A study was conducted to assess the effects of redesigning the physical environment (i.e., the classroom) on the occurrence of self-stimulation, on-task behavior, inappropriate behavior, and inactivity. Three developmentally disabled males, diagnosed as autistic-type, participated. Data were collected using a withdrawal design. Results showed a decrease of self-stimulation and inactivity and an increase of on-task behavior. Inappropriate behavior remained unchanged across experimental conditions.

Follow-up or natural history outcome data for various DSM-III child and adolescent psychiatric diagnoses are presented. The data are relevant not only to our understanding of the specific disorders but also to the validity of the DSM-III diagnostic categories. "Semi-blind" psychiatric evaluations of 151 children were made as they presented to a community speech/language clinic and again approximately 4 years later. The follow-up data revealed high stability for only three diagnoses:
infantile autism
, attention deficit disorder with hyperactivity, and oppositional disorder. The data revealed that several of the DSM-III subcategories lacked predictive validity. This was true for the distinctions between attention deficit disorder with versus without hyperactivity; and between avoidant, separation anxiety, and overanxious disorders. Surprisingly low stability was found for conduct disorder diagnoses as were surprisingly poor prognoses for parent-child problems and adjustment disorders.

Ten cases of disintegrative disorder were identified within a larger sample of 165 individuals who met behavioral criteria for
autism
. These cases were compared to autistic individuals whose disorder had been recognized by age 2 and to those whose disorder had been recognized after age 2. Consistent with previous reports, children with disintegrative disorder had a period of normal development preceding the onset of a profound developmental regression from which they made, at best, only a limited recovery. Both clinical features at the time of regression and various outcome measures support the validity of the diagnostic concept, particularly when such cases are compared to "late onset" autistic ones.

Self-injury is a severe behavioral problem commonly observed in institutionalized mentally retarded individuals. While several forms of therapy are available, pharmacological treatment is most often used in institutions to control this behavior. This paper evaluated the clinical and experimental literature on the effects of pharmacotherapy for self-injury. Few general conclusions could be drawn mainly due to the small number of studies and the general lack of methodological rigor of these studies. However, there was some indication that antipsychotics and antimanics may prove to be useful in the treatment of self-injury and warrant further investigation. Several areas of future research were discussed.

Various theories have been provided in the literature regarding the etiology of
infantile autism
. It seems that the biological causation dominates the thinking of mental health practitioners and researchers irrespective of discipline. Areas of research include the following: neuropathological studies of the brain, autopsies, electroencephalograms, epileptic seizures, brain lateralization, studies in asymmetry; neurochemistry; genetics; and pre-, peri- and postnatal factors. A critical analysis of these studies has indicated that the evidence of organic factors tends to be rather weak and furthermore has been found to be contradictory. It is felt that the major problem lies in the fact that a large number of investigators include in their sampling children with various mental and physical disabilities and label them autistic. In general, there seems to be little information regarding the selection and little background information is offered on these children. It is suggested that a standardized and world wide diagnostic system be constructed which will provide objective etiological results.

In a survey study of psychiatric morbidity, based on a representative sample from the Danish mental retardation register, 44 adults with Down's syndrome (DS) were compared with 258 other mentally retarded adults. Assessed by the parameters psychiatric disorder, behaviour problems, neurotic traits, and deviant social interaction, the DS group functioned better on all parameters. However, male and female DS patients were very different, the females constituting a superior well-functioning group while the males had major problems in every area. High prevalence rates of dementia and
infantile autism
were found in the DS group. General function rapidly decreased with age in DS patients.
