Using the method of double-blind trial the effectiveness of treatment with Anginin (pyrridinolcarbamate) and Stugeron (cinnarizine) was evaluated. The somatic and mental condition and the results of motor rehabilitation of the patients were estimated. In all the evaluated elements the results of Stugeron treatment were better. The results were assessed in two groups of 20 cases each of patients with cerebral atherosclerosis observed during two months.

The author describes the results of treatment of 299 patients with clinical evidence of cerebral atherosclerosis receiving three groups of antiatherosclerotic agents: improving cerebral blood flow (naftidrofuryl, cinnarazine, pyridinol carbamate, low-molecular dextran), normalizing lipid metabolism (clofibrate, heparinoid, alpha-tocopherol, Procetophen) and improving the metabolism of nerve cells (piracetam, pyritinol). After a statistical analysis of the results of the author evaluated the effectiveness of these drugs in various groups and suggested practical conclusions and treatment schedules in various forms of cerebral atherosclerosis.

PMID:[Comparative evaluation of the therapeutic effectiveness of drugs of different mechanisms of action in cerebral atherosclerosis]. 714 12

The authors describe 5 male patients aged 38 to 69 years with cardiac diseases, arterial hypertension and atherosclerosis in whom cortical blindness developed suddenly associated with other symptoms such as asynergia (4 cases), delirium (3 cases) and pyramidal syndrome (in 2 cases). In 3 cases the outcome was lethal due to brainstem damage. In all cases lesions of both occipital lobes were demonstrated by means of computerized tomography (4 cases) or autopsy (3 cases). In 3 cases the vertebral artery was occluded and in 1 case it was stenosed. Presence of additional symptoms makes possible intravital diagnosis of vertebrobasilar insufficiency and arteriography is not indicated in such cases. The treatment is conservative.

In order to explain whether the arterial smooth muscle exhibits altered contractile properties in atherosclerosis, normal and atherosclerotic rabbit aortic strips were exposed to noradrenaline (NA) and potassium chloride (KCl). The experiments were repeated with normal blood and the containing an excess of free fatty acids (FFA) added to the bath. Contraction induced by NA was markedly enhanced in the atherosclerotic trips. In contrast the response of atherosclerotic aorta strips to KCl was reduced. Excess FFA blood added to the bath with NA normalized the previously enhanced contractile response in atherosclerotic aortic strips. An opposite change in KCl induced contraction was observed under the some conditions namely increased response occurred. We conclude that the enhanced reaction of atherosclerotic aortic strips to NA depending on a specific mechanism might be reversed by elevation of FFA in blood.

Metabolic efficiency of the liver of 16 dogs, mixed breed, sexually mature, of both sexes was studied in a lasting experiment. The animals were kept in cages without a run, divided into 3 groups - k, A6 and A12. They were fed an atherogenic diet for 6 and 12 months. Beside cholesterol pork fat and cholic acid, the diet contained metizol. Hepatic lipids were analysed by the method of thin-layer chromatography and densitometric measurements. The numerical results statistically verified have been given in mg/g of wet liver tissue taken from the right lobe, homogenized and extracted after the method of Folch et al. Pathomorphological changes in the abdominal aorta were evaluated macro- and microscopically. The magnitude of aortosclerotic changes was compared with mean values of hepatic lipids. The numerical values and the image of chromatographic distribution are shown in Table 1 and Figs 1, 2 and 3. After 6 month feeding, in group A6 the authors observed a considerable increase of hepatic lipids, preserved capability of cholesterol esterification and the occurrence of early atherosclerotic changes in the aorta. After 12 months, the level of lipids such as phospholipids, free fatty acids, triglycerides and free cholesterol increased. However, a turning point in the capability of cholesterol esterification was observed, which was expressed by a decrease of cholesterol esters. In the abdominal aorta developed atherosclerosis marked with 2 and 3 pluses was found. It is concluded that beside metizol and overloading highcaloric diet, lack of movement contributed to metabolic disturbances expressed by hepatopathy, hyperlipemia and aortosclerosis, which has a great influence in dogs on preservation of homeostasis of the organism.

