Twenty-two (11.3%) of 194 gouty patients were female. The age at onset was earlier than the 40th year in only 4.5 per cent of the females compared with 19.7 per cent of the males. However, in 54.5 per cent of the female patients compared with 38.4 per cent of the males, the age at onset was 60 years or more. Although the initial manifestation of the disease was acute monoarticular arthritis, with involvement of the ankle joint being more commonly observed in the female patients (63.3%) than in the males (37.2%), and by contrast, with involvement of the first metatarsophalangeal joint (podagra) being more frequently noted in the males (45.9%) than in the females (22.7%). In subsequent attacks, oligoarticular arthritis and arthritis of the ankle joints were predominant features of the disease in both sexes. However, podagra was less frequently evident in the female patients (31.8%) than in the males (68.5%) as were tophi (18.2 and 31.4%, respectively); specifically, in those females with tophi, the tophaceous manifestation developed in all of them within four years of the onset of the disease compared with only 27.8 per cent of the males with tophi. Moreover, although provocative factors and associated diseases were present in both sexes, haematologic malignancy was more common in the females (22.7%) than in the males (2.9%). In conclusion, although the clinical features of gout in females are similar to those in males, gout in females is characterized by later onset of the disease, earlier development of tophi and arthritis of the ankle joint.

A 43-year-old monk had generalized purpura, arthritis of both shoulders, erythrocyanosis of lip and oral mucous membrane, Raynaud's phenomenon and uveitis. Platelets were normal. Cryoglobulin and cryofibrinogen were positive. Biopsy revealed vasculitis. No underlying infection, collagen vascular disease, lymphoproliferative, myeloproliferative and malignancy were found. He was diagnosed as having essential cryoglobulinemia and cryofibrinogenemia.

Anticentromere antibodies (ACA) and anti-topoisomerase I (anti-topo I) were assayed in serum samples from 355 patients: 89 with proximal scleroderma; 54 with CREST syndrome (calcinosis, Raynaud's phenomenon, esophageal dysmotility, sclerodactyly, telangiectasias), without proximal scleroderma; 154 with primary and secondary Raynaud's disease; and 58 with other rheumatic diseases, without Raynaud's disease. Sera from healthy control subjects were also assayed. Using immunoblotting techniques, anti-topo I was detected in 28% of the patients with proximal scleroderma; using immunodiffusion techniques, this antibody was found in only 20% of the same group of patients. Anti-topo I and ACA were found primarily in patients with scleroderma, CREST syndrome, and Raynaud's phenomenon. ACA identified patients with less severe disease, whereas anti-topo I identified patients with skin and cardiac involvement and patients with malignancies.

Eicosapentaenoic acid (EPA), a long chain fatty acid of the n-3 series, is found in marine foods. Beneficial effects of these foods containing EPA on factors associated with cardiovascular disease risk and arterial thrombosis have been demonstrated. More recently, studies have suggested that EPA may also have a favourable effect on other human diseases such as arthritis, renal disorders, psoriasis and possibly also cancer. EPA is metabolized in a manner generally similar to that of arachidonic acid (AA) although some significant differences between the two are apparent. The metabolic fate of dietary EPA in human subjects is reviewed herein with inclusion of information from animal studies where human data is not available. The metabolism of EPA in the phospholipids of human platelets is emphasized to some extent. Effects of EPA on AA metabolism are also described.

Thrombotic microangiopathy (TM) occurred 14 to 27 months after last chemotherapy in two patients with diffuse large cell lymphoma and in one patient with Hodgkin's disease. Death from TM occurred in two patients who had no evidence of lymphoma at postmortem examination, and the lone survivor's disease is in complete clinical remission. All three patients had prominent arthritis. Occurrence of TM in non-Hodgkin's lymphoma has not previously been reported: its cause is uncertain; it may be to a late sequela of successful lymphoma chemotherapy.

The clinical studies of infusion schedules for cancer chemotherapy have established that this schedule substantially alters the pattern of drug toxicity without necessarily compromising therapeutic effectiveness. Although increased tumor cell killing has not been definitively established, in at least two prospective comparative trials of 5-FU the infusion schedule was superior to bolus delivery in terms of response rate. It is possible that the infusion schedule may be the primary determinant of the effectiveness of hepatic arterial chemotherapy, and preliminary reports of prospective trials comparing systemic infusion with hepatic infusion do suggest that response rates for both routes are similar. An important consideration with regard to infusion schedules is establishing whether or not tumor cell resistance is augmented by the continuous exposure. Almost all chronic disease therapy necessitates maintaining adequate drug levels to ensure disease control including infections, seizure disorders, arthritis, and cardiovascular diseases. For cancer, the intermittent schedule of drug delivery has been dictated by the concept of the primacy of dose and the substantial toxicity associated with these agents. With the technological capability of providing an ambulatory setting for infusion chemotherapy and the substantial modification of toxicity, therapeutic regimens may be more cost effective with the constant infusion schedule.

The authors studied data on psychiatric disorders and eight chronic medical conditions in a community sample of 2,554 persons. The sex- and age-adjusted prevalence of any psychiatric disorder in the preceding 6 months was 24.7% and of lifetime psychiatric disorder was 42.2% among persons with one or more medical conditions, compared to 17.5% and 33.0%, respectively, for persons with no medical condition. Persons with chronic medical conditions were more likely to have lifetime substance use disorders and recent affective and anxiety disorders. Arthritis, cancer, lung disease, neurological disorder, heart disease, and physical handicap were strongly associated with psychiatric disorders, but hypertension and diabetes were not.

A new era is emerging in the therapy of rheumatic disease involving the use of biologically based immunomodulators. Cytokines, such as the interleukins and interferons, are proteins produced by activated lymphoid cells which can now be synthesised by genetic engineering techniques. These agents have already been used to correct "immune deficiencies" in patients with neoplastic and inflammatory disease e.g. gamma-interferon in rheumatoid arthritis (RA). Monoclonal antibodies (mca) targeted against lymphocyte cell surface receptors or against autoantibodies have been successfully applied in human lymphoid malignancies, organ transplantation and in animal models of arthritis. Potential problems in man include the development of allergic reactions to mouse proteins and of antibody resistance. Although in its infancy, immunomodulation represents an exciting approach to the treatment of rheumatic disease.

1. Vasculitis syndromes in children present with special features. The hypersensitivity vasculitides predominate with Henoch-Schoenlein purpura as a typical representative. Within the polyarteritis-nodosa-group we can distinguish an infantile form with involvement of coronary vessels. This disease resembles Kawasaki's syndrome. 2. The different vasculitis syndromes show a great variety in their clinical presentation; overlapping syndromes are possible. Diagnosis, however, is mostly based on clinical features, supported by laboratory data. In doubtful cases histologic and immunohistologic findings can prove helpful. Their diagnostic value should only be interpreted together with the clinical picture. Since specific findings are often missing we have to exclude all similar diseases. Especially systemic juvenile chronic arthritis, connective tissue diseases, several infections and sometimes malignancies have to be considered. 3. For the future we expect further differentiation and identification of new disease entities, thus hoping for a better classification.
