in the CNS has become an intense focus of scientific pursuit. The safety of
mercury
-containing dental amalgam has been hotly debated for well over a century. Dental exposures from
mercury
have been suggested as the cause of numerous diseases including multiple sclerosis,
autism
and many others. Known health effects of
mercury
exposure include CNS and renal damage. However, these effects have only been shown at occupational or higher levels of exposure, and have not been conclusively shown to be present at levels of
mercury
exposure consistent with that from dental amalgam fillings. The use of
mercury
amalgam fillings remains a state-of-the-art treatment for dental caries throughout the world. Although there have been a small number of peer-reviewed reports examining the health effects of dental
mercury
in children, only very recently have the only randomised, controlled clinical trials (two) of the safety of
mercury
amalgam been published. The purpose of this review is to discuss the scientific evidence on the safety of the use of
mercury
-containing dental amalgam as a treatment for dental caries.

This study examined the difference between sulfhydryl-reactive metals (
mercury
, lead, arsenic, and cadmium) in the hair of 45 children with
autism
(1-6 yr of age) as compared to 45 gender-, age-, and race-matched typical children. Hair samples were measured with inductively coupled mass spectrometry. Some studies, such as Holmes et al. (2003), suggested that children with
autism
may be poor detoxifiers relative to normally developing children. Metals that are not eliminated sequester in the brain. Our study found that arsenic, cadmium, and lead were significantly lower in the hair of children with
autism
than in matched controls.

was in the same direction (lower in

has a long and interesting history deriving from its use in medicine and industry, with the resultant toxicity produced. In high enough doses, all forms of
mercury
can produce toxicity. The most devastating tragedies related to
mercury
toxicity in recent history include Minamata Bay and Niagata, Japan in the 1950s, and Iraq in the 1970s. More recent
mercury
toxicity issues include the extreme toxicity of the dimethylmercury compound noted in 1998, the possible toxicity related to dental amalgams, and the disproved relationship between vaccines and
autism
related to the presence of the
mercury
-containing preservative, thimerosal.

Impairments in social relatedness and communication, repetitive behaviors, and stereotypic abnormal movement patterns characterize
autism
spectrum disorders (ASDs). It is clear that while genetic factors are important to the pathogenesis of ASDs,
mercury
exposure can induce immune, sensory, neurological, motor, and behavioral dysfunctions similar to traits defining or associated with ASDs. The Institutional Review Board of the Institute for Chronic Illnesses (Office for Human Research Protections, U.S. Department of Health and Human Services, IRB number IRB00005375) approved the present study. A case series of nine patients who presented to the Genetic Centers of America for a genetic/developmental evaluation are discussed. Eight of nine patients (one patient was found to have an ASD due to Rett's syndrome) (a) had regressive ASDs; (b) had elevated levels of androgens; (c) excreted significant amounts of
mercury
post chelation challenge; (d) had biochemical evidence of decreased function in their glutathione pathways; (e) had no known significant
mercury
exposure except from Thimerosal-containing vaccines/Rho(D)-immune globulin preparations; and (f) had alternate causes for their regressive ASDs ruled out. There was a significant dose-response relationship between the severity of the regressive ASDs observed and the total
mercury
dose children received from Thimerosal-containing vaccines/Rho (D)-immune globulin preparations. Based upon differential diagnoses, 8 of 9 patients examined were exposed to significant
mercury
from Thimerosal-containing biologic/vaccine preparations during their fetal/infant developmental periods, and subsequently, between 12 and 24 mo of age, these previously normally developing children suffered
mercury
toxic encephalopathies that manifested with clinical symptoms consistent with regressive ASDs. Evidence for

intoxication should be considered in the differential diagnosis as contributing to some regressive ASDs.

This study determined the level of
mercury
, lead, and zinc in baby teeth of children with
autism
spectrum disorder (n = 15, age 6.1 +/- 2.2 yr) and typically developing children (n = 11, age = 7 +/- 1.7 yr). Children with
autism
had significantly (2.1-fold) higher levels of
mercury
but similar levels of lead and similar levels of zinc. Children with
autism
also had significantly higher usage of oral antibiotics during their first 12 mo of life, and possibly higher usage of oral antibiotics during their first 36 mo of life. Baby teeth are a good measure of cumulative exposure to toxic metals during fetal development and early infancy, so this study suggests that children with
autism
had a higher body burden of
mercury
during fetal/infant development. Antibiotic use is known to almost completely inhibit excretion of
mercury
in rats due to alteration of gut flora. Thus, higher use of oral antibiotics in the children with
autism
may have reduced their ability to excrete
mercury
, and hence may partially explain the higher level in baby teeth. Higher usage of oral antibiotics in infancy may also partially explain the high incidence of chronic gastrointestinal problems in individuals with
autism

Though causes of

are considered largely genetic, considerable concern remains that exposure to Rh immune globulin (RhIg), which until 2001 in the United States contained the preservative thimerosal, can cause
autism
. To determine whether mothers of children with
autism
are more likely to be Rh negative (Rh(-)) or to have received RhIg preserved with thimerosal, which is 49.6% ethyl
mercury
, we surveyed families of children with an
autism
spectrum disorder (ASD) ascertained through a University-based
autism
clinic considered free of ascertainment biases related to type of
autism
or severity. Between 2004 and 2006, 305 mothers of 321 children with an ASD agreed to participate in a telephone interview. Analysis of complete records including the blood group status and RhIg exposure of 214 families showed that Rh(-) status is no higher in mothers of children with
autism
than in the general population, exposure to antepartum RhIg, preserved with thimerosal is no higher for children with
autism
and pregnancies are no more likely to be Rh incompatible. This was also true for
autism
subgroups defined by behavioral phenotype, gender, IQ, regressive onset, head circumference, dysmorphology, birth status, essential, or complex phenotype. These findings support the consensus that exposure to ethylmercury in thimerosal is not the cause of the increased prevalence of
autism
. These data are important not only for parents in this country but also for the international health community where thimerosal continues to be used to preserve multi-dose vials which in turn makes vaccines affordable.
