We present two cases of infection by Coxiella burnetii which developed with sustained fever symptoms. During its evolution, the first case presented granulomatous hepatitis, whereas the second case presented left Cosofemoral
Arthritis
. We describe the clinical-evolutive characteristics of these clinical forms and within the evolution of the chronic forms of
Q fever
.

418 workers were surveyed at a meat-packing plant. The presence of complement-fixing antibodies to the causative agent of enzootic abortion of sheep (EAS) was detected in 59 workers (14.27%), brucellosis in 32 workers (7.64%) and
Q fever
in 5 workers (1.18%). EAS antibodies were found to reach titers of 1 : 10 to 1 : 40 and higher, changing dynamically. The persons found to be seropositive belonged mainly to the workers of the sausage-making, slaughtering, intestinal, subproduct and skin-salting departments, as well as to the workers of the sanitary slaughter-house. Such diseases as acute respiratory infections, pneumonia, arthralgia,
arthritis
, and in women inflammatory urogenital infections, as well as spontaneous abortions were more frequent among the workers seropositive to the causative agent of EAS than in the control groups. Enzootic halprovial abortion of sheep was suggested to be potentially dangerous for certain groups of workers at the meat-packing plant, but the results of the survey indicate that further research in the fields of the epidemiology, clinical picture and laboratory diagnostics of infectious pathology in persons having contacts with the animals infected with the causative agent of EAS is necessary.

On five newly established large goat farms the incidence of antibodies to some chronic and latent infections (
arthritis
and encephalitis,
Q-fever
, caseous lymphadenitis and toxoplazmosis) was investigated in the first six months of the year 1994. Agar-gel immunodiffusion did not reveal any antibodies to
arthritis
and encephalitis of goats (CAE). Complement fixation test did not demonstrate any antibodies to
Q-fever
. Neither agar-gel immunodiffusion nor neutralization test confirmed any antibodies to caseous lymphadentitis. Complement fixation test (titer 1:8 and more) revealed antibodies to toxoplazmosis at 20.2% of the cases. No clinical symptoms of toxoplazmosis were observed on the investigated goat farms. It was recommended to take a preventive serological examination of goats against CAE,
Q-fever
and caseous lymphadenitis before they were housed on the given farms. By maintaining high zoohygienic parameters on the large goat farms it is possible to except a decrease in prevalence of antibodies to toxoplazmosis.

We describe two cases of chronic
Q fever
in hemodialysis patients (HD). In the first case, we discovered chronic
Q fever
when looking for the cause of an unexplained fever. In the second case,
Q fever
was diagnosed in a patient who complained of an unexplained shoulder
arthritis
. To our knowledge these are the first reported cases of chronic
Q fever
in HD.

Reactive arthritis is a well-defined clinical syndrome occurring after various infections, although most cases are usually associated with Chlamydiae and gastrointestinal pathogens. Its immunologic background has been extensively studied, as has its relationship with HLA-B27. We describe two cases of reactive
arthritis
arising after infections with two pathogens not so far related to the occurrence of reactive
arthritis
: one patient exhibited migratory oligoarthritis shortly after the course of acute
Q fever
, and another patient developed monoarthritis during recovery from leptospirosis.
Arthritis
was transient and did not exhibit a chronic course in either patient. We further discuss the context of pathophysiology of the
arthritis
in these patients, with an emphasis on the immunomodulatory properties of these two pathogens.

Osteoarticular infection is an uncommon presentation of
Q fever
. Positron emission tomography (PET) scanning is a valuable tool for the diagnosis of Coxiella burnetii graft prosthesis infection and endocarditis. Our objective was to test a series of culture-negative osteoarticular samples using molecular assays for Coxiella burnetii. We tested for C. burnetii by molecular assays targeting the IS1111 and the IS30A spacer regions, using culture-negative osteoarticular samples obtained in our laboratory between January 2011 and December 2012. We examine a total of 1,410 osteoarticular samples, and we observed two cases of
arthritis
and subacromial bursitis caused by C. burnetii. The infections were localized using PET scanning, and the diagnosis was confirmed through serology. For one, a C. burnetii strain with a multispacer sequence type 8 genotype was isolated from synovial fluid culture.
Q fever
articular infections could be undiagnosed because of the long evolution of articular attack, and patients with high antibody titers against C. burnetii should be tested using PET scanning to localize the site of infection.
