Blood serotonin levels have been measured by spectrofluorometry in 145 patients with various manifestations of acute disorders of the cerebral circulation (ADCC): with ischemic stroke, cerebral hypertensive crises, transient impairments of the cerebral circulation, and with circulatory encephalopathy (DE) in the presence of arterial hypertension combined with atherosclerosis. In ADCC patients the blood serotonin has been measured 7 times, in DE ones 3 times. This level has been found changed in the patients with ischemic stroke and with cerebral hypertensive crises for a long time after the cerebral circulation impairment. Serotonin levels correlated with the condition severity and course in ADCC and DE. The vestibulocerebellar syndrome in DE has been treated more effectively, particularly with trental. The blood serotonin level may be regarded as a prognostic criterion of the disease outcome and of the treatment efficacy.

Two cases of subcortical arteriosclerotic encephalopathy (Binswanger's disease) are reported. The two patients lacked a clinical history of hypertension, relevant pathogenetic factor in the development of the small and medium size cerebral arteries atherosclerosis, which is the main pathologic finding of the disease. The two subjects clinically showed a marked intellectual deterioration, together with mood depression and focal neurological signs, that were an expression of the multifocal neurologic involvement. In both cases CT scans evidentiated a mainly periventricular leucoencephalopathy associated, in the first patient, with small multiple ischemic lesions and, in the second, with a unique hypodense area in the centrum semiovale. A review of the literature on the subjects is proposed, together with an attempt of pathogenetic interpretation of our two cases.

The inclusion of hemosorption through activated coals into the comprehensive treatment of 580 patients sick with atherosclerosis has made it possible to achieve a positive clinical effect among 81 per cent of the patients sick with the ischemic heart disease, 79 per cent of atherosclerotic discirculatory encephalopathy and 76 per cent of obliterating atherosclerosis of lower limbs. The positive dynamics is especially visible among patients who are administered the programmatic hemosorption, making it possible over a long period of time (3-5 years) to sustain remission in the clinical state and the normal level of the lipids in the blood. The direct effect is expressed in the improved microcirculation, disappearance of the sludge-syndrome and the reduction of ischemic disorders. The remote effect of hemosorption is connected with the repeated removals of cholesterin of the atherogenic lipoproteids, which leads, we believe, to the interruption or even regression of the atherosclerotic process.

Using formalized documentation and computerized techniques, the authors analyzed clinical, electrophysiological, and biochemical semiotics in 2180 individuals--healthy subjects, patients with subclinical and initial manifestations of cerebral circulation insufficiency, initial dyscirculatory encephalopathy developing in the presence of neurocirculatory dystonias, atherosclerosis, and arterial hypertension. The author has established characteristics of the semiotics which depend on the nature of the underlying vascular disease, the stage of the process and the patient's age. Tabulated methods and a computerized system of the diagnosis of cerebrovascular disease have been developed. They are based on the employment of a bank of clinical and physiological information used for prophylactic examinations and population screening. The social, medical and economic effectiveness of the proposed methods of the diagnosis and prevention of cerebrovascular diseases allows their recommendation for wide introduction into health care practice.

Parameters of the cerebral and systemic circulation were studied with the help of rheoencephalography in 119 patients with initial circulatory encephalopathy secondary to atherosclerosis (50 patients aged 45 to 59 years; 69, 60 to 74 years). Fifty-six healthy subjects served as a control group. The data obtained confirm the development of combined cardio-cerebrovascular insufficiency already at the initial stage of the disease and indicate the necessity of a comprehensive approach to the assessment of the function of two circulatory systems at an early stage of cerebrovascular pathology.

An 80-year-old hypertensive woman developed right hemiplegia and died 24 hours after admission. Neuropathologic examination revealed multiple cerebral infarcts of various ages and diffuse subcortical arteriosclerotic encephalopathy. Clusters of asymptomatic "expanding" lacunes, due to dilatation of the perivascular spaces, were found in both dentate nuclei. These cavities, which presented as space-occupying lesions, were surrounded by a single layer of flattened cells and contained 1 or more sections of normal-looking arterioles. Such a topographic grouping of lacunes in the dentate nucleus has not been described previously. The mechanism of widening of the perivascular compartment remains unclear; its occurrence in a hypertensive patient and its association with typical Binswanger's subcortical arteriosclerotic encephalopathy and severe atherosclerosis with multiple infarcts suggest a common pathophysiologic mechanism possibly including an alteration of the blood-brain barrier.

The authors investigated the diagnosis and treatment of the initial forms of venous encephalopathy in atherosclerosis in 42 patients, using synchronous rheoencephalography and jugular rheophlebography under conditions of graded exercise on a bicycle ergometer. The study revealed signs of venous encephalopathy with dystonic, asthenovegetative, psychopathological, pseudotumorous and other syndromes which were confirmed by changes in the parameters of the REG, rheophlebogram, etc. Graded exercise in a compensated phase of the disease led to a reduction in signs of venous congestion in the brain, while in a subcompensated phase these signs increased. The effect of a multiple modality treatment with due regard to the phase of compensation and the leading syndrome was good or satisfactory in the majority of patients.

Whether long-term glycemic control will prevent the chronic vascular complications of diabetes mellitus remains unknown. Microangiopathy and accelerated macroangiopathy are prevalent in both type I, or insulin-dependent diabetes mellitus, and type II, or non-insulin-dependent diabetes mellitus. Microangiopathy is predominantly responsible for the excessive morbidity and mortality in type I diabetic patients, whereas accelerated macroangiopathy directly relates to the excessive morbidity and mortality in type II diabetic patients. Institution of euglycemia for short periods will reverse preclinical, functional, renal, and retinal abnormalities, but will not reverse clinical nephropathy and retinopathy. Intensive insulin therapy, although it increases the risk of hypoglycemic encephalopathy, seems rational for type I diabetic patients without vascular complications who can recognize and respond normally to hypoglycemia. In patients with type II diabetes, sulfonylurea therapy, which is associated with fewer adverse reactions than intensive insulin therapy, may lower the risk of atherosclerosis development by correcting hyperglycemia and associated lipid abnormalities.

On the basis of radiocardiographic and radiocirculatory examination of the central and cerebral hemodynamics in 130 patients with dyscirculatory encephalopathy (DE) caused by essential hypertension and atherosclerosis, the authors have demonstrated that the development of DE is facilitated by disturbances of the central and peripheral hemodynamics. In turn, DE reduces the function of the autoregulatory mechanisms of the cerebral blood flow and leads to insufficiency of compensatory adaptive possibilities of the cardiovascular system. It is recommended that not only cerebral disorders but also the type (hyperkinetic, hypokinetic, eukinetic) of the blood circulation in these patients be taken into consideration in selecting the optimal method of treatment.