An attempt was undertaken to find out to what extent a partial ileal bypass in healthy dogs fed for 6 and 12 months on overloaded atherogenic diet with an addition of metizol influences the level of lipids and the development of atherosclerosis of the aorta. For the investigations 16 mongrels, both sexes, from 2 to 5 years old were used. an unoperated, control group consisted of 6 dogs, which were fed on the atherogenic diet for 12 months. The dogs underwent a partial ileal bypass operation after Buchwald and Varco's method. When the dogs had recovered, 5 of them were on the atherogenic diet for 6 months, and 5 others for 12 months. After this period the animals were put to sleep. The isolated aorta was examined macro and microscopically. A segment of the right lobus of the liver was homogenized and from lg of a wet mass the lipids were extracted after the method of Folch et al. The level of lipids was estimated by thin-layer chromatography. The spots of the chromatograms were densitometrically analyzed and the results were statistically evaluated. In the control group the level of hepatic lipids in most components increased considerably. In the operated dogs and those fed for a long time, the level of these compounds decreased distinctly, especially in comparison with the control group, and was statistically highly significant in 1,3 - DG, TG and CE. The occurrence of statistically significant differences among the animals fed on the atherogenic diet for 6 and 12 months was not noticed. In dogs of both these groups other arteriosclerotic changes in the aorta were not demonstrated. During the experiment the animals felt well and their body weight did not change. No negative influence of the operation connected with the overloaded feeding of the level of hepatic lipids was observed. In both groups the above-mentioned level was near its norm.

The author carried out ultrasonography of the common carotid arteries by the method based on Doppler's phenomenon in 100 subjects aged 16--86 years. The maximal, minimal and mean flow velocity, vascular resistance, and symmetry and shape of the recorded curve were evaluated. Early and progressing with age changes were found in the curve indicating decreasing vascular wall elasticity. The maximal flow velocity decreased also rapidly. On the other hand, the mean and the minimal flow velocity and the index of vascular resistance remained without substantial changes up to the age of 70 years. In later age the minimal and mean flow velocity decreased rapidly and the index of vascular resistance increased greatly. This suggested accelerated rate of atherosclerosis development and/or exhaustion of compensatory mechanisms. In 3 cases of considerable asymmetry of the curve was observed caused by asymptomatic stenosis of one artery. Doppler ultrasonography could be useful at the time of intensification and evolution of atherosclerotic changes in the carotid arteries, and their effect on the condition of cerebral circulation.

On the basis of a study of 218 control subjects in 10-year age groups the authors determined the mean levels and critical values of certain serum lipid components (cholesterol, triglycerides, beta-lipoproteins, esterified fatty acids and total lipids). Forty-two subjects were isolated (11.2%) with hyperlipoproteinaemia. Other atherosclerosis risk factors were determined in them as well. Hypertension was found in 19%, diabetes in 7%, obesity in 14%, smoking in 43%. The authors discuss the criteria qualifying the patients to the control group in the studies on lipid metabolism.

Experimental atherosclerosis was produced in domestic pigs by administration for 24 weeks a high lipid diet containing cholesterol and coconut oil. ECG changes were not uniform: negative T, prolonged intraventricular conduction, and premature ventricular beats were observed. Macroscopic and microscopic anatomo-pathological studies revealed atheromatous changes in the aorta.

Rheoencephalographic (REG) investigations were carried out in a group of 44 women and 58 men aged from 18 to 67 years. It was found that the amplitude of REG waves on the group of women was significantly smaller than in men. A characteristic tendency was observed with progressing age of the subjects for a decrease of the amplitude of REG waves with prolongation of the duration of the ascending arm of the wave and with an increase in the value of the dicrotic index, as well as a decrease in the value of the angle of inclination of this ascending arm to the base. Atherosclerosis of the cerebral vessels was a factor contributing significantly to progression of these changes. On the basis of visual analysis three main types of REG waves were isolated which corresponded to successive phases of elasticity loss of brain vessel walls. The authors suggest accepting the value of 10--90 centile of the determined values of REG wave parameters as the range of normal values for different age groups and the type of REG apparatus used.
