Infection
with N. gonorrhoeae stimulates the production of antibodies to many common, species-specific, and type-specific antigens. The L-antigen is an envelop antigen and antibodies to it could be demonstrated by various methods in more than 90% of the patients after the first 10 days of infection. Serologic tests are not yet available in the United States. If and when they become available, they may be recommended for: (1) Screening asymptomatic men and women, (2) Use as an adjunct diagnostic tool in cases of prostatitis,
arthritis
, disseminated gonococcal infection, and pelvic inflammatory disease, (3) Use (alone or in conjunction with culture) when specimens must be mailed to a central laboratory, when mailing conditions do not allow for incubation within 24-48 hr, or when proper media and qualified bacteriologists are not available.

Infections
of the intestinal tract with Yersinia enterocolitica and Yersinia pseudotuberculosis are known the world over. Post-infection reactions like
arthritis
, erythema nodosum, erythema multiforme, scarlatiniform exanthemata and septicemic types deserve particular clinical attention. The clinical importance of enteric yersinioses has been shown in studies with the identification of significant antibody titers against Yersinia enterocolitis and Yersinia pseudotuberculosis in 6.71 and 4.58% respectively of 4411 patients examined for the first time. It was possible to isolate Yersinia enterocolitica from 5.05% of 616 samples of stool and Yersinia pseudotubercolosis from 0.66% of 136 lymph nodes taken from 751 patients.

Following erysipeloid a 46-year-old man fell ill with septicaemia and endocarditis. Treatment with high doses of antibiotics could not prevent his death.
Infection
with Erysipelothrix rhusiopathiae in man occurs through a skin lesion and only rarely leads to spreading of the agent in the body presenting as
arthritis
, meningitis, or endocarditis. Erysipelothrix endocarditis is a severe disease leading to widespread destruction of the involved cardiac valves. The aortic valve is most commonly involved. 16 out of 28 patients with erysipelothrix endocarditis reported in the literature died, eleven of them despite treatment with antibiotics.

Infection
with Borrelia burgdorferi is suspected to be a cause of lameness and
arthritis
in cattle. Interleukin-1 (IL-1) activity has been detected in joint fluids from human patients affected by various arthritides, including Lyme arthritis. In addition, human monocytes and murine macrophages have been reported to release IL-1 activity when incubated with B. burgdorferi in vitro. To address a possible mechanism by which B. burgdorferi might cause a bovine arthritic syndrome, we determined whether bovine peripheral blood monocytes released IL-1 activity when coincubated with B. burgdorferi in vitro. High-passage and low-passage isolates of B. burgdorferi stimulated release of IL-1 activity from bovine monocytes. The amount of IL-1 activity released was dependent on the number of borreliae added to the monocyte cultures. In addition, live and heat-killed B. burgdorferi cells stimulated release of similar amounts of IL-1. We also obtained no evidence that soluble components released from in vitro-cultured B. burgdorferi stimulated IL-1 release from bovine monocytes. A recombinant IL-1 receptor antagonist blocked the proliferative activity of monocyte-conditioned medium in a thymocyte costimulation assay, thus demonstrating that the costimulatory activity detected was due to IL-1.

We describe a patient with relapsing polychondritis in whom aortic valve inflammation developed 3 years after diagnosis, when the polychondritis had been in apparent remission for an extended period of time.
Infection
and cardiac involvement can be significant complications of relapsing polychondritis. Recommendations for monitoring and treatment of patients with this disease are discussed.

The human parvovirus B19 agent causes infectious erythema (fifth disease). However, a wide range of other pathological manifestations may also be seen: atypical exanthema, ARD (also obstructive forms, e.g. bronchiolitis), acute gastroenteritis, chronic anemia or aplastic crises (in constitutional or malignant hematological diseases or immunological deficiency), arthralgia/
arthritis
(e.g. rheumatoid arthritis, jcA), diseases of the central nervous systems (e.g. febrile convulsions in young children), lymphadenopathies (e.g. lymphadenitis mesenterialis or pseudoappendicitis); prenatal infection can lead to fetal death (not malformations!).
Infection
occurring concomitantly with vaccination may suggest complications of the latter. To clarify the true etiological situation, modern laboratory investigations are then required. Vaccination against parvovirus B19 (initially indicated in the case of non-immune girls and women wanting children) is a desirable future development.

Although arthralgia is a common complaint in Mediterranean spotted fever, frank
arthritis
is very unusual. We report two cases of this condition during the course of Mediterranean spotted fever. Synovial fluid was inflammatory in both patients and the disease resolved after doxycycline therapy. Immunofluorescence test for Rickettsia conorii in synovial fluid was negative, suggesting a reactive
arthritis
.

We describe a case of a male teenager, affected by acne fulminans. He had lytic bone lesions in both clavicles and
arthritis
of the right sacroiliac joint and spine. A bone scan showed increased uptake in these areas.
Infection
or a neoplastic disease was suspected but the cytological examination and the bacterial cultures of the material collected with a fine needle biopsy from the clavicular lesion were negative. The patient was treated with diclofenac and promptly recovered. Our report emphasizes that, although unusual, acne fulminans can be complicated by an erosive systemic
arthritis
and bone lesions. An immunological mechanism seems to be involved in this association.

Live recombinant vaccinia viruses, expressing antigens from pathogenic microorganisms, are studied for their use as vaccines designed for the protection against infectious diseases.
Infections
with these vaccinia virus recombinants, expressing proteins or epitopes from viruses, parasites, or bacteria, have resulted in the development of specific neutralizing antibodies or cytotoxic T lymphocytes. Here, we describe the generation of a recombinant vaccinia virus expressing the mycobacterial 65-kDa heat shock protein (HSP65). A vaccinia recombinant virus was constructed by placing the gene for the Mycobacterium bovis BCG HSP65 under control of a vaccinia virus promoter and inserting this mycobacterial gene in the thymidine kinase locus of the vaccinia virus genome. Mycobacterial HSP65 is a critical antigen in the autoimmune model of adjuvant
arthritis
induced in Lewis rats by the immunization with Mycobacterium tuberculosis. We report the induction of immunity directed to this mycobacterial HSP65 by testing for the presence of specific antibodies and T-cell proliferation. Furthermore, induction of such immunity resulted in a reduction of
arthritis
severity when given to rats before or, even more interestingly, during development of
arthritis
. Disease reduction was not found after administration of HSP65 in the absence of vaccinia virus as a vector when given during
arthritis
development. Therefore, recombinant vaccinia virus may offer new prospectives for specific intervention in autoimmunity.

Disorders of immunoglobulin synthesis are of great interest to rheumatologists. At the diagnostic level, such disorders not uncommonly mimic commoner rheumatic diseases, so it is important that the diagnostic possibility of immunodeficiency be kept in mind.
Infections
often complicate immunodeficiency and may present in an atypical manner. From the theoretical standpoint, the interactions between infectious agents and patients with impaired immunity suggest ways in which the same or similar agents could be responsible for
arthritis
of unknown etiology. Furthermore, many immunodeficiency disorders predispose to autoimmunity; establishing the mechanism of this association may offer good insights into the factors that trigger autoimmune disorders in patients with seemingly normal immune competence.
