Sulfasalazine (SSZ), 3 gm daily, was compared with placebo for treatment of rheumatoid arthritis, in a 15-week randomized, parallel, double-blind trial. Joint tenderness and swelling, morning stiffness, grip strength, and pain score all showed significantly more improvement with SSZ than with placebo. Adverse effects, particularly gastrointestinal reactions, led to withdrawal from the study of 28% of the patients who had been receiving SSZ, but these effects were all readily reversible and not life-threatening. These results confirm previous findings that suppression of rheumatoid synovitis may be induced by SSZ, within 2 months after full maintenance doses are reached.

A comparison was made of two series of consecutive outpatients with a presumptive diagnosis of rheumatoid arthritis (RA) attending referral centres in Melbourne and Shanghai. No significant differences were observed in disease onset, course, presence of antinuclear antibodies (ANA), or seropositivity. In the Australian series there was a higher frequency of nodules, Raynaud's phenomenon, carpal tunnel syndrome, and 'classical' in comparison with 'definite' disease, and a lower frequency of lymphadenopathy and hepatomegaly. Joint tenderness and soft tissue swelling tended to be more marked in the Chinese series, while deformity and limited range of movement were less severe. Drug therapy was similar overall but influenced by drug availability. Peptic ulceration was recorded in 28% of the Australian series but in only 6% of the Chinese; although 25% of the Chinese were receiving antacids and 6% antiulcerants. X-rays of hands and feet showed more severe disease in the Australian series. The older age group and longer duration of the disease in the Australian patients, who had more chronic and less active disease, may have influenced some of these results.

The present study addresses the relationship between early rheumatoid arthritis and an element of personality i.e. the self-esteem, as an intervening variable on psychological well-being. From a sample of 292, more or less recently diagnosed RA-patients, the relationships between disease parameters, self-esteem and psychological well-being were studied. Five subgroups of patients have been formed according to the duration of the disease: from four years disease duration up to incident cases. Disease parameters such as the duration of RA or the sedimentation rate is not associated with patients' self-esteem. The self-esteem of RA-patients is negatively related to the level of joint tenderness. Also, the lower the self-esteem of patients, the lower the level of psychological well-being. Joint tenderness with the patients' level of self-esteem explain 34% of the scoring on psychological well-being.

20 patients with rheumatoid arthritis and atlanto-axial dislocation subjected to occipito-cervical fusion were studied. The patients were evaluated by a rheumatologist before surgery and 6-12 months after the fusion procedure. Joint tenderness was assessed by Ritchie's Index while the functional capacity was evaluated using a health assessment questionnaire and according to the classification by Steinbrocker. Localization and character of the symptoms from the head-neck region were registered. The neck pain was measured on a visual analogue scale. Radiographs of hands and wrists were obtained before surgery and joint destruction was classified according to the Larsen Dale Index. Reduction of pain and neurological symptoms was observed in the majority. There was, however, little evidence of improved functional capacity.
