We report a case of cholesterol crystal embolization associated with secretory diarrhea,
megacolon
and acalculous cholecystitis. Cholesterol emboli were found within the submucosal arterioles of the small and large bowel as well as in the gallbladder wall. Cholesterol crystal embolization is an often unrecognized disease, occurring in elderly patients with severe
atherosclerosis
. Cholesterol emboli may induce misleading gastrointestinal manifestations with both hemorrhagic and ischemic lesions. Though an uncommon symptom in this setting, secretory diarrhea did not seem to be fortuitous and could have been also a consequence of cholesterol crystal embolization.

A total of 145 BB Wistar diabetic rats, 46 of their nondiabetic siblings, and 43 outbred Wistar rats were autopsied and the frequency of lesions in all organ systems were determined. Common strain-related lesions included pulmonary infections, granulomas, lymphoid hyperplasia, lymphomas, lymphocytopenia, eosinophilia, supradiaphragmatic accessory lobes of the liver, and prostatic atrophy. These suggest some basic strain-related abnormalities of the immune system that were selected by the process of inbreeding. Diabetes-related lesions were insulitis, testicular atrophy, cataracts, hepatic fatty change, pancreatitis, lymphocytic thyroiditis, hypoglycemic brain damage, central pontine myelinolysis, stomach erosions, and idiopathic
megacolon
. Many of these are sequelae of human juvenile-onset diabetes and support the validity of the BB Wistar rat as an animal model for human diabetes mellitus. The absence of several important sequelae of the human disease (i.e., diabetic nephropathy,
atherosclerosis
, and severe microangiopathy) suggests a degree of infidelity as a model for human diabetes mellitus. Nonspecific lesions occurring in all three groups of rats included myocardial degeneration and fibrosis, splenic extramedullary hematopoiesis, and chronic progressive glomerulonephropathy.

Ischemic colitis occurs as a result of the decrease of arterial blood supply at the level of the colon and may have consequences which vary from completely reversible lesions to ischemic necrosis. It is a condition that affects both sexes, predominantly people over the age of 50, having multiple etiologies in which the main role is played by
atherosclerosis
. The symptomatology varies from mild symptoms, which are reversible, to severe ones, with a vital risk. The colonoscopy does not show pathognomonic signs, but it has high sensitivity due to the involvement of the mucosa. There are different treatments according to the clinical form. In general, it goes into remission spontaneously. Emergency surgical treatment is required in acute pancolitis, toxic
megacolon
, massive hemorrhages, when a subtotal colectomy is performed.
